Inhaled Treprostinil for Idiopathic Pulmonary Fibrosis.

BACKGROUND: Preclinical data indicate that inhaled treprostinil may be useful for the treatment of idiopathic pulmonary fibrosis (IPF) through an antifibrotic mechanism, a premise that is supported by clinical observation. METHODS: In this phase 3, double-blind trial, we randomly assigned patients with IPF to receive inhaled treprostinil or placebo (12 breaths four times daily) over a period of 52 weeks. The primary end point was the change from baseline in the absolute forced vital capacity (FVC) at week 52. Secondary end points, which were analyzed in a prespecified order to control for mul

Adrenal suppression secondary to inhaled corticosteroids.

Inhaled corticosteroids (ICS) are the foundation of treatment for pediatric asthma, and ICS are significantly safer than repeated courses of systemic steroids. However, systemic effects from ICS include adrenal suppression (AS), which is under recognized and can be life-threatening. Signs and symptoms of AS are often subtle and can be difficult to recognize. The Canadian Pediatric Society and Adrenal Suppression Working Group recommend screening for AS in: asymptomatic children receiving high dose ICS therapy for ≥3 months, any dose ICS in conjunction with a CYP3A4 inhibitor, and any children

Selecting an Inhaled Delivery System in COPD.

Challenges facing health care professionals (HCPs) in selecting an inhaled medication delivery system for patients with COPD include (1) num … factors for selecting an inhaled delivery system include cognitive function, manual dexterity, and inhalation ability. Continuity of the type of handheld inhaler can be beneficial if the patient is using it correctly. To assess the efficacy of the inhaled medication-device at follow-up, HCPs can ask the patient, "Does your current treatment help your breath …

Inhaled budesonide and pulmonary sarcoidosis revisited.

Results of a few controlled clinical studies have been reported with inhaled corticosteroids (ICS) in patients with pulmonary sarcoidosis. Some evidence of efficacy has been observed, but mainly with the ICS budesonide (BUD). These clinically important and statistically significant results are restricted to maintenance therapy with BUD after induction of treatment with systemic corticosteroids for a few weeks or months.  Positive results have also been described in patients with relapses after earlier treatment with oral corticosteroids. A possible explanation for BUD's efficacy in patients wi

[COPD inhaled therapy narrative review].

The basis of COPD maintenance treatment is the long-acting bronchodilators and the inhaled corticosteroids. Faced with the recent modifications in the clinical practice guidelines, we have carried out a review of studies that contrast the various therapeutic alternatives and pharmacological agents within each category, with the fundamental purpose of shedding light on which of these options prove to be more effective. Triple therapy stands out as essential in poorly controlled patients or with an eosinophilic phenotype, surpassing dual therapy. However, among the combinations of LAMA/LABA or L

Inhaled heparin: Past, present, and future.

While heparin has traditionally served as a key anticoagulant in clinical practice for nearly a century, recent years have witnessed a growing interest in its role as a potent antiinflammatory and antiviral agent, as well as an anticancer agent. To address challenges with injection-based delivery, exploring patient-friendly routes such as oral and pulmonary delivery is crucial. This review specifically highlights the multiple therapeutic benefits of inhaled heparin. In summary, this review serves as a valuable source of information, providing deep insights into the diverse therapeutic advantag

Bronchoconstriction with inhaled ATP in healthy volunteers.

Inhaled ATP is a potent bronchoconstrictor, capable of inducing significant bronchoconstriction even in some healthy volunteers. Pre- and post-challenge spirometry, and pulse oximetry monitoring should be considered in studies utilising ATP inhalation. https://bit.ly/4dkpvWe

Use of mucolytics and inhaled antibiotics in the NICU.

Clearance of airway secretions and treatment of respiratory tract infections (RTIs) are two common problems caregivers face in the neonatal intensive care unit (NICU). Mucolytics degrade crosslinks in mucus gel, reducing mucus viscosity and facilitating their removal by cough or endotracheal suctioning. While such medications have been studied in older children and adults, their use is not as well described in the NICU. For RTIs, systemic antibiotics are usually prescribed, although their use is often associated with adverse effects. Inhaled antibiotics may provide increased drug concentration

Inhalable nanocatalytic therapeutics for viral pneumonia.

Pneumonia is a ubiquitous disease caused by viral and bacterial infections, characterized by high levels of reactive oxygen species in inflamed areas. Therapeutic strategies targeting reactive oxygen species levels in pneumonia have limited success due to the intricate nature of lung tissues and lung inflammatory responses. Here we describe an inhalable, non-invasive therapeutic platform composed of engineered cerium-based tannic acid nanozymes bound to a self-assembling peptide. In vitro and in vivo studies show that the nanozyme is internalized mostly by activated macrophages and epithelial

Inhaled corticosteroids for the treatment of COVID-19.

BACKGROUND: Inhaled corticosteroids are well established for the long-term treatment of inflammatory respiratory diseases such as asthma or chronic obstructive pulmonary disease. They have been investigated for the treatment of coronavirus disease 2019 (COVID-19). The anti-inflammatory action of inhaled corticosteroids might have the potential to reduce the risk of severe illness resulting from hyperinflammation in COVID-19. OBJECTIVES: To assess whether inhaled corticosteroids are effective and safe in the treatment of COVID-19; and to maintain the currency of the evidence, using a living sy

Laryngeal inhalational injuries: A systematic review.

Laryngeal inhalation injury carries a significant increase in mortality rate and often indicates immediate airway evaluation. This may be difficult in the setting of clinical deterioration necessitating immediate intubation, which itself can synergistically cause mucosal damage. Prior studies do not encompass predictive factors or long-term outcomes for the laryngotracheal complex. This systemic review of PubMed, Embase, and Cochrane identified studies investigating inhalational injuries of the upper airway. Demographic data as well as presentation, physical findings, and delayed sequelae were…
