Visual phenomenal consciousness: a neurological guided tour.

The scientific study of the cerebral substrate of consciousness has been marked by significant recent achievements, resulting partially from … lain how their generality has been demonstrated in healthy subjects using conditions such as visual illusions or subliminal perception. Finally, I describe the bases of a scientific model of consciousness on the basis of the concept of a "global workspace," which takes into account the data reviewed.

Potential drug development and therapeutic approaches for clinical intervention in COVID-19.

While the vaccination is now available to many countries and will slowly dissipate to others, effective therapeutics for COVID-19 is still illusive. The SARS-CoV-2 pandemic has posed an unprecedented challenge to researchers, scientists, and clinicians and affected the wellbeing of millions of people worldwide. Since the beginning of the pandemic, a multitude of existing anti-viral, antibiotic, antimalarial, and anticancer drugs have been tested, and some have shown potency in the treatment and management of COVID-19, albeit others failed to leave any positive impact and a few also became cont

Heritable Genome Editing: Who Speaks for "Future" Children?

Approximately 80% of rare and often incurable and serious conditions affect newborns and children, and roughly half of all rare diseases are considered to have an onset in childhood. Somatic gene therapies are already in clinical trials for spinal muscular atrophy, beta thalassemia, and macular degeneration. If proven to be safe and effective, could heritable genome editing be seen as a form of preventive personalized medicine and as fostering the right to health of the child? The latest calls for global moratoria on clinical applications of heritable genome editing are troubling in that they

Reversible palinacousis from intracranial metastases.

Palinacousis is an auditory illusion consisting of perseveration or echoing of an external auditory stimulus after it has ceased. This rare clinical symptom has been reported in ictal (seizure), postictal, and nonictal states, and causative lesions have been most consistently found in or near the temporal lobes. It is distinct from the auditory hallucinations seen in psychiatric illness. We report the case of a 61-year-old man who experienced several days of palinacousis while undergoing treatment for newly diagnosed metastatic lung adenocarcinoma. Palinacousis was presumed to be triggered by

Reversal of vision metamorphopsia caused by pons and cerebellum infarction.

Metamorphopsia is a visual illusion related to the perception of an object's shape, size, colour or angle. Reversal of vision metamorphopsia is a rare, transient form of metamorphopsia described as an inversion of the field of vision, usually a 180-degree reversion within the frontal plane. We describe the case of a 64-year-old male patient who first experienced a 90-degree rotation of the field of vision and then had the impression of his body rotating in space. The symptoms were preceded by disequilibrium, astigmatism and vomiting. Magnetic resonance imaging of the head showed focuses of vas

Trichotillomania and the mourning process: a case report and review of the psychodynamics.

We present our theory of trichotillomania (hair pulling) as a manic phenomenon, complementary to its classic comprehension as a mourning response. Under certain conditions, separation or threat of separation can generate sadness or grief, while in pathological cases, when the loss is experienced as unbearable, the response might be depressive or manic. We suggest that in trichotillomania (TR), like in mania, manic defenses are used to cope with separation. We believe hair pulling is an expression of primary magical thinking, offering the subject an illusion of control and victory over the obje

The hidden costs of imperfection: transcription errors in protein aggregation diseases.

At first glance, biological systems appear to operate with remarkable precision and order. Yet, closer examination reveals that this perfection is an illusion, biological processes are inherently prone to errors. Here, we describe recent evidence that indicates that errors that occur during transcription play an important role in neurological diseases. These errors, though transient, can have lasting consequences when they generate mutant proteins with amyloid or prion-like properties. Such proteins can seed aggregation cascades, converting wild-type counterparts into misfolded conformations,

Micropsia and Polyopia Following Multi-Focal Infarction: A Case Report.

Individuals with Alice in Wonderland syndrome can present with a wide variety of visual symptoms. Most commonly, visual disturbances in size perception such as micropsia or macropsia are observed. However, rarer disturbances such as the visual perception of multiple images, termed polyopia, as well as kinetopsia, a visual illusion in which stationary objects are perceived as moving, have also been described. Previous neuroimaging of different individuals with Alice in Wonderland syndrome has shown the involvement of topographically separate brain regions. Here, we describe an individual who se

The place of Free Will: the freedom of the prisoner.

Debates about the concept of Free Will date back to ancient times. About 40 years ago, Benjamin Libet designed an experiment showing that the conscious intention to move is preceded by a specific pattern of brain activation. His finding suggested that unconscious processes determine our decisions. Libet-style experiments have continued to dominate the debate about Free Will, pushing some authors to argue that the existence of Free Will is a mere illusion. We believe that this dispute is because we often measure Free Will using arbitrary human decisions rather than deliberate actions. After rev

Masking of Enamel Fluorosis Discolorations and Tooth Misalignment With a Combination of At-Home Whitening, Resin Infiltration, and Direct Composite Restorations.

