Diabetic foot screening programs through an implementation science lens: A rapid review.

BACKGROUND: Diabetes-related foot conditions drive amputations and premature mortality worldwide. Although international guidelines recommend annual foot screening based on risk, implementation remains insufficient and inequitable. This rapid review synthesized evidence on primary-care diabetic foot screening programs, examined barriers and facilitators using the Consolidated Framework for Implementation Research, and mapped strategies with an equity lens. METHODS: A rapid review following Dobbins (2017) was reported according to PRISMA guidelines. MEDLINE, EMBASE, and CINAHL were searched fr

How to screen and provide rapid initial treatment for sepsis in adult patients.

Sepsis is a serious, life-threatening condition in which a dysregulated host response to infection causes organ dysfunction, potentially leading to death. It is a time-critical condition, so early recognition of deterioration potentially due to sepsis, escalation to a senior clinician and rapid initial treatment are key for improving outcomes and saving lives. Nurses screening patients for sepsis must have the knowledge and skills required to do so effectively, working within their level of competence. • Sepsis is a life-threatening and time-critical condition that requires prompt recognition

[Screening for chronic kidney disease in children].

Chronic kidney disease (CKD) in children has an epidemiology that differs significantly from that in adults. Its main causes vary with age with a predominance of congenital anomalies of the kidney and the urinary tract. Early diagnosis is essential to prevent complications affecting growth, cognition, and long-term cardiovascular health, and to avoid, or at least to slow, progression to end-stage renal disease. Specific clinical contexts warrant targeted screening: family history, perinatal abnormalities, at-risk conditions, or suggestive clinical signs. Screening relies on simple tools such a

Developing a national implementation strategy for enhancing the detection of familial hypercholesterolemia: An Australian experience.

BACKGROUND: Familial hypercholesterolemia (FH) is a common and treatable inherited condition of cholesterol metabolism that increases cardiovascular health risk. Less than 10% of people with FH have been detected and few receive recommended treatment. OBJECTIVE: We report on the development of a national implementation strategy for enhancing the detection of FH in Australia. METHODS: A modified intervention mapping study was conducted, through workshops to identify barriers and facilitators of FH detection, models for detection and their objectives. Practical implementation strategies were s

New tablet-based screening tool for assessment of cognitive decline in Nigeria.

We comment on the importance of the novel tablet-based screening tool for cognitive decline recently studied in Nigeria by Ogbuagu et al. TabCAT-BHA was administered on an iPad in urban and rural settings and was found to be a sensitive and culturally appropriate tool for assessing cognitive function and impairment. With accuracy, portability, speed, and ease of use, this study shows that such tests, computerized or online, have the potential to improve the screening of cognitive decline in diverse countries such as Nigeria, to facilitate early treatment and improved care and quality of life.

Echocardiographic Screening for Rheumatic Heart Disease: A Brief History and Implications for the Future.

Transthoracic echocardiography is the gold standard for early detection of rheumatic heart disease (RHD) in asymptomatic children living in high-risk regions. Advances in technology allowing miniaturisation and increased portability of echocardiography devices have improved the accessibility of this vital diagnostic tool in RHD-endemic locations. Automation of image optimisation techniques and simplified RHD screening protocols permit use by non-experts after a brief period of training. While these changes are welcome advances in the battle to manage RHD, it is important that the sensitivity a

A Meta-Review to Guide Military Screening and Treatment of Gambling Problems.

INTRODUCTION: Excessive gambling can cause substantial biopsychosocial problems (e.g., difficulties with finances, relationships, mental, and physical health). For military Service Members, it can also result in security clearance denial or revocation, failure to achieve promotions, and premature career termination. Recent congressional mandates have obligated the U.S. Department of Defense to screen for problematic gambling, the predictive values of which are a function of (i) problem prevalence and (ii) tool sensitivity and specificity. This meta-review (i.e., systematic review of systematic

Consistencies among miscellaneous scales for evaluation of post-stroke dysphagia.

PURPOSE: Post-stroke dysphagia (PSD) is the most common type of dysphagia. Stroke patients with sustained dysphagia have poorer outcomes. The severity of PSD is assessed using miscellaneous scales with unknown consistencies. We aim to investigate the consistencies among miscellaneous scales, which could aid in the assessment of PSD. METHODS: A total of 49 PSD patients were enrolled. Functional Oral Intake Scale (FOIS), Dysphagia Severity Scale (DSS), Ohkuma Questionnaire, Eating Assessment Tool-10, and Repetitive Saliva Swallowing Test were performed. FOIS was performed by physicians, and DSS

Evaluation of Candida auris acquisition in US international travellers using a culture-based screening protocol1.

We establish the feasibility of evaluating US international travellers for Candida auris acquisition using a culture-based screening protocol. Candida auris was not identified in any of the travellers in this small cohort; further study is needed to determine the overall risk and risk factors for travel-associated acquisition.

Screening screeners: calculating classification indices using correlations and cut-points.

Given the recent push for universal screening, it is important to take into account how well a screener identifies children at risk for reading problems as well as how screener and sample information contribute to this classification. Picking the best cut-point for a particular sample and screening goal can be challenging given that test manuals often report classification information for a specific cut-point and sample base rate which may not generalize to other samples. By assuming a bivariate normal distribution, it is possible to calculate all of the classification information for a screen

The Case For Cadmium and Lead Heavy Metal Screening.

Exposure to cadmium and lead is widespread, and is related to environmental contamination, occupational sources, food, tobacco and other consumer products. Lower socioeconomic status increases the risk of heavy metal exposure and the diseases associated with cadmium and lead toxicity. Concurrent toxicity with both cadmium and lead is likely but has not often been assessed. There is now substantial evidence linking cadmium and lead to many diseases including hypertension, diabetes mellitus, obesity, cancer, coronary artery disease, chronic kidney disease (CKD) and lung disease. Both chronic ren

Psoriatic arthritis screening: A systematic literature review and experts' recommendations.

OBJECTIVE: To analyze the performance of psoriatic arthritis (PsA) screening tools, examine their implementation in daily practice, and reach a consensus about the best screening tool for implementation in daily practice in different medical settings. METHODS: A systematic literature review (SLR), structured telephone interviews to hospitals, and a multidisciplinary nominal group meeting were all conducted. The SLR employed sensitive search strategies using Medline, Embase, and the Cochrane Library up to January 2020. Two reviewers independently selected articles that reported data on PsA scr
