Obesity is a clinical condition characterized by weight gain and a progressively increasing prevalence that poses a significant threat to human health. It may also predispose individuals to numerous chronic diseases. Physical limitations such as musculoskeletal problems (e.g., osteoarthritis and low back pain), spinal cord injuries, hemiplegia, and other comorbidities both increase the development of obesity and make exercise participation more difficult in obese patients. This review summarizes current evidence regarding exercise approaches used in obese individuals with physical limitations

OBJECTIVES: In individuals with normal hearing, tinnitus may be the initial manifestation of cochleovestibular lesions. Tinnitus is a multifactorial symptom arising from complex interactions within the auditory pathway and related neural networks. Anatomically, the auditory and vestibular system is one unit and functions as an integrated whole; therefore, tinnitus should be approached not only from the auditory perspective but also from the vestibular perspective, in particular, the otolith organs. This study aimed to assess the sacculo-collic, utriculo-ocular, and vestibulo-masseteric reflex

OBJECTIVE: Define interaural pitch matching in individuals with suspected endolymphatic hydrops (sus-EH) and evaluate the stability of interaural pitch comparisons over time.
METHODS: Sixteen adults with clinically diagnosed Ménière's disease or otherwise suspected EH (10 unilateral, 6 bilateral; aged 32-65 years) and 14 normal-hearing controls (aged 20-53 years) completed an adaptive binaural pitch-comparison task at three low-to-mid frequencies (≤1500 Hz). Using a two-alternative forced-choice paradigm, participants indicated whether sequential tones presented to each ear were higher or low

BACKGROUND: Tuberculosis (TB) is a significant cause of mortality among people living with HIV (PLWH), in Nepal. HIV increases the risk of progression from latent to active TB, increasing mortality. Despite established TB control programs, there is a significant lack of recent data measuring the tuberculosis burden among PLWH specially within the Western region of Nepal. This study was conducted to assess the prevalence of pulmonary tuberculosis among HIV-positive patients attending a tertiary care centre in Gandaki Province, Nepal.
METHODS: A hospital-based cross-sectional study was conducte

Depression is a mental disorder associated primarily with mood and cognitive symptoms. Several studies show that depression is additionally accompanied by decrements in gross and fine motor functioning. The present study examined to what extent depression affects the ability to adapt sensorimotor behavior. A total of 20 participants with depression and 22 controls performed a manual adaptation task in which they had to adapt joystick movements to a visual perturbation. We assessed the rate of adaptation following the introduction of the visual perturbation (both for early and later stages of a

IMPORTANCE: As on-axis metrics, spherical equivalent refraction (SER) and axial length (AL) are limited in capturing individual-level differences in posterior segment anatomy.
OBJECTIVE: To propose a fundus-level metric-fundus refraction offset (FRO)-and investigate its association with ocular parameters derived from optical coherence tomography (OCT).
DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional, population-based study used data from 45 180 healthy eyes in the UK Biobank (2009-2010). Fundus photographs from a random subset (70%) were used to train a deep learning model to predict

Despite its critical function in the immune system and accumulating evidence of immunological abnormalities in schizophrenia, the thymus has long been overlooked. We aimed to investigate thymic morphological alterations and their corresponding heterogeneity in patients with schizophrenia. Imaging-derived thymic morphology was assessed and compared between 419 patients with schizophrenia and 460 age- and sex-matched control participants aged 16-40 years who underwent chest computed tomography (CT) scanning. These included measurements reflecting thymic size and density, such as average maximal

Hypoparathyroidism is the inability of parathyroid hormone (PTH) to maintain calcium homeostasis. Patients with post-surgical hypoparathyroidism may have an increased risk of mortality; there is clinical and molecular evidence of the effects of this condition on the cardiovascular system. The aim of this study was to evaluate arterial stiffness by measuring the carotid-femoral pulse wave velocity (PWV) in post-surgical hypoparathyroidism patients. A cross-sectional study was conducted with 30 post-surgical hypoparathyroidism patients and 25 volunteers from the Endocrinology Outpatient Clinic o

Categorization is an essential task for sensory perception. Individuals learn category labels using a variety of strategies to ensure that sensory signals, such as sounds or images, can be assigned to proper categories. Categories are often learned on the basis of extreme examples, and the boundary between categories can differ among individuals. The trajectories for learning also differ among individuals, as different individuals rely on different strategies, such as repeating or alternating choices. However, little is understood about the relationship between individual learning trajectories

BACKGROUND AND OBJECTIVES: Approximately 11 million people in the United States self-identify as Deaf and use American Sign Language (ASL) as their primary form of communication. Yet, little is known about the challenges and solutions in everyday activities of individuals who are aging with long-term deafness.
RESEARCH DESIGN AND METHODS: We used a community-engaged research approach to understand everyday challenges and solutions of 60 older ASL users who self-identified as Deaf. Semi-structured interviews were conducted in ASL to probe challenges and responses to those challenges across six

INTRODUCTION: Schizophrenia recovery is perceived as a one-time symptom remission. However, it is a process that may continue throughout an individual's life. Understanding the recovery experience of a person with schizophrenia is an essential component of filling the mental health service gap and enhancing a smooth recovery process. The objective of this study was to explore the recovery experience of individuals with schizophrenia in a developing country.
METHODS: To understand these experiences, we used qualitative research. Seventeen participants were purposefully selected from two hospit

The aging prison population in the United States presents urgent challenges for providing appropriate end-of-life (EOL) care. Currently, there are two primary approaches to deliver this in a compassionate manner: medical release programs (MRPs) and prison-based end-of-life care programs (EOLCPs). Using New York State as a case study, we argue expanded MRPs represent the most ethical path forward, while acknowledging the continuing need for prison-based EOLCPs. Our analysis reveals significant barriers to accessing medical release, including lack of knowledge about the application processes, pr

Sarcopenia is a known risk factor for cardiovascular disease (CVD) in individuals with diabetes or prediabetes, but the impact of changes in sarcopenia status on CVD risk remains unclear. This study aimed to examine how changes in sarcopenia status between baseline and the second follow-up survey, conducted 2 years later, influence the risk of developing incident CVD. Incident CVD was identified based on self-reported physician diagnoses of heart disease, such as angina, myocardial infarction, heart failure, or stroke. Cox proportional hazard models were used to estimate hazard ratios (HRs) an
