The Davies Z-plasty repair of the unilateral cleft lip can generate adequate lip height with little transverse tightness and should be considered in the presence of a vertically short lateral lip. Separate repair of the orbicularis muscle may contribute to good long-term lip height and shape. It is possible to revise the Z repair to increase height.

The risks and complications associated with tongue and lip piercings range from abnormal tooth wear and cracked tooth syndrome to gingival recession and systemic infections. PURPOSE: An asymmetric smile, caused by loss of function of the lip depressors, can be functionally and cosmetically debilitating. Desmoplastic melanoma is a rare variant of malignant melanoma and one of the main causes of clinical and histological misdiagnosis in the realm of melanocytic neoplasms. Its relatively innocuous clinical appearance and the absence of pigmentation usually lead to a clinical misdiagnosis. The authors report a case of desmoplastic melanoma located in the lower lip of a 42-year-old man and review the literature regarding pathogenesis, clinicopathological features, differential diagnosis, and therapeutic management of desmoplastic melanomas.

The technique for primary correction of the cleft lip and nose, as practised by The North Thames Cleft Lip and Palate Centre at Great Ormond Street Hospital/St Andrew's Centre (GOStA) has evolved over 28 years of personal experience. It is an amalgamation of techniques developed by many others with some personal modifications. Patients are routinely audited at 5, 10, 15 and 20 years and outcomes evaluated and compared with other centres where possible. Secondary surgery of lip and nose is based on the same principles of anatomical reconstruction used in the primary surgery.

OBJECTIVE: To study the results 10 years after early surgical cleft lip and nose repair.
PATIENTS AND METHODS: We present the outcome of 123 early cleft lip repairs whose condition was managed in a multidisciplinary team according to a strict protocol. We give the observation results of operations of a single surgeon's neonatal surgery over a 10-year period in term of aesthetic assessments and dental arch relationships. Ectopic dental eruption into the nasal cavity is a rare phenomenon with an obscure cause. A case of an early erupted intranasal tooth in a patient with a repaired cleft lip, an unrepaired cleft alveolus, and fusion of lower incisor teeth is reported. Only three similar cases of intranasal teeth in cleft patients have been reported in the literature. The unusual features in the present case are highlighted.

In summary, Aicardi syndrome is defined by its tetrad of infantile spasms, agenesis of the corpus callosum, mental retardation, and chorioretinal lacunae. We report a case of Aicardi syndrome with associated cleft lip and palate. This is an infrequent finding that is present in approximately 3 percent of reported cases. Plastic surgeons should be aware of this association when treating patients with cleft lip and palate.
