Hypercalcemia in chronic myelogenous leukemia: evidence for excessive parathyroid hormone secretion.

Hypercalcemia was associated with osteolytic bone lesions in a 60-year-old woman with chronic myelogenous leukemia in the accelerated phase. Using highly specific antisera to parathyroid hormone, radioimmunoassays disclosed elevated levels of carboxyl-terminal (53-84) and intermediate (44-68) fragments. In addition, concomitant variations of serum calcium level and leukocyte counts, increased urinary c-AMP excretion, morphological integrity of parathyroid glands, and absence of bone resorbing activity in myeloblast culture supernatants are consistent with the hypothesis that the humoral hyperc…

Corticosteroid-responsive hypercalcemia with elevated serum 1-alpha, 25 dihydroxyvitamin D.

A 51-year-old man has had absorptive hypercalciuria and corticosteroid-responsive hypercalcemia for at least 12 years. There has been no clinical or laboratory proof of primary hyperparathyroidism, hypervitaminosis D, or other known causes of hypercalcemia and absorptive hypercalciuria. Hypercalciuria as well as the elevated serum level of calcium and 1 alpha, 25(OH)2D fell to normal during treatment with corticosteroids. The disturbed calcium metabolism in this patient is characteristic of that observed in sarcoidosis, but extensive studies have failed to uncover evidence of this condition.

IgG4-related disease presenting with hypercalcemia: case report and mechanistic insights.

Immunoglobulin G4-related disease (IgG4-RD) is a rare, multisystem disorder with diagnostic challenges. We report a case of IgG4-RD initially presenting with acute kidney injury (AKI) and hypercalcemia in a 61-year-old woman with a history of recurrent eyelid edema. On admission, serum creatinine and calcium were significantly elevated (414 μmol/L and 3.69 mmol/L, respectively). After excluding other etiologies, diagnosis was confirmed by renal biopsy and elevated serum IgG4 levels. Treatment with prednisone acetate and supportive care resulted in gradual clinical and laboratory improvement. T…

Atypical Calcitriol-Mediated Hypercalcemia in Late Life: A Hypothesis-Generating Case.

Calcitriol-mediated hypercalcemia is most commonly associated with granulomatous disease or lymphoma and rarely occurs without an identifiable cause. We present the case of a 96-year-old woman who developed severe symptomatic hypercalcemia with elevated 1,25-dihydroxyvitamin D (calcitriol), low 25-hydroxyvitamin D (calcidiol), suppressed parathyroid hormone, and no evidence of malignancy, granulomatous disease, endocrine dysfunction, or vitamin D intoxication. She improved with intravenous fluids, calcitonin, and corticosteroids. We hypothesize that age-related impairment of the fibroblast gro…

Immobility-induced hypercalcemia following complex lower limb trauma: A case report.

Immobilization-induced hypercalcemia is a rare but potentially life-threatening complication in patients with prolonged immobility, particularly in those with severe trauma. We describe a 22-year-old male polytrauma patient with a Gustilo-Anderson grade 3b open tibial-fibular fracture and traumatic brain injury. During prolonged recovery complicated by multiple surgical interventions, he developed moderate hypercalcemia two months post-injury. Common causes such as malignancy and primary hyperparathyroidism were excluded. Hypercalcemia was attributed to prolonged immobilization and successfull…

Ionized hypercalcemia in cats with azotemic chronic kidney disease (2012-2018).

BACKGROUND: Hypercalcemia is associated with chronic kidney disease (CKD) in cats, but studies assessing the physiologically relevant ionized calcium fraction are lacking. OBJECTIVES: To describe the prevalence and incidence rate of ionized hypercalcemia, and to explore predictor variables to identify cats at risk of ionized hypercalcemia in a cohort of cats diagnosed with azotemic CKD. ANIMALS: One hundred sixty-four client-owned cats with azotemic CKD. METHODS: Variables independently associated with ionized hypercalcemia at diagnosis of azotemic CKD were explored by binary logistic regre…

Toxic thyroid adenoma with hypercalcemia mimicking an intra-thyroidal parathyroid adenoma.

Hypercalcemia is a clinical condition characterized by elevated circulating serum calcium levels either due to raised parathyroid hormone in hyperparathyroidism or due to secondary causes of hypercalcemia without elevated parathyroid hormone levels. However, hyperthyroidism may occasionally present with incidentally detected hypercalcemia. We present a case of a 53-year-old woman with a previous history of an underlying thyroid disorder, now presented with features of hypercalcemia and mildly elevated parathyroid hormone levels. Her ultrasonography of the neck was suggestive of an intra-thyroi…

Hypercalcemia in IgG4-related disease: coincidental or associated? Case based review.

