Behçet Disease: New Developments in the Etiopathogenesis of an Old Silk Road Disease.

Behçet disease (BD) is a systemic inflammatory disorder that affects the skin, mucosa, eyes, joints, blood vessels, brain, and gastrointestinal tract. The etiopathogenesis of BD has not yet been fully elucidated, but disorganized immune responses against the stimuli of environmental triggering factors have been considered to play a major role in the pathogenesis of the disease in individuals with genetic susceptibility. Human leukocyte antigen (HLA)-B*51 is known to be the main factor involved in genetic susceptibility to BD. Among the environmental factors, infectious agents in particular are

Etiopathogenesis of Behçet's disease with emphasis on the role of immunological aberrations.

Behçet's disease (BD) is a chronic multisystemic inflammatory disorder of unknown origin consisting of oral aphthous ulcers, ocular symptoms, skin lesions, and genital ulcerations. It has many features in common with systemic vasculitides and is more prevalent in countries along the ancient Silk route. Immune-mediated mechanisms play a major role in the pathogenesis of the disease, and inflammatory mediators are also involved. BD is not considered to be an autoimmune disorder, and the character of the disease needs to be clarified. Immunological aberrations in BD have been extensively studied

How undifferentiated arthritis evolves into chronic arthritis.

Undifferentiated arthritis (UA) is a frequently occurring clinical presentation with a variable outcome. While some forms of UA will spontaneously remit, other forms will progress to chronic arthritis; an outcome that would preferably be prevented. Which immunological factors are normally at the basis of resolution of inflammation, and what, on the other hand, causes inflammation to persist? This review provides an overview of the immunological mechanisms involved in these two scenarios, including specific examples of how these mechanisms apply, or can be influenced in rheumatic diseases. Furt

Transfer factor therapy in the Wiskott-Aldrich syndrome. Results of long-term follow-up in 32 patients.

Thirty-two patients with the Wiskott-Aldrich syndrome have been treated with transfer factor provided by this laboratory. Apparent clinical benefit was observed in 44 per cent of them. The mean age of the patients who showed clinical benefit was significantly greater than that of the patients who showed no benefit. Conversion of immunologic reactivity correlated with clinical benefit. Thirteen of the patients who received transfer factor are alive, and 17 have died (43 per cent survival). Clinical benefit was correlated with survival. The median survival was greater than five years in the pati

Impact on weight and height with the use of HAART in HIV-infected children.

BACKGROUND: There are few data on long-term effects of highly active antiretroviral therapy (HAART) on weight and height in HIV-infected children. OBJECTIVES: Our objective was to assess the effect of HAART on the weight and height of HIV-infected children over time in the Madrid cohort, and analyze possible factors associated with the effect. PATIENTS AND METHODS: This was a retrospective study of HIV-infected children starting HAART in 1997 or later. Serial measurements of weight, height and body mass index (BMI) were performed and converted to z-scores using the Spanish revised reference

Evaluation of triggers for corneal graft rejection.

BACKGROUND AND OBJECTIVE: To evaluate potential triggering factors contributing to corneal graft rejection. PATIENTS AND METHODS: A prospective, case control study was conducted over a five month period. All patients presenting with new onset corneal graft rejection were enrolled into the study group. Two patients with clear grafts who presented shortly after each enrolled study group patient served as controls. Groups were matched for age, preoperative diagnosis, and number of previous keratoplasties. Participating patients and examining ophthalmologists filled out questionnaires regarding i

[Immunological aspects of Menière's disease].

Among the various aetiopathogenetic hypotheses regarding Ménière's disease, the immunologic one has recently received a considerable amount of interest. Several studies have been performed using specific inner ear antigens (cell-mediated immunity) or by the evolution of circulating autoantibodies (i.e. against type II collagen or immune complex, etc...). We studied the immunological condition of 40 patients suffering from Ménière's disease, through the analysis of the following immunological blood tests: IgG, IgA, IgM, C3c, C4 and C-haemolytic complement level; circulating immune complexes, au

[Bone marrow graft in hereditary diseases].

Since 1968, bone marrow transplantation became the first line therapy for selected metabolic and immunological hereditary disorders. Actually, advances in the supportive care in bone marrow transplantation and a better knowledge of the immunology of BMT complications has been associated with a better disease correction and an increase in long term survival. New approaches are under investigation and include: hematopoietic growth factors, enzymatic replacement and gene therapy. However at the present time BMT is still the only curative treatment for selected hereditary disorders.

The immunological landscape of primary biliary cholangitis: Mechanisms and therapeutic prospects.

Primary biliary cholangitis (PBC) is a chronic cholestatic liver disease characterized by the progressive destruction of intrahepatic bile ducts, leading to fibrosis, and potentially cirrhosis. PBC has been considered a prototypical autoimmune condition, given the presence of specific autoantibodies and the immune response against well-defined mitochondrial autoantigens. Further evidence supports the interaction of immunogenetic and environmental factors in the etiology of PBC. An immunological attack on biliary epithelial cells with secondary failure of biliary transporters, eg, the anion exc

The role of "bone immunological niche" for a new pathogenetic paradigm of osteoporosis.

