Effect of limb tourniquet on cerebral perfusion pressure in a head-injured patient.

Thirty percent of patients with severe head injury also have significant extracranial injuries. Treatment for these injuries should not be allowed to jeopardise the brain which is more susceptible to damage in these circumstances. A case is presented in which significant decrease in cerebral perfusion pressure occurred consequent upon use of a lower limb tourniquet.

Vascularised tube pedicle: a case report.

The conventional 4-stage migration of a tube pedicle extending over a period of several months can be reduced to a 2-stage procedure taking 4 weeks. By using the groin flap and revascularising its medial end by anastomsis to wrist vessels, the flap can be applied to its intended site in one stage. Four weeks later it is detached and insert. This method of transfer could sound the death knell of the tube pedicle as conventionally conceived if economics and patient comfort are to be considered.

Quantitative Sensory Testing and Current Perception Threshold Testing in Patients With Chronic Pain Following Lower Extremity Fracture.

BACKGROUND: Chronic pain is a significant problem for patients with lower extremity injuries. While pain hypersensitivity has been identified in many chronic pain conditions, it is not known whether patients with chronic pain following lower extremity fracture report pain hypersensitivity in the injured leg. PURPOSE: To quantify and compare peripheral somatosensory function and sensory nerve activation thresholds in persons with chronic pain following lower extremity fractures with a cohort of persons with no history of lower extremity fractures. METHOD: This was a cross-sectional study wher

Full-Season Injury Epidemiology in TeamGym-A Prospective Cohort Study Involving 474 Gymnasts.

Team gymnastics (TeamGym), a sport primarily practiced by girls/women, has rapidly gained popularity among adolescents. Despite a high pain prevalence among gymnasts, comprehensive epidemiological studies on injuries are lacking. This study aimed to investigate injury incidence rates, both overall and by body region including information on time-loss and non-time-loss injuries, type and severity, in competitive TeamGym gymnasts aged 10-30 years. During a 10-month season (August 2021-June 2022), a cohort of 474 gymnasts (73% women) was prospectively followed through weekly text-message queries

Active muscle response contributes to increased injury risk of lower extremity in occupant-knee airbag interaction.

OBJECTIVE: Recent field data analysis has demonstrated that knee airbags (KABs) can reduce occupant femur and pelvis injuries but may be insufficient to decrease leg injuries in motor vehicle crashes. An enhanced understanding of the associated injury mechanisms requires accurate assessment of physiological-based occupant parameters, some of which are difficult or impossible to obtain from experiments. This study sought to explore how active muscle response can influence the injury risk of lower extremities during KAB deployment using computational biomechanical analysis. METHODS: A full-fact…

Spare part's free contralateral medial plantar flap for emergency reconstruction in a bilateral lower legs ballistic trauma.

The medial plantar flap is frequently used in heel reconstruction and has been described since 1969. We took care of a 25-year-old patient with a bilateral ballistic trauma and open fractures to the distal third of both legs. Given the seriousness of the damage to the left lower limb, a trans-tibial amputation had to be performed. Concerning the right lower limb, we decided to cover the defect with a spare part's free medial plantar flap thanks to the amputated fragment. The extremity spare tissues principle is already known and described, more particularly concerning hands, in emergency traum

[Compartment syndrome of the lower part of the leg, exceptional trauma mechanism].

Acute compartment syndrome (CS) is a limb threatening condition which warrants emergency treatment. We describe a case of a 37-year-old man with acute CS developed without major trauma. Early diagnosis and prompt treatment by decompressive fasciotomy is of vital importance in order to preserve limb function and to avoid complications. As a consequence of this case the department purchased a pressure transducer. Measurement of high pressure could have saved the patient for an undue risk of serious complications.

The island pedicle rotation advancement gastrocnemius musculocutaneous flap for complete coverage of the popliteal fossa.

The medial head of the gastrocnemius flap is a large, versatile flap that can be used to cover areas above the popliteal fossa, thereby minimizing the necessity for amputation in many traumatic injuries to this area. By advancing the medial head of the gastrocnemius muscle and rotating it on the neurovascular pedicle of the medial sural vessels, we have covered a 17 cm X 20 cm defect of crushed muscle, tendon, and bone in a leg that was traumatically amputated proximal to the knee. This procedure makes possible muscular coverage for bone, stainless steel plate, and reanastomosed arteries and v

Effect of wobble board training on movement strategies to maintain equilibrium on unstable surfaces.

Standing on unstable surfaces requires more complex motor control mechanisms to sustain balance when compared to firm surfaces. Surface instability enhances the demand to maintain equilibrium and is often used to challenge balance, but little is known about how balance training affects movement strategies to control posture while standing on unstable surfaces. This study aimed at assessing the effects of isolated wobble board (WB) training on movement strategies to maintain balance during single-leg standing on a WB. Twenty healthy men were randomly assigned to either a control or a training g

[Epiphyseal injuries of the lower extremity in sports].

Sports-related epiphyseal injuries have been becoming more prevalent during the last years. We are reporting 85 patients with epiphyseal fractures of the lower extremity treated in our hospital during the last twenty years. The injuries were sustained during soccer in 28% and during alpine skiing in 26% of the cases, of which the former was responsible for most of the injuries reported in males and the latter for those in females. The most frequently seen localization was the upper ankle joint (48 cases), followed by the proximal tibial (15 cases) and the distal femoral epiphysis (10). Of the

Interactions between rearward-facing child restraint systems and the front row seatback in frontal impact sled tests.

OBJECTIVE: Some child restraint system (CRS) manufacturers specify a minimum distance between the CRS and the seatback, whereas others require that the CRS may contact the seatback but cannot be "braced"; however, few studies have investigated these interactions. Therefore, the aim was to investigate the interactions between the front row seat and rearward-facing CRS models with and without a support leg during frontal crashes. METHODS: Sled tests using the FMVSS 213 frontal crash pulse were performed with the Q1.5 and Q3 anthropomorphic test devices (ATDs) seated in rearward-facing infant an

Acute biomechanical responses to wearing a controlled ankle motion (CAM) Walker boot during walking.

BACKGROUND: Controlled ankle motion (CAM) boots are often prescribed during the rehabilitation of lower limb injuries and pathologies to reduce foot and ankle movement and loading whilst allowing the patient to maintain normal daily function. RESEARCH QUESTION: The aim of this study was to quantify the compensatory biomechanical mechanisms undergone by the ipsilateral hip and knee joints during walking. In addition, the compensatory mechanisms displayed by the contralateral limb were also considered. METHODS: Twelve healthy participants walked on an instrumented treadmill at their preferred
