Case report of symptomatic porphyria cutanea tarda associated with histiocytic lymphoma.

A fifth case is reported of symptomatic porphyria cutanea tarda (PCT) associated with lymphoma (of the histiocytic type) in a 45-year-old female. The PCT was documented in detail histologically and biochemically. The lymphoma was complicated by two paraneoplastic phenomena: inappropriate antidiuretic hormone secretion and peripheral neuropathy. It is possible that the PCT might also be a paraneoplastic phenomenon of the lymphoma. Search for an occult lymphoma may be of diagnostic and therapeutic value in patients presenting with PCT.

Addison's disease, psychosis, and the syndrome of inappropriate secretion of antidiuretic hormone.

A case of tuberculous Addison's disease presenting with psychosis, profound hyponatraemia, and detectable plasma antidiuretic hormone is reported. Clinical and biochemical improvement after corticosteroid replacement was followed by relapse with further psychosis and inappropriate antidiuretic hormone secretion: both were promptly reversed by demethylchlortetracycline. The association of psychological symptoms with Addison's disease, the role of anti-diuretic hormone secretion in Addison's disease, and the inter-relationship between Addison's disease, psychosis and anti-diuretic hormone secret

Intermittent fluid retention in women. Is it idiopathic edema?

When a capillary or renal disturbance is not apparent in a woman of reproductive age who presents with edema, a disorder referred to as idiopathic edema should be considered. This disorder, which is often intermittent, causes fluid to accumulate when the patient is upright. Because of this postural relationship, research into pathogenesis has focused on homeostatic adjustments to changes in posture. A number of mechanisms have been proposed, the most tenable of which seem to be increased capillary permeability, increased mineralocorticoid activity, and a faulty "escape mechanism." The diagnosi

[EEG changes in a patient with acute intermittent porphyria and a Schwartz-Bartter syndrome (SIADH)].

A 24-year-old female with gastrointestinal disturbances, nausea and vomiting, had a convulsion with loss of urine and bitten lips on the 5th day of hospitalization. A significant decrease of sodium and potassium levels and lowered osmolality of the serum as well as urinary hyperosmolality permitted the diagnosis of the so-called syndrome of inappropriate antidiuretic hormone release (SIADH) of unknown aetiology, described by Schwartz-Bartter. Twice short tests for porphyria were negative; then the elevated porphyrin precursors collected in 24 h urine indicated the existence of an acute intermi

Fluid management in Haemophilus influenzae meningitis.

The fluid management of 50 children with Haemohpilus influenzae type B meningitis was reviewed. Clinical hydration status on admission, serum sodium values, and overall fluid balance was assessed to determine the contribution of empiric fluid restriction in preventing the development of syndrome of inappropriate antidiuretic hormone (SIADH). Thirty-three of 50 patients were well hydrated on admission. Sixteen of 50 patients (32%) initially had signs of dehydration and five out of 16 were in shock. Only two patients had evidence of SIADH. Twenty patients were empirically fluid restricted, inclu

SIADH and Lung Small Cell Carcinoma: A Clinical Case.

Hyponatremia is the most common electrolyte disorder found in clinical practice. One of the causes is the syndrome of inappropriate antidiuretic hormone secretion (SIADH). SIADH is characterized by hypotonic and euvolemic hyponatremia with elevated urinary osmolarity and increased ADH secretion without a triggering stimulus. This pathology is often associated with neoplastic conditions, namely lung neoplasms. We report the case of a 54-year-old woman admitted with a history of vertigo and uncontrollable nausea with hyponatremia of 100meq/L at admission and diagnosed with paraneoplastic SIADH s

[Symptomatic therapy of SIADH in small cell lung cancer by tolvaptan].

