Acute alcoholic pancreatitis does not lead to complete recovery.

The aim of the study was to determine the late outcome of acute alcoholic pancreatitis (a.a.p.), as assessed by clinical examination, functi … on tests and imaging techniques, in about one third of patients even a moderate attack of a.a.p may lead to chronic pancreatitis. We suggest that patients after acute alcoholic pancreatitis should undergo prospective evaluation including both the function and structure estimation in order to early recognize and treat the revealed changes.

New concepts regarding events that lead to myocardial infarction.

In addition to new knowledge concerning the mechanisms whereby conventional risk factors act, other risk factors have been newly described, such as dietary antioxidants, lack of exercise, insulin resistance, excess iron stores, increased plasma angiotensin-converting enzyme, and left ventricular hypertrophy. An intact endothelium protects both by the formation of nitric oxide, which is a vasodilator and also an inhibitor of platelet aggregation and neutrophil adhesion, and by manufacturing tissue plasminogen activator. The acute thrombotic event occurs with a diurnal variation but may be preci

Elevated environmental lead levels in a day care setting.

OBJECTIVE: To determine the risk of lead poisoning among children enrolled in day care centers with elevated environmental lead burdens. DESIGN: Survey. SETTING: Six day care centers on properties owned by a major state-supported university. PATIENTS AND OTHER PARTICIPANTS: One hundred fifty-five of 234 eligible children (mean age, 4.8 years) enrolled in these centers were screened by questionnaire for risk factors of lead exposures. Blood samples for lead levels were also obtained. Observations of day care activities relative to lead exposure risks were recorded. Analyses of lead levels in…

Mortality and cancer incidence among secondary lead smelter workers.

OBJECTIVES: To examine the mortality pattern and the cancer incidence in a cohort of long term smelter workers exposed to lead. METHODS: The cohort consists of 664 male lead battery workers, employed for at least three months in 1942-87. From 1969 the values of all blood lead samples repeatedly obtained from these workers every two to three months, have been collected in a database. The expected mortality and morbidity 1969-89 was estimated from the county rates, specified for cause, sex, five-year age groups, and calendar year. Individual exposure matrices have been calculated and used for d…

Lead poisoning from mobilization of bone stores during thyrotoxicosis.

We describe a case of thyrotoxicosis accompanied by markedly elevated blood lead levels (initially 53 micrograms/dl) in a 37-year-old woman. No current source of lead exposure was found; the woman gave a history indicative of lead exposure as a child and as an adult 7 years previously, however. In addition, she was found to have markedly elevated bone lead levels, as measured by K-x-ray fluorescence (154 +/- 5 in the mid-tibia and 253 +/- 6 micrograms/g bone mineral in the patella), and an increased serum osteocalcin level (2.76 nmol/l), reflecting the increased bone turnover that often accomp…

Exposure to lead and specific attentional problems in schoolchildren.

A pilot study was carried out to investigate the relationship between exposure to lead and attention in children. The participants were 43 boys, 8 to 12 years of age, attending special schools for children with educational and/or learning problems (so called LOM schools). Children with probable causes of attentional or memory problems other than lead contamination were excluded from the study. Various aspects of attention were measured using neuropsychological tests. As an assessment of body lead burden, lead concentration in the boys' hair was measured by means of the Synchrotron Radiation-In…

[Permanent pacemaker lead insertion for totally occluded subclavian vein].

We experienced 4 patients with total occlusion of the subclavian vein in whom permanent pacemakers were implanted. Occlusion of the subclavian vein was not clinically apparent in these 4 patients. The pacing lead was inserted through the contralateral subclavian vein in a 65-year-old male. In a 80-year-old male, the pacing lead was inserted trough the internal jugular vein because of occlusion of both subclavian veins. The pacing lead was passed through the occluded portion of the innominate vein in a 84-year-old male. Phlebography-guided venipuncture was not difficult in a 79-year-old female …

Effects of hyperparathyroidism on blood lead concentrations in man.

To investigate whether hyperparathyroidism has an effect on blood lead (Pb) concentrations and whether parathyroidectomy will alter blood Pb concentrations, we studied 15 subjects with primary hyperparathyroidism (HPTH) and 7 control subjects with thyroid nodules (TC). Blood Pb concentrations were determined several weeks prior to surgery and then again 4 months later. A reference group (REF) of nonsurgical lab/office workers was examined over the same periods of time. Initial mean blood Pb concentrations were 6.6 +/- 2.8, 6.0 +/- 2.9, and 5.0 +/- 1.7 micrograms/dl for the HPTH, TC, and REF gr…

Chronic exposure to lead attenuates cocaine-induced behavioral activation.

