Sharp thinking.

Nurses are the healthcare professionals most at risk from neeldestick and sharps injuries, according to the latest figures from the RCN's EPINet project. During 2004, 1,990 injuries were reported from the 20 self-selecting participating sites. Nurses sustained 43 per cent of these injuries. Disposable syringes and needles were the most common equipment to cause injury. Although discouraged, 4 per cent of injuries occurred during recapping of needles. Just under a third of injuries were sustained by staff other than the original user of the sharp.

Patient and family physician preferences for care and communication in the eventuality of anthrax terrorism.

BACKGROUND: The threat of bioterrorism consequent to the September 11, 2001 attack in the USA generated suggestions for improved medical response mainly through hospital preparedness. OBJECTIVES: The aim of the present study was to investigate the impact of this period of tension on patients' first choice for care and for receiving relevant information, and on primary care doctors' feelings of responsibility in the eventuality of an anthrax attack. METHODS: During October 11-31, 2001, 500 patients from 30 clinics throughout Israel were asked to complete a questionnaire on their awareness of

The influence of genetic counselling in the era of DNA testing on knowledge, reproductive intentions and psychological wellbeing.

Subjects of reproductive age at risk of having an affected child with a severe single gene disorder such as Duchenne muscular dystrophy (DMD) or cystic fibrosis (CF) were surveyed to ascertain: their views on genetic counselling and antenatal testing; their knowledge of their risk of having an affected child; and their psychological wellbeing. Questionnaires were posted to 209 individuals at 130 addresses; a 65% response rate was achieved. The majority of those surveyed were under 40 years of age (91%), half of them had received genetic counselling only once and for 47% the first encounter was

Medicine, market and communication: ethical considerations in regard to persuasive communication in direct-to-consumer genetic testing services.

BACKGROUND: Commercial genetic testing offered over the internet, known as direct-to-consumer genetic testing (DTC GT), currently is under ethical attack. A common critique aims at the limited validation of the tests as well as the risk of psycho-social stress or adaption of incorrect behavior by users triggered by misleading health information. Here, we examine in detail the specific role of advertising communication of DTC GT companies from a medical ethical perspective. Our argumentative analysis departs from the starting point that DTC GT operates at the intersection of two different conte

High coverage of vitamin A supplementation and measles vaccination during an integrated Maternal and Child Health Week in Sierra Leone.

BACKGROUND: In May 2012, the twice-yearly Maternal and Child Health Week (MCHW) integrated vitamin A supplementation (VAS) and supplementary measles vaccination to reach all children 6-59 months in Sierra Leone. Following the MCHW, a post event coverage survey was conducted to validate VAS coverage and assess adverse events following immunization. METHODS: Using the WHO Expanded Program on Immunization sampling methodology, 30 clusters were randomly selected using population proportionate to size sampling. Fourteen caregivers of children 6-59 months were interviewed per cluster for precision

Dissemination, adoption, and implementation of acamprosate for treating alcohol use disorders.

OBJECTIVE: Acamprosate has been available in the United States for treating alcohol use disorders (AUDs) for nearly a decade, yet few studies have examined its use within AUD treatment organizations. In addition to describing dissemination and adoption of acamprosate, this study provides novel data regarding organizational processes that underlie its implementation within adopting programs. METHOD: Data were drawn from interviews with leaders of a nationally representative sample of 307 organizations delivering AUD treatment. Quantitative indicators of organizational characteristics, dissemin

Joint analysis of binary and quantitative traits with data sharing and outcome-dependent sampling.

We study the analysis of a joint association between a genetic marker with both binary (case-control) and quantitative (continuous) traits, where the quantitative trait values are only available for the cases due to data sharing and outcome-dependent sampling. Data sharing becomes common in genetic association studies, and the outcome-dependent sampling is the consequence of data sharing, under which a phenotype of interest is not measured for some subgroup. The trend test (or Pearson's test) and F-test are often, respectively, used to analyze the binary and quantitative traits. Because of the

Impact on hospital functions following the 2010 Chilean earthquake.

OBJECTIVE: The objective of this study was to assess the impact of the 2010 Chilean earthquake on hospital functions and services. Hospitals functioning in a post-disaster environment must provide emergency medical care related to the event, in addition to providing standard community health services. This study focused on damage to both structural and nonstructural components, as well as to utility services. METHODS: Site visits were made to every hospital in a single province (Bio-Bio). Engineers conducted damage assessments while interviews of hospital administrators were conducted. The su

Physicians' knowledge and opinions about medication abortion in four Latin American and Caribbean region countries.

