Clinical Outcomes Following the Use of Constrained Condylar Knees in Primary Total Knee Arthroplasty.

BACKGROUND: Certain clinical or adverse intraoperative situations require the use of increased constraint in primary total knee arthroplasty (TKA). These include significant angular deformities causing incompetent collateral ligaments, or inadvertent intraoperative injury to collateral structures as well as the inability to achieve a balanced flexion and extension gap. Clinical success has been described with the use of constrained condylar knee arthroplasty in the primary setting in these situations. Traditionally, increasing constraint has been in conjunction with intramedullary stems, refer…

Association of frontal plane knee alignment with foot posture in patients with medial knee osteoarthritis.

BACKGROUND: To examine the association of radiographic frontal plane knee alignment with three-dimensional foot posture in patients with medial knee osteoarthritis (OA). METHODS: Participants in orthopedic clinics with Kellgren/Lawrence (K/L) grade ≥1 (88 patients and 88 knees; age, 61-91 years; 65.9% female) were enrolled. An anteroposterior radiographic view was used to assess the anatomical axis angle (AAA) after subtracting a sex-specific correction factor. The three-dimensional foot posture was also evaluated. RESULTS: Multiple regression analyses showed that increased corrected AAA (i.…

Total knee arthroplasty in valgus knees using minimally invasive medial-subvastus approach.

BACKGROUND: An ideal approach for valgus knees must provide adequate exposure with minimal complications due to approach per se. Median parapatellar approach is most commonly used approach in TKA including valgus knees. A medial subvastus approach is seldom used for valgus knees and has definite advantages of maintaining extensor mechanism integrity and minimal effect on patellar tracking. The present study was conducted to evaluate outcomes of total knee arthroplasty (TKA) and efficacy of subvastus approach in valgus knees in terms of early functional recovery, limb alignment and complication…

Sagittal kinematics of mobile unicompartmental knee replacement in anterior cruciate ligament deficient knees.

BACKGROUND: There is a greater risk of tibial component loosening when mobile unicompartmental knee replacement is performed in anterior cruciate ligament deficient knees. We previously reported on a cohort of anterior cruciate ligament deficient patients (n=46) who had undergone surgery, but no difference was found in implant survivorship at a mean 5-year follow-up. The purpose of this study was to examine the kinematic behaviour of a subcohort of these patients. METHODS: The kinematic behaviour of anterior cruciate deficient knees (n=16) after mobile unicompartmental knee replacement was co…

Development of a Modern Knee Society Radiographic Evaluation System and Methodology for Total Knee Arthroplasty.

To accompany the new clinical Knee Society Score, a committee was formed to develop an updated radiographic assessment and evaluation system. The purpose is to accumulate radiographic data in a standardized manner to facilitate more accurate interpretation, documentation and clinical correlation. We systematically reviewed the TKA radiographic evaluation literature as well as the original Knee Society Radiographic Evaluation and Scoring System. A modern system was developed, approved by the Knee Society membership, which ensured proper radiographic documentation of coronal and sagittal implant…

Impact of systematic ultrasound of the knee on the rheumatologist's clinical decision in patients consulting for knee pain.

This clinical practice study aimed to determine whether the results of systematic US in patients with knee pain modified the rheumatologist's choices concerning diagnostic management and therapy. Patients consulting for non-traumatic knee pain, with recent radiography of the knee, were consecutively included over 9 months. After the radio-clinical assessment, the rheumatologist made a principal diagnosis concerning the knee pain and defined the therapeutic management and a complementary imaging strategy if necessary. US of the painful knee was then done in accordance with the reference protoco…

Posterolateral reconstruction of the knee using capsular procedures for evaluation and treatment of posterolateral instability of the knee.

Posterolateral rotatory instability of the knee causes significant morbidity if unrecognized and left untreated. With both acute and chronic injuries, restoration of the normal anatomy is crucial for a good outcome. During capsular repairs, detailed knowledge of the anatomy will help the surgeon repair the pathoanatomy. To complement the direct approach, an osteotomy of the lateral femoral condyle that includes the attachments of the fibular collateral ligament and popliteal tendon allows superior visualization of the lateral meniscus and its attachments, as well as the associated deep structu…

The effect of knee extensor open kinetic chain resistance training in the ACL-injured knee.

PURPOSE: To investigate the effect of different loads of knee extensor open kinetic chain resistance training on anterior knee laxity and function in the ACL-injured (ACLI) knee. METHODS: Fifty-eight ACLI subjects were randomised to one of three (12-week duration) training groups. The STAND group trained according to a standardised rehabilitation protocol. Subjects in the LOW and HIGH group trained as did the STAND group but with the addition of seated knee extensor open kinetic chain resistance training at loads of 2 sets of 20 repetition maximum (RM) and 20 sets of 2RM, respectively. Anteri…

A new approach to measure functional stability of the knee based on changes in knee axis orientation.

There is a lack of measures that quantify functional knee stability, which is of particular relevance in knee rehabilitation. Therefore, the aim of this study was to investigate the usefulness of knee finite helical axis (FHA) variables in 33 healthy subjects during two different functional tasks; One leg side hop (SH) and Two Leg Squat (TLS), and to investigate correlations of these variables with laxity. Laxity was assessed with a KT-1000 arthrometer and the Beighton Hypermobility Score. Movements were registered with an optical motion capture system. Knee rotation and translation were defin…

The minimally invasive far medial subvastus approach for total knee arthroplasty in valgus knees.

