Experimental hypercholesterolemia in rabbits induces cavernosal atherosclerosis with endothelial and smooth muscle cell dysfunction.

Hypercholesterolemia and other vascular risk factors for atherosclerosis are commonly associated with impotence. To characterize cavernosal smooth muscle reactivity in hypercholesterolemia, we performed isometric tension studies (with norepinephrine, acetylcholine, papaverine and electrical field stimulation) on isolated strips of corpus cavernosum from rabbits fed a 1% cholesterol diet. To assess the impact of cholesterol reduction, a group of rabbits was fed a cholesterol diet for 4 weeks and was then returned to a normal diet for 4 weeks before testing. Potential structure-function relation…

Efficacy and Safety of Enlicitide in Hypercholesterolemia: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Atherosclerotic cardiovascular disease remains a leading cause of global mortality, with low-density lipoprotein cholesterol (LDL-C) reduction being the cornerstone of prevention. Despite effective therapies, many high-risk patients do not achieve recommended LDL-C targets. Enlicitide (MK-0616), an oral proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor, offers a potential alternative to injectable PCSK9-directed therapies. OBJECTIVE: To evaluate the efficacy and safety of enlicitide in adults with hypercholesterolemia and assess its dose-dependent effects on lipid pa…

Optimized flow cytometry assay for functional characterization of variants of uncertain significance in familial hypercholesterolemia.

BACKGROUND: The definitive diagnosis of Familial Hypercholesterolemia (FH) relies on the identification of deleterious variants in FH-related genes, but many detected variants are classified as of uncertain significance (VUS). This study aims to evaluate the functional impact of LDLR VUS using an optimized flow cytometry assay in autologous activated CD4+ T lymphocytes. METHODS: Twenty unique LDLR variants were tested. Six known pathogenic variants served as positive controls, while T lymphocytes from normocholesterolemic individuals were used as negative controls. Peripheral blood mononuclea…

Gut microbiome taxonomic and predicted functional profiles in Tunisian individuals with hypercholesterolemia: a pilot study.

Hypercholesterolemia is a major cardiovascular risk factor that has been increasingly associated with alterations of the gut microbiota. However, microbial signatures remain inconsistent across populations, and data from North African populations are currently lacking. This study aimed to characterize gut microbiota alterations associated with hypercholesterolemia in a Tunisian cohort while integrating taxonomic and functional analyses. Fecal samples from 18 hypercholesterolemic patients and 17 normocholesterolemic controls were analyzed by 16S rRNA gene (V3-V4) sequencing. Taxonomic compositi…

MicroRNA modulation and crosstalk of SREBP and NF-κB pathways in herbal nanoparticle therapies for hypercholesterolemia.

Hypercholesterolemia is increasingly recognized as a multifactorial metabolic disorder characterized by dysregulated cholesterol homeostasis and chronic inflammation, both of which accelerate atherosclerosis and cardiovascular disease. MicroRNAs (miRNAs) have emerged as key post-transcriptional regulators linking sterol regulatory element-binding proteins (SREBPs) and nuclear factor-kappa B (NF-κB), thereby integrating lipid metabolism with inflammatory signaling. Herbal phytochemicals possess promising hypolipidemic and anti-inflammatory activities; however, their clinical application is limi…

Severe Hyperlipidemia and Multifocal Atherosclerosis With Negative Genetic Testing for Familial Hypercholesterolemia: A Case Report.

Familial hypercholesterolemia (FH) is a genetic disorder characterized by severely elevated cholesterol levels and increased cardiovascular risk. While typically associated with mutations in the low-density lipoprotein receptor (LDLR), apolipoprotein B (APOB), or proprotein convertase subtilisin/kexin type 9 (PCSK9) genes, many clinical cases remain genetically unexplained. Identifying novel variants in non-canonical genes is essential for understanding the complex genetic architecture of lipid disorders. A 75-year-old female patient presented with severe hyperlipidemia, multifocal atheroscler…

Lipid profile effects on donor and recipient of a domino liver transplant with heterozygous familial hypercholesterolemia.

BACKGROUND: Familial hypercholesterolemia (FH) is caused by genetic mutations in lipid metabolism that increase atherosclerotic cardiovascular disease (ASCVD) risk. OBJECTIVE: We wanted to assess lipid profile effects of liver transplant in i) a donor patient with heterozygous FH (HeFH), and ii) a HeFH recipient patient with colorectal adenocarcinoma with isolated liver metastasis. METHODS: A 34-year-old donor female with HeFH and intolerance to all lipid lowering therapies underwent living donor HeFH-negative liver transplant. Her HeFH liver was subsequently transplanted into a 49-year-old …

Assessment of CoQ10 Dietary Intake in a Mediterranean Cohort of Familial Hypercholesterolemia Patients: A Pilot Study.

BACKGROUND/OBJECTIVES: Coenzyme Q 10 is a lipid molecule that works as a mobile electron transporter in the mitochondrial respiratory chain and, in addition, plays the role of an antioxidant. Interestingly, CoQ10 synthesis in human cells derives from the mevalonate pathway, the same metabolic route that delivers endogenous cholesterol. Mutations leading to Familial Hypercholesterolemia (FH) alter the levels of CoQ10 production and remarkably, statin therapy associated muscular symptoms (SAMSs) might also be modulated by CoQ10 supplementation. CoQ10 is also provided by diet and only a few studi…