Adult, male rats were exposed to a diet containing 500 ppm (0.05%) lead for 105 days before testing for cocaine-related changes in activity using a Digiscan activity system. Behavioral testing occurred on 6 successive test days. Activity was recorded for 20 min prior to and 40 min after IP injections of either 10, 20, or 40 mg/kg cocaine HCl, with saline injections on the day preceding each drug test day. Cocaine-induced behavioral activation was evident in control diet animals for all three doses (10, 20, and 40 mg/kg). While 10 mg/kg cocaine HCl did not produce behavioral activation in lead-…

[Does occlusal overload lead to periodontal recession? Literature review].

The etiology of periodontal clefts has been widely discussed throughout the years. Trauma from the occlusion is claimed to be one of the most important causative factors. The present review of the scientific literature did, however, not show that an occlusal overload could be regarded as the main etiologic factor in gingival recessions.

Sources of lead exposure in urban slum school children.

A school based study was undertaken to trace various sources of lead exposure in urban slum children. Two cases with elevated venous blood lead (PbB) levels were detected and confirmed in 100 consecutive Bombay school children, referred to a hospital school clinic for clinical pallor. A common source of lead exposure in one suburb, was implied by the significantly higher PbB levels here. Systematic family and environmental studies identified the source was a nearby factory manufacturing lead storage batteries. Similar studies for the other index case traced the source to the dust produced by s…

The persistent threat of lead: medical and sociological issues.

Lead exposure is an ancient malady. Its history serves as a useful paradigm through which to understand many other pollutants that our technological society has inserted into the human environment and may guide preventive steps for other agents. Lead poisoning was first recognized in workers exposed to high doses. The discovery of childhood toxicity occurred a century ago in Australia, when children with striking symptoms of paralysis, ophthalmoplegia, or meningitis were found to be highly lead exposed. Encephalopathy generally occurs at blood lead levels of 80 micrograms/dL or more, but unequ…

A ceramic glazer presenting with extremely high lead levels.

A case of lead poisoning in a ceramic glazer is reported. The patient had an extremely high level of blood lead at 29.5 mumol/l, and many of the laboratory features of lead toxicity: normocytic anaemia with marked basophilic stippling, abnormal blood and urinary porphyrins, and elevated liver enzymes. Surprisingly, the patient had no electromyographic evidence of neurologic involvement. The patient was treated with intravenous EDTA-calcium followed by oral penicillamine. Urinary porphyrin and porphyrin precursor excretion followed an interesting pattern, correlating with the chelator used. Thi…

Lead poisoning from a gunshot wound to the hand.

Lead intoxication from retained bullets is uncommon but potentially fatal. A 45-year-old man was shot in the left wrist with a .32-caliber rifle. He was first seen 16 months later with symptoms, signs, and laboratory values that were consistent with the diagnosis of plumbism. The factors leading to plumbism, its evaluation, and treatment are reviewed.

Lead poisoning in inherited delta-aminolevulinic acid dehydratase deficiency.

delta-Aminolevulinic acid dehydratase (ALA-D), respectively porphobilinogen synthase, EC 4.2.1.24) activity can be lowered by toxic, metabolic and hereditary factors. A 30-year-old painter was suffering from lead poisoning with an acute abdominal-neurologic syndrome and anemia. Blood lead was measured at 414 micrograms/l. Urinary ALA and coproporphyrin excretion as well as erythrocyte protoporphyrin had increased extremely, whereas ALA-D activity in erythrocytes had decreased extremely to 8% of controls. Excretion parameters, protoporphyrin, hemoglobin and lead returned to normal after treatme…

Lead accumulation by rat renal brush border membrane vesicles.

Pb++ accumulation by rat renal cortical brush border membrane vesicles was evaluated by in vitro incubation with rapid filtration technique. Pb++ uptake was time- and concentration-dependent, with apparent saturation of binding sites at 100 to 200 microM (5 sec initial rate experiments). Equilibrium binding studies (60-min incubation) showed that the ratio of bound Pb++ to free Pb++ was constant at 1.25 +/- 0.07 between 0.01 to 10 microM Pb, with decreasing bound to free ratios at higher concentrations. Osmotic experiments showed that Pb++ uptake was due primarily to membrane binding rather th

Lead and hyperactivity revisited. An investigation of nondisadvantaged children.

The purposes of this study were to examine whether hyperactive children had elevated lead levels, to assess whether lead levels were associated with poor psychometric performance in hyperactive children, and to replicate previous reports of hyperactive children with perinatal insult having lower lead levels than those with normal perinatal history. Hyperactive, learning-disabled children and normal siblings were studied. The hyperactive children had higher chelated urine lead levels than their own siblings. Other contrasts were not significant. Cognitive performances and lead levels were weakl…