To examine physicians' knowledge and attitudes in regard to medication abortion, we conducted focus-group discussions with general practice physicians and obstetrician-gynecologists in Honduras, Mexico, Nicaragua and Puerto Rico. Physicians were familiar with the practice of several types of medication and surgical abortion methods. Medication abortion with misoprostol is most common among women of higher socioeconomic status and is prescribed by physicians, pharmacists or self-administered. Conflicting opinions regarding safety, efficacy, cost, potential for self-medication and acceptability

The dissemination of 17D yellow fever vaccine in Africans in Kenya in relation to the interpretation of results of protection-test surveys.

The extent of dissemination of 17D yellow fever vaccine among the population of Kenya, especially among Africans, is estimated, largely from information in the records of the Kenya Medical Department. The total recorded vaccinations of Africans between 1941 and 1951 amount to about 379,000, representing 7.2% of the African population in 1948; it is, however, possible that the actual number of vaccinations is considerably higher. At Mombasa, 78,000 persons of races other than Africans were given routine inoculation over the same period. After a study of the economy of use of vaccine, the author

Patient empowerment via 'pushed' delivery of Personalised Healthcare educational content over the internet.

We present an Internet-based Personalised Healthcare Information (PHI) dissemination system. Information personalisation is guided by the individual's current health profile as recorded in his/her EMR. A PHI package is composed by intelligently selecting and synthesizing various topic-specific documents, each corresponding to some health parameter noted in the EMR. To ensure medical consistency, constraint satisfaction techniques are employed during the information selection phase. The resultant PHI package--covering both long-term and immediate health-maintenance requirements--can be pro-acti

Getting clinical trial results into practice: design, implementation, and process evaluation of the ALLHAT Dissemination Project.

BACKGROUND: Conventional dissemination of clinical trial results has inconsistent impact on physician practices. A more comprehensive plan to influence determinants of prescribing practices is warranted. PURPOSE: To report the response from the Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial to the National Heart, Lung, and Blood Institute's requirement for dissemination and evaluation of trials with potential immediate public health applicability. METHODS: ALLHAT's dissemination plan had two-components: (1) a traditional approach of media coverage, scientific pre

Effect of family history on disclosure patterns of cystic fibrosis carrier status.

As general population screening becomes more common, an increasing number of cystic fibrosis (CF) carriers will be identified who do not have a family history of CF. Whether these carriers inform their relatives of their carrier status and whether their relatives are motivated to pursue carrier screening is unknown. We surveyed CF carriers with and without a family history of CF to understand whether and how information dissemination patterns differ, why information is or is not shared, and to what extent relatives are known to undergo testing. CF carriers were identified from a general popula…

Using a think-aloud methodology to understand online physical activity information search experiences and preferences of parents of children and youth with disabilities.

BACKGROUND: The Internet is a preferred source of physical activity (PA) information. However, limited research exists regarding the experiences of parents of children and youth with disabilities (CYWD) in searching for PA programme information online. This research examined the experiences and preferences of parents of CYWD in searching for PA programme information online. METHOD: Parents of CYWD (n = 10) participated in a think-aloud exercise while searching for PA programme information online. Following the think-aloud exercise, semi-structured interviews were used to further understand pa

Consulting 'Dr YouTube': a content analysis of YouTube® videos related to hidradenitis suppurativa treatments.

YouTube® is a powerful resource for the dissemination of health information to the general public. We assessed the quality, understandability and evidence-based content of YouTube videos discussing hidradenitis suppurativa treatments, categorized by source: dermatologists, other healthcare professionals, or patients. We demonstrated patient videos were the most popular by viewers, but had the worst ratings for quality and understandability of information presented. Moreover, patient videos were the least likely to recommend the evidence-based treatments. Moving forward, there should be a partn

Development of a national survey on foot involvement among people with psoriatic arthritis in Australia using a best practice approach: a survey development protocol.

BACKGROUND: Limited research to date has defined the nature and extent of foot involvement in a psoriatic arthritis-specific population in Australia and the scale of the problem remains unclear. Survey research provides the ideal opportunity to sample a large population over a wide geographical area. Although quality criteria for survey research have been developed, research shows that adherence is low and that survey studies are poorly reported in peer-reviewed survey articles, which limits the ability to inform future survey design. The objective of this paper was to develop a national surve
