Resurfacing of the patella.

The results of use of twenty-nine patellar resurfacing prostheses in twenty-eight patients were studied. The diagnosis was osteoarthritis in twenty-two knees, chondromalacia in five knees, and habitual dislocation with severe osteoarthritis in two. The follow-up time was from three to six years. A cemented chromium-cobalt prosthesis was used. Of the twenty-nine knees studied, two were rated excellent; fourteen, good; three, fair; and ten, poor. Failure was always due to unrelieved pain.

An analysis of the medial patellofemoral ligament length change pattern using open-MRI.

Recently, for a recurrent patellar dislocation, medial patellofemoral ligament (MPFL) reconstruction has come to be commonly performed, yet the precise in vivo MPFL biomechanics still remains to be clarified. The purpose of this study was to analyze the length change pattern of MPFL in vivo. The subjects were comprised of ten male and ten female healthy volunteers. The length of the MPFL was measured using an open-MRI both from a leg extended position and a highly knee-flexed position in order to analyze the length change pattern of the MPFL. The MPFL length for men and women resulted in an av

A modified Hauser procedure for recurrent dislocation of the patella. A long-term follow-up study with special reference to osteoarthritis.

A modified Hauser operation was used for the treatment of recurrent dislocation of the patella. Thirty-seven patients (40 knees) were traced for the purpose of long-term follow-up with regard to subjective and objective results and the degree of osteoarthritis in the patellofemoral joint. The mean follow-up time was 18 years. Only 12% of the patients had objectively satisfactory results, but 48% were satisfied with their operation. There was no correlation between the opinion of the patients and the objective findings. Dislocation recurred in 20% of the knees. More than two-thirds of the patie

Dislocation of the proximal tibiofibular joint, do not miss it.

We present a case of a 45-year-old woman with a right proximal tibiofibular dislocation she sustained after a fall during roller skating. Anteroposterior and lateral radiographs confirmed the diagnosis; there were no other injuries. The dislocation was reduced by direct manipulation after intra-articular infiltration, in our emergency department. The patient was treated with a long, non-weight bearing leg cast for 1 week. After 4 weeks, she had no pain and a full range of motion of the knee.

Medial dislocation of the medial meniscus.

We present the first reported case of symptomatic medial dislocation of the medial meniscus in a patient who had no previous history of trauma and who had an otherwise normal knee. The treatment of instability of the medial meniscus is controversial and studies have indicated that certain individuals without a firm meniscal bony insertion may be predisposed to meniscal dislocation. In our patient, the meniscal instability interfered with daily activities. Operative stabilisation by reconstruction of the meniscotibial ligaments cured the symptoms.

Reconstruction of the medial patello-femoral and patello-tibial ligaments for treatment of patellar instability.

PURPOSE: The importance of the medial patello-femoral (MPFL) and medial patello-tibial ligaments (MPTL) to the stability of the patella is undoubted. The purpose of this work was to present a technique for the reconstruction of both ligaments and prospectively record its outcome. METHODS: Cadaveric part: the MPFL and MPTL were identified in five knee specimens. Sequential cutting and reattaching of these ligaments were performed to study their effect on patellar stability. CLINICAL PART: In the years between 2005 and 2008, twenty-five knees in twenty-one patients had reconstruction of their M

[The value of arthroscopy following traumatic patellar dislocation].

Traumatic dislocations of the patella and associated osteochondral fractures can be easily diagnosed by arthroscopy. During 1984-1986 acute arthroscopy was performed in 280 patients with traumatic haemarthrosis of the knee joint. In 26 cases the intraarticular bleeding was caused by patellar dislocation. Most of these injuries occurred during sports activities. Arthroscopic surgery gave excellent results in most of the cases. Arthrotomy had to be performed in 3 patients only.

Trochleoplasty as a Solitary Treatment for Recurrent Patellar Dislocation Results in Good Clinical Outcome in Adolescents.

BACKGROUND: The essential static patellar stabilizer is a normal-shaped trochlear groove. A dysplastic groove destabilizes the patella. Trochleoplasty approaches this underlying condition and reshapes the trochlea. However, studies have reported on trochleoplasty for revision cases or as accompanied by other interventions. The effect of trochleoplasty alone remains unexplained. PURPOSE: To introduce trochleoplasty as a stand-alone treatment for recurrent patellar dislocation and to compare its pre- to postoperative functional and clinical variables. STUDY DESIGN: Case series; Level of eviden

The Number of Patellar Dislocation Events Is Associated With Increased Chondral Damage of the Trochlea.