HISTORY AND CLINICAL FINDINGS: A 69-year-old woman with small cell lung cancer presented in the emergency ward because of severe back pain. She had received tolvaptan treatment in hospital for paraneoplastic SIADH but had discontinued the drug after discharge from hospital 3 days ago. Restriction of fluid intake was not tolerated. INVESTIGATIONS: Consistent with SIADH, there were profound hyponatraemia, elevated urine osmolality and urine sodium. TREATMENT AND COURSE: After a generalized seizure triggered by hyponatraemia, tolvaptan was reintroduced in addition to radiochemotherapy. Serum so

Endocrine failure after traumatic brain injury in adults.

OBJECTIVE: To review histopathological and clinical data linking endocrine failure to traumatic brain injury (TBI) during acute neurosurgical treatment and rehabilitation. METHODS: A focused search of the Medline (PubMed) medical literature database and the authors' files were used to identify selected publications. RESULTS: Endocrine failure may produce clinically important consequences during acute and convalescent care after TBI, and may be caused by direct injury to the hypothalamic- pituitary axis (HPA), neuroendocrinological effects from catecholamines and cytokines, or from systemic i

[Association of hyponatremia and eosinophilia: correlated idiopathic hypereosinophilic syndrome and SIADH or adrenal insufficiency with secondary eosinophilia].

A 56 year old man was admitted cause he had increasing symptoms as weakness, lethargy, disorientation. The total eosinophil count was 3000/mm3, the serum sodium concentration was 120 mmol per litre. In spite of severe hyponatriemia, urinary sodium excretion was not suppressed and serum osmolality (240 mOsm/Kg was lower than urine osmolality (488 mOsm/Kg). SIADH and Idiopathic Hypereosinophilic Syndrome was diagnosed because we found systemic failure signs due to hypereosinophilia (hepatitis, gastritis, pulmonary hypertension, and encefalopathy). Cortisonic treatment was started with symptoms i

Transient diabetes insipidus following Escherichia coli meningitis complicated by ventriculoperitoneal shunt.

Although disorders of ADH secretion associated with meningitis are usually consistent with the Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH), central diabetes insipidus (DI) is an exceptional complication of meningitis. Transient DI as a complication of Escherichia coli (E. coli) meningitis due to ventriculoperitoneal shunt in an 18-month-old boy is presented. Blood and spinal fluid cultures yielded E. coli, sensitive to cefotaxime. The DI arose on the day 3 after admission and continued to the day 20. Treatment comprised cefotaxime, dexamethasone, fluid adjustment and vasop

Correlation of parameters of urinary excretion with serum osmolality among patients with psychosis, intermittent hyponatremia, and polydipsia (PIP syndrome).

Nine patients (seven men and two women, mean age 36.3 +/- SD 6.7 years), six of whom had schizophrenic disorders, two of whom had bipolar disorder (manic-depressive illness), and one of whom had schizoaffective disorder, manifested psychosis, intermittent hyponatremia, and polydipsia (PIP syndrome). Their stable pattern of hyposthenuria allowed us to predict 24-hr urinary volume on the basis of estimated daily urinary creatinine and early morning urinary creatinine concentration. Lithium and carbamazepine (Tegretol) had little, if any, effect on polyuria. Correlations of parameters of urinary

[Electrolyte disorders in oncological patients].

Electrolyte disorders (ED) are common in patients with cancer and in most cases, the etiologies do not differ from the general population. They may also be induced by the cancer, its therapy or paraneoplastic syndromes. ED are associated with poor outcomes, increased morbidity and mortality in this population. Hyponatremia is the most common disorder, often multifactorial, iatrogenic or secondary to the syndrome of inappropriate antidiuretic hormone secretion, usually due to small cell lung cancer. More rarely, hyponatremia may reveal adrenal insufficiency. Hypokalemia is generally multifactor

[Endocrine encephalopathies].

Endocrine encephalopathy, excepting hypoglycaemia and diabetes mellitus, is usually the clinical expression of non specific neuronal suffering secondary to a severe endocrine disturbance. The presentation is sometimes atypical and the prognosis poor but reversible with appropriate treatment; the condition should be recognised as the diagnosis can be made by simple investigations.

