Clinical characteristics and treatment outcomes of women with recurrent uterine leiomyosarcoma.

OBJECTIVE: To determine the clinical characteristics and treatment outcomes of women with recurrent uterine leiomyosarcoma (uLMS). METHODS: We conducted a retrospective cohort study to evaluate the clinical characteristics and survival of women with recurrent uLMS and identify prognostic factors. RESULTS: Overall, 71 patients with first recurrence of uLMS were included in our study. 19 patients (26.8%) received systemic therapy and 52 patients (73.2%) received secondary cytoreductive surgery (SCS). In SCS subgroup (n = 52), a complete resection with no residual disease was performed in 47 pa…

Leiomyosarcoma of the small bowel presenting as an acute small bowel obstruction.

Leiomyosarcoma is a subtype of soft-tissue sarcoma, which is a rare soft-tissue malignancy comprising < 1% of adult cancers. There are a variety of etiologies of small bowel obstruction. Infrequently, small bowel malignancies can first present as small bowel obstruction. In exceedingly rare cases, leiomyosarcomas can be the offending malignancy. A 53-year-old male presented to the emergency department with several weeks of persistent right abdominal pain, nausea, and vomiting. Computed tomography scan revealed a central necrotic mass within the right lower quadrant originating from the small b…

Conjunctival leiomyosarcoma: A clinico-pathological study with in deep molecular characterization.

BACKGROUND: Primary and metastatic leiomyosarcomas (LMS) involving the orbital region are well known to occur however, the conjunctiva represents an extremely rare site of occurrence. METHODS: A 97-year-old male was referred to the Ocular Oncology Unit due to a rapidly growing painful mass (16×12×20 mm) in the nasal conjunctiva of his left eye. Wide excision followed by radiotherapy was performed. RESULTS: Based on the microscopic features (hypercellular neoplasm composed of spindle cells with cigar shaped and blunt ended nuclei with brightly eosinophilic fibrillary cytoplasm) and immunohist…

Cellular Schwannoma Arising After the Resection of an Anterior Mediastinal Leiomyosarcoma.

Cellular schwannoma is a schwannoma subtype with histopathologic features resembling those of malignant tumors. We report a case of a cellular schwannoma arising at the resection margin of an anterior mediastinal leiomyosarcoma. An 88-year-old woman who had undergone resection of an anterior mediastinal leiomyosarcoma 2 years previously developed a mediastinal tumor at the resection margin, raising suspicion of leiomyosarcoma recurrence. Pathologic analysis after thoracoscopic resection confirmed a cellular schwannoma. This is an extremely rare cellular schwannoma arising in the anterior media…

Dedifferentiated leiomyosarcoma of the uterus: a clinicopathologic and immunohistochemical analysis of 23 cases.

AIMS: To morphologically and immunophenotypically characterize dedifferentiated uterine leiomyosarcoma (LMS). METHODS AND RESULTS: We identified 23 dedifferentiated uterine LMS, defined as a malignant uterine smooth muscle tumour containing discrete differentiated and dedifferentiated components (i.e. with and without morphologic and immunophenotypic evidence of smooth muscle differentiation, respectively). The differentiated component was leiomyosarcoma in most cases (17/23), though some arose from a leiomyoma (n = 4) or smooth muscle tumour of uncertain malignant potential (n = 2). The dedi…

A rare case of primary hepatic poorly differentiated leiomyosarcoma in a goat.

A 12-year-old Saanen goat presented with a history of hyporexia and sudden recumbency. Euthanasia was indicated due to suspicion of hepatic … vimentin but immunonegative for pancytokeratin. The Ki-67 index was 18.8%. A poorly differentiated leiomyosarcoma was diagnosed on the basis of the gross, histopathological and immunohistochemical findings and should be included in the differential diagnosis of liver disease in goats.

Liver metastasis or a pseudocyst? A rare presentation of leiomyosarcoma's metastasis in the liver.

Gastrointestinal neoplasms most commonly metastasize to the liver, where they are typically found as solid and hypervascular lesions. Here, we describe a case of a 44-year-old man with a leiomyosarcoma of the rectum, who at the time of diagnosis presented with a small (5 mm in diameter) cyst-like lesion in the liver. Positron emission tomography demonstrated no increased metabolism in the area of the cyst, suggesting a benign character of the lesion. However, after 3 years, CT scans revealed enlargement of the cyst, and local surgical excision was performed. The results of histopathological ex…

MRI Evaluation of Uterine Masses for Risk of Leiomyosarcoma: A Consensus Statement.

Laparoscopic myomectomy, a common gynecologic operation in premenopausal women, has become heavily regulated since 2014 following the dissemination of unsuspected uterine leiomyosarcoma (LMS) throughout the pelvis of a physician treated for symptomatic leiomyoma. Research since that time suggests a higher prevalence than previously suspected of uterine LMS in resected masses presumed to represent leiomyoma, as high as one in 770 women (0.13%). Though rare, the dissemination of an aggressive malignant neoplasm due to noncontained electromechanical morcellation in laparoscopic myomectomy is a de…

Epithelioid leiomyosarcoma of the vulva: report of a rare case and literature review.