Immunoglobulin (Ig) G4-related disease (IgG4RD) is a chronic autoimmune disorder characterized by dense lymphoplasmacytic infiltrations and fibrosis of storiform pattern. The most typical manifestations include major salivary or lacrimal gland involvement, autoimmune pancreatitis, and retroperitoneal fibrosis. While the increase in IgG4 is the typical feature of the disease, hypercalcemia has been rarely reported in IgG4RD so far, only one of these cases has been shown parathyroid gland involvement (isolated involvement). In this study, we present a 43-year-old female patient with weight loss,…

Concomitant familial hypocalciuric hypercalcemia and single parathyroid adenoma: a case report.

BACKGROUND: Primary hyperparathyroidism (PHPT) is a common endocrine disorder and the most frequent benign cause of hypercalcemia. PHPT is characterized by autonomous hypersecretion of parathyroid hormone (PTH), regardless of serum calcium levels. Familial hypocalciuric hypercalcemia (FHH) is a rare, benign syndrome only affecting the regulation of calcium metabolism. FHH is an autosomal-dominant genetic disease with high penetrance, caused by an inactivating variant in the CASR gene encoding the calcium-sensing receptor (CaSR). We present a unique case of concomitant PHPT and FHH without clin…

Antacid-induced acute hypercalcemia: An increasingly common and potentially dangerous occurrence.

Hypercalcemia is frequently encountered in both hospital wards and the primary care setting; 90% of cases can be attributed to primary hyperparathyroidism and malignancy. However, a minority are caused by medications, of which calcium supplements have been an increasingly common etiology. We are presenting a case of hypercalcemia resulted after acute oral intake of a moderate amount of antacids (calcium tablets) and normalized after supplement withdrawal.

Oral cinacalcet responsiveness in non-parathyroid hormone mediated hypercalcemia of malignancy.

Hypercalcemia of malignancy develops in approximately 20-30% of patients with advanced cancer and is an ominous sign. This condition is subdivided into three categories: i) humoral hypercalcemia of malignancy (80% of cases), mediated by systemic parathyroid hormone-related protein; ii) osteolytic metastases (20% of cases), mediated by inflammatory cytokines locally released by tumor cells and/or peri-tumor macrophages; and iii) ectopic production of 1,25-dihydroxyvitamin D (<1% of cases), leading to intestinal hyperabsorption of calcium and increased osteoclastic bone resorption. Humoral hyper…

Phosphate matters when investigating hypercalcemia: a mutation in SLC34A3 causing HHRH.

SUMMARY: Hereditary hypophosphatemic rickets with hypercalciuria (HHRH) is a rare, autosomal recessive disorder caused by mutations in the SLC34A3 gene that encodes the renal sodium-dependent phosphate cotransporter 2c (NaPi-IIc). It may present as intermittent mild hypercalcemia which may attract initial diagnostic attention but appreciation of concomitant hypophosphatemia is critical for consideration of the necessary diagnostic approach. A 21-year-old woman was assessed by adult endocrinology for low bone mass. She initially presented age two with short stature, nephrocalcinosis and mild in…

[Acute pancreatitis due to hypercalcemia during pregnancy. Report of one case].

Acute pancreatitis during pregnancy is uncommon and usually associated with gallstones. However other etiologies must be considered. We report a 24 years old woman with a 32 weeks pregnancy consulting for abdominal pain, nausea and vomiting. She had elevated lipase and amylase levels, a corrected serum calcium of 13.1 mg/dl and a serum phosphate of 1.6 mg/dl. A magnetic resonance colangiopancreatography showed an enlarged pancreas with inflammatory changes and a normal Wirsung duct. A parathyroid nodule was found on cervical ultrasonography. The patient was treated initially with cinacalcet wi

Chronic Myeloid Leukemia Associated Hypercalcemia: A Case Report and Literature Review.

BACKGROUND Hypercalcemia associated with chronic myeloid leukemia (CML) is an ominous sign. Although rare, several cases have been reported and multiple pathophysiologic mechanisms have been independently proposed. We present a patient case and a literature review of the clinical presentation and mechanisms of CML-associated hypercalcemia. CASE REPORT A 58-year-old male with a past medical history of CML diagnosed six years earlier, presented to the emergency department with one week of acute confusion, disorientation, polyuria, and polydipsia. On physical examination, we observed tachycardia,…

HYPERCALCEMIA AND METASTATIC MINERALIZATION INVOLVING FOOTPADS IN ROCK HYRAXES (PROCAVIA CAPENSIS).

Three rock hyraxes (Procavia capensis) maintained in a zoological collection had chronic hypercalcemia and azotemia. In addition, all animals displayed signs of lameness due to footpad lesions that were histologically characterized as marked metastatic mineralization with granulomatous inflammation, reminiscent of calcinosis circumscripta. Although the animals were managed with aggressive fluid therapy, calciuresis, and dietary modification, all were eventually humanely euthanized due to the severity of their footpad lesions and/or progression of renal disease. Metastatic mineralization was al…