This clinical report illustrates a conservative technique to mask enamel discolorations in maxillary anterior teeth caused by hypomineralization associated with enamel fluorosis and subsequent direct resin composite to improve the anterior esthetics. The treatment consisted of at-home whitening with 10% carbamide peroxide gel with potassium nitrate and sodium fluoride in a custom-fitted tray to mask the brown-stained areas, followed by resin infiltration to mask the white spot areas. An existing resin composite restoration in the maxillary right central incisor was subsequently replaced after

An approach to vertigo in general practice.

BACKGROUND: Dizziness is a common and very distressing presentation in general practice. In more than half of these cases, the dizziness is due to vertigo, which is the illusion of movement of the body or its surroundings. It can have central or peripheral causes, and determining the cause can be difficult. OBJECTIVE: The aim of this article is to provide a clear framework for approaching patients who present with vertigo. A suggested approach to the assessment of vertigo is outlined. DISCUSSION: The causes of vertigo may be central (involving the brainstem or cerebellum) or peripheral (invo

[Specifics of interactions of mono- and binocular mechanisms of spatial perception in cecutient children].

Interactions of mono- and binocular mechanisms of spatial perception were estimated using stereokinetic test proposed by L.N. Mogilev in 117 children with low vision and 317 healthy children aged 5-18 years old. Cecutient and healthy children had similar values of steriokinetic illusion in monocular visual conditions, while in binocular visual conditions the results were significantly different. It was shown that in children with low vision there is no increase in contribution of binocular mechanisms with aging. The mean value of coefficient K characterizing this contribution in healthy group

Risk factors for problematic gambling: a critical literature review.

This article is a critical review of risk factors for pathological gambling categorized by demographics, physiological and biological factors, cognitive distortions, comorbidity and concurrent symptoms, and personality symptoms and characteristics. There is also a varia section (availability, parents playing, sensory characteristics, schedules of reinforcement, age of onset, and playing duration). The review found very few well established risk factors for pathological gambling (i.e. more than two studies to support the conclusions). Well established risk factors included demographic variables

An event-related potential investigation of the time-course of temporal ventriloquism.

Temporal ventriloquism refers to the phenomenon that a sound presented in close temporal proximity of a visual stimulus attracts its perceived temporal occurrence. Here, we investigate the time-course of the neuronal processes underlying temporal ventriloquism, using event-related brain potentials. To measure shifts in perceived temporal visual occurrence, we used a paradigm in which a sound modulates the magnitude of a visual illusion called the flash-lag effect. A sound presented before the flash reduced both the size of the flash-lag effect and the amplitude of visual N1 compared with when

Diagnosis and management of vertigo.

Vertigo is an illusion of rotation due to a disorder of the vestibular system, almost always peripheral. In the history it must be distinguished from pre-syncope, seizures and panic attacks. A single attack of acute, isolated spontaneous vertigo lasting a day or more is due either to vestibular neuritis or cerebellar infarction; distinguishing between the two requires mastery of the head impulse test. Recurrent vertigo is mostly due to benign paroxysmal positioning vertigo (BPPV), Meniere's disease or migraine. With a good history, a positional test, an audiogram and a caloric test, it is usua

Spatial consequences of bridging the saccadic gap.

We report six experiments suggesting that conscious perception is actively redrafted to take account of events both before and after the event that is reported. When observers saccade to a stationary object they overestimate its duration, as if the brain were filling in the saccadic gap with the post-saccadic image. We first demonstrate that this illusion holds for moving objects, implying that the perception of time, velocity, and distance traveled become discrepant. We then show that this discrepancy is partially resolved up to 500 ms after a saccade: the perceived offset position of a post-

[Informing the transfused patient of the possible transmission of variant Creutzfeldt-Jakob disease by blood transfusion].

UNLABELLED: TRUE RISKS AND THEORETICAL RISKS: The texts ruling the obligation of the physician to inform the patient appears not to include theoretical risks in their application. In France however, the field of blood transfusions extends this obligation to the theoretical risk of potential transmission through the blood of the infectious agent responsible for Creutzfeldt-Jakob's disease. UNANSWERED QUESTIONS: From an ethical point of view, is information on the theoretical risk of transmission founded? From a formal point of view, the means used for such an extension (i.e.,a bulletin) is deb

A new instrument to measure cognitive distortions in video lottery terminal users: the Informational Biases Scale (IBS).

This paper reports on the development of a new scale, the Informational Biases Scale (IBS), to measure cognitive distortions such as the illusion of control, gambler's fallacy, illusory correlations, and the availability heuristic in video lottery terminal (VLT) players. Ninety-six VLT players recruited from bars in New Brunswick took part in the study. Their average (lifetime) South Oaks Gambling Screen score was in the probable pathological gambler range. The 25-item IBS was shown to have good internal consistency reliability. An exploratory principal components/factor analysis revealed the v