Leiomyosarcoma of the vulva is a rare soft tissue sarcoma that accounts for approximately 1% of all primary vulvar neoplasms, but it is the most common type of vulvar sarcoma. It usually originates from the smooth muscle within erectile tissue or blood vessel walls, the round ligament, the dartos muscle or the arrector pili muscle. No treatment algorithms have been established to date. Surgical resection is preferred for vulvar leiomyosarcoma. Currently, the recommended surgical method is extensive local resection with a safe surgical margin of at least 2 cm. The use of chemoradiotherapy for v…

Eribulin in Patients with Liposarcoma and Leiomyosarcoma: A Retrospective Single-Center Experience.

INTRODUCTION: Soft tissue sarcomas are rare and heterogenous malignancies with a poor prognosis in advanced disease stages. Eribulin is used in metastatic liposarcoma (LPS) patients, who have failed first-line chemotherapy and has been approved for use in patients with LPS in the USA and Europe due to its efficacy in this histological subtype in a phase 3 trial. We have evaluated efficacy and tolerability of eribulin in LPS and leiomyosarcoma (LMS) patients in the routine clinical setting at our department. METHODS: In this retrospective single-center analysis, efficacy and safety of eribulin…

Stathmin expression alters the antiproliferative effect of eribulin in leiomyosarcoma cells.

Uterine leiomyosarcoma is an aggressive soft tissue tumor. Stathmin, a phosphoprotein that modulates microtubule dynamics, is highly expressed in many malignancies including leiomyosarcoma. The microtubule-depolymerizing agent eribulin has been recently approved for treating malignant soft tissue tumors. Although eribulin inhibits microtubule polymerization, little is known about the relationship between eribulin treatment and stathmin dynamics. In this study, we explored the role of stathmin expression in the action of eribulin in leiomyosarcoma cells. Eribulin induced phosphorylation of stat…

"Malignant Mesenchymoma" Revisited: A Clinicopathologic Study of Leiomyosarcomas With Osteosarcomatous Differentiation.

Leiomyosarcoma (LMS) is the most common sarcoma in adults. Rarely, LMS dedifferentiates into an undifferentiated sarcoma. Very few cases of LMS with heterologous osteosarcomatous differentiation (OS) have been reported. The purpose of this study was to evaluate the clinicopathologic features of LMS with OS. Of 5570 LMS cases diagnosed from 2006 to 2022, 15 cases (0.2%) of LMS with OS were identified, affecting 13 females and 2 males; ages ranged from 32 to 66 years (median: 53 y). Ten tumors arose in the uterus, 2 in the retroperitoneum, and 1 each in the mesentery, mediastinum, and rectum. Pr…

Endoscopic Treatment of Sinonasal Leiomyosarcoma: A Case Report in Light of the Literature.

A 49-year-old Japanese man presented with a rare case of sinonasal leiomyosarcoma with left nasal bleeding for 12 months. He had no history of irradiation or malignancies, including retinoblastoma. Preoperative histological examination suggested vascular leiomyoma. Complete resection with endoscopic surgery was performed. Histological examination during the operation suggested that the tumor was a leiomyoma. However, immunohistochemical staining for α smooth muscle actin and desmin were helpful in establishing a definitive diagnosis of sinonasal leiomyosarcoma. The resection margins were posit…

Leiomyosarcoma of the Vulva Mimicking as Chronic Bartholin Cyst: A Case Report.

Smooth muscle neoplasms of the vulva are rare, and when localized in the Bartholin's gland, it can be mistaken as a benign lesion leading to a delay in diagnosis. We report a case of leiomyosarcoma of the Bartholin's gland in a 63-year-old postmenopausal woman, which clinically mimicked a chronic Bartholin's gland cyst. This case report emphasizes the importance of suspecting malignancy in any vulvar lesion in women of postmenopausal age, despite its location and asymptomatic appearance. A prompt, careful evaluation with adequate clinical judgment will help to manage such cases efficiently.

Twin pathologies: synchronous leiomyosarcoma and squamous cell carcinoma of the larynx.

The synchronous discovery of leiomyosarcoma (LMS) and squamous cell carcinoma (SCC) of the larynx is extremely rare. We describe our experience of managing a patient with this unusual presentation. A 35-year-old man was initially diagnosed with advanced stage (T4bN1M0) laryngeal LMS. Neoadjuvant sarcoma-directed chemotherapy and radiation therapy allowed a successful pharyngolaryngo-oesophagectomy to be performed in an otherwise inoperable cancer. Histological examination of the resection specimen revealed poorly differentiated SCC, indicating the underlying diagnosis was a sarcomatoid carcino…

A case of leiomyosarcoma originating from a bronchogenic cyst: A case report.

Mediastinal leiomyosarcoma is an aggressive tumor that primarily occurs in the posterior mediastinum. A bronchogenic cyst is a benign lesion that often develops in the mediastinum close to the thoracic trachea and has been reported to be the origin of certain malignancies. The present study reports an extremely rare case of an anterior mediastinal leiomyosarcoma that was considered to have originated from a bronchogenic cyst. An 82-year-old woman complained of chest pain, presenting an abnormal 70 mm mass shadow in the anterior mediastinum of a chest CT scan. Mediastinal tumor resection was pe…