PURPOSE: The lateral approach in the valgus knee asks for a lot of soft tissue releases during the arthrotomy. The hypothesis of this study was that the far medial subvastus approach could be used in valgus knees and would guarantee both functional and radiological good to excellent results. METHODS: This is a retrospective study on 78 patients (84 knees) undergoing primary total knee arthroplasty (TKA) for type I or II fixed valgus knees. The mean (SD) preoperative mechanical alignment was 187° (4°) HKA angle. Functional recovery, pain, tourniquet times, necessary soft tissue releases as wel…

Validation of the Korean version of the Oxford Knee Score in patients undergoing total knee arthroplasty.

BACKGROUND: Although translated versions of the Oxford Knee Score (OKS) in several languages are available, the absence of a Korean version precludes comparing data from Korea with that from other countries using the OKS. QUESTIONS/PURPOSES: We therefore evaluated the reliability and validity of the adapted Korean version of the OKS. METHODS: We first translated the English version of the OKS into Korean, then back into English, then held expert committee discussions to finalize the Korean version. We then mailed the Korean version of the VAS for pain, OKS, and the previously validated SF-36…

Examination of knee joint moments on the function of knee-ankle-foot orthoses during walking.

The goal of this study was to investigate clinically relevant biomechanical conditions relating to the setup and alignment of knee-ankle-foot orthoses and the influence of these conditions on knee extension moments and orthotic stance control during gait. Knee moments were collected using an instrumented gait laboratory and concurrently a load transducer embedded at the knee-ankle-foot orthosis knee joint of four individuals with poliomyelitis. We found that knee extension moments were not typically produced in late stance-phase of gait. Adding a dorsiflexion stop at the orthotic ankle signifi…

Self-reported knee malalignment in early adult life as an independent risk for knee chondrocalcinosis.

OBJECTIVE: To determine if self-reported current and early adult life knee malalignment is associated with knee chondrocalcinosis (CC). METHODS: A case-control study embedded in the Genetics of Osteoarthritis and Lifestyle (GOAL) study was performed. A validated self-reported line-drawing instrument was mailed to 3,022 participants of GOAL, inquiring about knee malalignment (straight, valgus, varus) in their third decade (20s) and currently. Self-reported weight in 20s and present weight and height measured at the initial visit were used to calculate body mass index (BMI) in 20s and currently…

Navigation-assisted total knee arthroplasty in knees with osteoarthritis due to extra-articular deformity.

PURPOSE: Extra-articular post-traumatic deformity may make difficult the implantation of total knee arthroplasty (TKA). Staged surgical procedures, including femoral or tibial osteotomy, can be required to restore proper alignment. These procedures may be inappropriate because of high rate of complications. Intra-articular resection is an alternative procedure, but it is limited by the potential compromise of collateral knee ligaments. Conventional instrumentation cannot be used in patients with previous trauma and residual bone deformity. We want to assess whether computer-assisted surgery ma…

Osteoarthritis knee with patellar fracture in the elderly: single-stage fixation and total knee arthroplasty.

Periarticular fractures that occur in a previously osteoarthritic knee, especially in an elderly patient, will be associated with other comorbidities and is a treatment challenge. Early mobilization is a key treatment principle in the geriatric population. We present a case report of an elderly diabetic patient with osteoarthritis of the knee joint who sustained a patellar fracture and was treated with single-stage primary total knee arthroplasty and fixation of the patellar fracture.

Influence of knee joint angle on muscle properties of paralyzed and nonparalyzed human knee extensors.

Muscles of individuals with a spinal cord injury (SCI) exhibit an unexpected leftward shift in the force (torque)-frequency relationship. We investigated whether differences in torque-angle relationships between SCI and able-bodied control muscles could explain this shift. Electrically stimulated knee-extensor contractions were obtained at knee flexion angles of between 30 degrees and 90 degrees. Torque-frequency relationships were obtained at 30 degrees, 90 degrees, and optimum angle. Optimum angle was not different between groups but SCI-normalized torques were lower at the extreme angles. A…

Changes in knee moments with contralateral versus ipsilateral cane usage in females with knee osteoarthritis.

BACKGROUND: Conservative treatment for osteoarthritis often involves educating the patient in methods of decreasing the load transmitted through the diseased joint. The use of a cane is one such method and the correct placement of the cane with respect to an abnormal knee joint is crucial. The purpose of this study was to compare effects on knee moments of force of contralateral versus ipsilateral cane usage in female subjects with osteoarthritic knees. METHODS: A convenience sample of 14 subjects volunteered for this study. Subjects walked over force platforms while ground reaction force and…

A broken sewing needle in the knee of a 4-year-old child: is it really inside the knee?

We report on a case of a broken needle that migrated inside the knee joint of a 4-year-old girl. Searching for any small foreign body in the knee joint is not easy in either open or arthroscopic procedures. In this case, the surgery was made more difficult because of technical delays and diagnostic difficulties in defining the surgical plane of the needle. Arthroscopic expertise and some basic precautions can minimize the morbidity to a young patient and prevent migration into the knee joint proper.