Osteoporosis is characterized by low bone mass and microarchitectural deterioration of bone tissue. The etiology and pathogenetic mechanisms of osteoporosis have not been clearly elucidated. Osteoporosis is linked to bone resorption by the activation of the osteoclastogenic process. The breakdown of homeostasis among pro- and antiosteoclastogenic cells causes unbalanced bone remodeling. The complex interactions among these cells in the bone microenvironment involve several mediators and proinflammatory pathways. Thus, we may consider the bone microenvironment as a complex system in which local

The monocyte-macrophage system in granulomatous inflammation.

Granulomas may be immunologically induced or non-immunologically induced. In immunologically induced granulomas cells of the monocyte-macrophage series take on the appearance of epitheloid cells. Ultrastructurally epithelioid cells may have a secretory appearance with much rough endoplasmic reticulum or take on highly degenerate vesicular appearance. Other epithelioid cells look like activated macrophages. Secretory epithelioid cells may be found associated with acute local inflammation as in borderline tuberculoid leprosy in reaction, the lepromin reaction, following injection of BCG vaccine

[Hashimoto's thyroiditis, the model of organ-specific autoimmune disorders].

Anti-thyroid antibodies were published first in this disease. Research of past fifty years provided evidence that the damage of thyrocytes was the result of a multistep process in which immunological, immunogenetic, epigenetic and environmental factors had been involved. It was clarified that the autoimmune thyroiditis was not identical with hypothyroidism since during this disease different functional stages were differentiated. Both the clinical picture and the immunological investigations have been proved to be important in the diagnosis. The author summarized the main principles for treatm

Infection as a stressor: a cytokine-mediated activation of the hypothalamo-pituitary-adrenal axis?

Infections are associated with increased plasma concentrations of adrenocorticotropic hormone (ACTH) and corticosterone. Hypothalamo-pituitary-adrenal (HPA) responses have also been observed with immunological stimuli that are not infective. Although such responses have been suggested to be mediated by ACTH secreted by lymphocytes, adrenocortical activation by immunological stimuli requires a functional pituitary. The most likely mechanism by which immunological stimuli activate the HPA axis involves production of cytokines by lymphocytes. The prime candidate is interleukin 1 (IL-1), because I

Body composition radiomics combined with machine learning for early recurrence prediction in intrahepatic cholangiocarcinoma following curative surgery: A Multi-Center study.

PURPOSE: Early recurrence (ER) of intrahepatic cholangiocarcinoma (ICC) after curative hepatectomy correlates with dismal prognosis. We hypothesized that body composition radiomics reflecting systemic metabolic-immunologic status could enhance ER prediction. This multi-center study aimed to develop and validate integrated radiomics-clinical machine learning (RCML) models for postoperative ER risk stratification. METHODS: In this retrospective study, 258 ICC patients (2011-2022) from three institutions who underwent curative resection were enrolled. Body composition features were extracted fro

Limiting the gamble: Risk and predictability for renal replacement therapy in patients receiving mechanical circulatory support★.

BACKGROUND: Patients receiving mechanical circulatory support (MCS) frequently require renal replacement therapy (RRT). Examining risk factors for requiring RRT in patients receiving MCS may allow improved understanding of these comorbidities and enhance patient outcomes. METHODS: Following IRB approval, patient characteristics, comorbidities, and the need for RRT were studied in 129 patients who received MCS from January 2017 to October 2023. The clinical variables underwent machine learning to examine their relationships to the outcome of interest, the need for RRT. RESULTS: In this study,

Septic shock in the immunocompromised cancer patient: a narrative review.

Immunosuppressed patients, particularly those with cancer, represent a momentous and increasing portion of the population, especially as cancer incidence rises with population growth and aging. These patients are at a heightened risk of developing severe infections, including sepsis and septic shock, due to multiple immunologic defects such as neutropenia, lymphopenia, and T and B-cell impairment. The diverse and complex nature of these immunologic profiles, compounded by the concomitant use of immunosuppressive therapies (e.g., corticosteroids, cytotoxic drugs, and immunotherapy), superimpose

Standardized, risk-adapted induction therapy in kidney transplantation.

BACKGROUND: The choice of induction therapy in kidney transplantation is often non-standardized and centre-specific. Clinicians can choose between depleting and non-depleting antibodies, which differ in their immunosuppressive capacity and the concomitant risk of infection. We herein present a standardized risk-stratified algorithm for induction therapy that might help to balance the risk of rejection and/or serious infection. METHODS: Prior to kidney transplantation, patients were stratified into low-risk, intermediate-risk or high-risk according to our protocol based on immunologic risk fac