Syndrome of Inappropriate Antidiuretic Hormone Secretion and Trimethoprim-Related Hyponatremia Following Transurethral Bladder Wall Biopsy.

Postoperative hyponatremia is common. It results from the physiological antidiuretic hormone (ADH) response to surgery and associated exacerbating factors. Common exacerbating factors include medications, excessive fluid administration, and syndrome of inappropriate antidiuretic hormone (SIADH) secretion. High-dose trimethoprim can cause transient salt-losing nephropathy, resulting in SIADH-like hyponatremia associated with hypovolemia, hyperkalemia, and metabolic acidosis. We present a patient who developed symptoms of vomiting, loss of appetite, fatigue, and abdominal discomfort six days aft

[Hyponatremia in cirrhosis with ascites].

Hyponatremia complicates ascitic hepatic cirrhosis with frequency and gravity related to the gravity of the cirrhosis itself. When hyponatremia develops, it worsens the already present secondary hyperaldosteronism and makes therapy with spironolactone inefficacious. From a pathophysiologic viewpoint a pathogenetic role in determining hyponatremia is attributable to the reduced plasmatic renal perfusion; in several patients a syndrome of inappropriate ADH secretion develops. Other neurohormonal systems (catecholamines, prostaglandins, natriuretic hormones) are probably very important in modifyi

Comparison of Voets and Adrogué-Madias equations in children with syndrome of inappropriate antidiuresis receiving 3% saline.

BACKGROUND: Syndrome of inappropriate antidiuresis (SIAD) is a common cause of euvolaemic hyponatraemia. Moderate-to-severe hyponatraemia, particularly in the neurosurgical setting, often necessitates administration of correctional fluids, typically hypertonic saline. However, predicting changes in plasma sodium levels following fluid administration is challenging and often inaccurate. In adults, the Voets equation has been shown to be superior to the classical Adrogué-Madias formula in predicting sodium levels achieved with correctional fluids, however, no studies have investigated its effica

Validation of ICD-10 codes used to identify the main reason for hospitalization due to hyponatremia in Sweden.

PURPOSE: Hyponatremia is the most common electrolyte disorder, with potentially serious consequences. Studies validating hyponatremia used as a principal diagnosis are lacking. Studying principal diagnoses is important since it indicates the main reason for inpatient care. The aim of this study was to assess the validity of principal diagnoses of hyponatremia according to the International Classification of Diseases, tenth revision (ICD-10) in patients discharged from hospital. METHODS: Three independent reviewers retrospectively analyzed medical records of 428 hospitalized patients, ≥18 year

Cerebral Salt Wasting Syndrome in an Elderly Patient With Traumatic Brain Injury: Diagnostic Challenges. A case report.

OBJECTIVE: Cerebral salt-wasting syndrome (CSWS) is an underdiagnosed cause of hyponatremia following intracranial injuries. This case highlights the diagnostic challenges of CSWS in elderly patients, particularly when typical volume depletion signs are absent. CASE: We report an 80-year-old male with multiple comorbidities who developed CSWS following motor vehicle accident-related subarachnoid hemorrhage. The patient was confused, with a Glasgow Coma Scale (GCS) score ranging from eight to ten. Serum sodium declined significantly from 145 to 117 mmol/L approximately 20 days. The patient rem

Refractory SIAD Triggered by Herpes Zoster Ophthalmicus: Impact of Postherpetic Neuralgia on Hyponatremia Duration and the Role of Tolvaptan.

An 86-year-old man developed syndrome of inappropriate antidiuresis (SIAD) following herpes zoster ophthalmicus (HZO). The patient presented with symptomatic hyponatremia and impaired consciousness. Hypertonic and isotonic saline infusions improved both parameters; however, oral sodium supplementation and fluid restriction alone failed to maintain normonatremia. Tolvaptan was added, resulting in temporary correction of sodium levels; however, SIAD relapsed after discontinuation. Ultimately, SIAD resolved with improvement in neuropathic pain. This case highlights a therapeutic strategy for symp