BACKGROUND: Patellofemoral instability is associated with chondral injuries to the patella, trochlea, and lateral femoral condyle. Although studies have demonstrated an association between patellar dislocations and chondral injuries, the influence of the number of dislocations on chondrosis is not established. PURPOSE: To elucidate the precise association between the number of patellar dislocation events and the severity of chondral injuries in a multicenter cohort study at the time of patellar stabilization procedures. STUDY DESIGN: Cross-sectional study; Level of evidence, 2. METHODS: A p

Rocket launcher: A novel reduction technique for posterior hip dislocations and review of current literature.

OBJECTIVE: We provide a review of literature on reduction techniques for posterior hip dislocations and present our experience with a novel technique for the reduction of acute posterior hip dislocations in the ED, 'the rocket launcher' technique. METHODS: We present our results with six patients with prosthetic posterior hip dislocation treated in our rural ED. We recorded patient demographics. The technique involves placing the patient's knee over the shoulder, and holding the lower leg like a 'Rocket Launcher' allow the physician's shoulder to work as a fulcrum, in an ergonomically friendl

Sudden peroneal nerve palsy in an osteoarthritic knee: a case report.

Peroneal nerve injuries have been reported in association with various causes around the knee such as traumatic varus injury, traumatic dislocation, upper tibial osteotomy, knee arthroscopy and total knee arthroplasty. Two instances of varus arthritic knee associated with a peroneal nerve palsy have been reported so far. One presented with gradual onset peroneal nerve palsy that recovered with time and the other with sudden onset peroneal nerve palsy that did not recover. We describe the case of a 63-year-old man who presented with a symptomatic varus arthritic knee and sudden onset peroneal n

Total Knee Arthroplasty in Severe Unstable Knee: Case-Report and Literature Review.

Multiplanar or global laxity in arthritic knee is rare, most of this patients have neuromuscular disorder (post poliomyelitis, spinal dystrophy) or history of knee trauma. Ligament insufficiency and severe bone loss is significant in this patient. The estimated prevalence for the concurrence of charcot marie-tooth (CMT) with myasthenia gravis (MG) suggests an extremely rare event. We have presented a 54-year-old female patient with CMT and MG complaining of progressive pain, swelling, and crepitation of the knee joints who had been undergone total knee arthroplasty (TKA) with rotating hinge pr

Neurologic Assessment and Management of the Multiple Ligament Injured Knee: A Review and Synthesis of Current Evidence.

Knee dislocations are rare but devastating injuries that can often be associated with concomitant nerve injury, most often of the common peroneal nerve. Prompt diagnosis, investigation, and appropriate treatment are important in this subset of patients. This article provides an overview of relevant injury demographics, anatomy, diagnosis, and prognosis, and suggests a treatment algorithm for nerve injury associated with multiligament knee injury.

A pre-operative grade 3 J-sign adversely affects short-term clinical outcome and is more likely to yield MPFL residual graft laxity in recurrent patellar dislocation.

PURPOSE: The purposes of this study were to investigate the anatomical risk factors of grade 3 J-sign and analyse the influence of J-sign grades on the short-term clinical outcomes of patients with recurrent patellar dislocation. METHODS: From 2016 to 2018, a total of 168 patients with recurrent patellar dislocation (187 knees) underwent medial patellofemoral ligament reconstruction with or without tibial tuberosity medialization. Pre-operative J-sign severity was graded according to a previously described classification (grades 1-3). Potential anatomical risk factors of J-sign were explored.

Recurrent dislocation of the patella. Treatment by a modification of the method of McCarroll & Schwartzmann.

In a retrospective examination of 40 knees operated on according to a modification of the method of McCarroll & Schwartzmann, results ranging from good to excellent were seen in 68 per cent of the cases. Poor results partly attributable to synovitis and chondromalacia, were found in 20 per cent. Twelve per cent had recurrence of the dislocation. There was very little correlation between the preoperative X-ray and the final results. Open epithyseal plates were found to be a contraindication for the operation. The duration of the condition and the number of dislocations had no bearing on the end

Patellofemoral instability in skeletally immature athletes.

Acute patellofemoral dislocation is the most common acute knee disorder in children and adolescents. The predisposing factors for acute patellar dislocation are multifactorial. Unless associated with substantial articular cartilage damage, nonsurgical management is typically used to treat a first-time acute patellofemoral dislocation in a skeletally immature athlete. In the setting of recurrent instability, surgical reconstruction is usually recommended. Surgical treatment of patellofemoral instability in the skeletally immature athlete is evolving from nonanatomic extensor mechanism surgical

Complications of posterolateral corner injuries of the knee and how to avoid them.

Knee dislocations and in particular posterolateral corner injuries to the knee are severe injuries demanding an organized approach and technically challenging surgery. Complications include unrecognized vascular or neurological injury and failure to reconstruct appropriately. During reconstruction, performing an inappropriate operation (failure to recognize malalignment) or technical error with tunnel or hardware placement can lead to delayed problems. Wound infection and wound breakdown is common as in arthrofibrosis often from overconstraint of the knee. Attention to principles and expertise…
