Facial nerve grafting and end-to-end anastomosis in the middle ear: tympanic cavity and mastoid.

UNLABELLED: Sectioned facial nerves can be repaired with grafting or end-to-end anastomosis. OBJECTIVE: To discuss these repair procedures and what can be expected of them. METHOD: Seven patients with sectioned facial nerves were included in the study. Four underwent grafting and three were offered end-to-end anastomosis. Facial nerve palsy was iatrogenic in five patients and was caused by bullet wounds in two. Assessment of motor function recovery was based on Janssen's scale. RESULTS: Mean motor recovery was rated at 72.5% for subjects offered grafting and 73.3% for patients submitted to

Evaluating the effectiveness of the Voxel-Man TempoSurg virtual reality simulator in facilitating learning mastoid surgery.

INTRODUCTION: The Chief Medical Officer's 2008 annual report highlighted the importance of simulation in medical training. Simulator development has focused on increasing authenticity and fidelity. Development has not necessarily been guided by evidence for educational improvement. On reviewing 34 years of literature, Issenberg et al identified ten features of high-fidelity medical simulators that facilitate learning. This study compares cadaveric temporal bone (CTB) simulation with the Voxel-Man TempoSurg (VT) virtual reality simulator in addressing these features. SUBJECTS AND METHODS: A qu

Comparison of bone-conducted vibration for eliciting ocular vestibular-evoked myogenic potentials: forehead versus mastoid tapping.

OBJECTIVE: This study compared bone-conducted vibration (BCV) stimuli at forehead (Fz) and mastoid sites for eliciting ocular vestibular-evoked myogenic potentials (oVEMPs). STUDY DESIGN: Prospective study. SETTING: University hospital. METHODS: Twenty healthy subjects underwent oVEMP testing via BCV stimuli at Fz and mastoid sites. Another 50 patients with unilateral Meniere's disease also underwent oVEMP testing. RESULTS: All healthy subjects showed clear oVEMPs via BCV stimulation regardless of the tapping sites. The right oVEMPs stimulated by tapping at the right mastoid had earlier nI…

SerenoCem -glass ionomeric granules: a 3-year follow-up assessment of their effectiveness in mastoid obliteration.

A chronically discharging cavity is a complication of canal-wall down mastoidectomy procedures. Mastoid obliteration can reduce this otorrhoea, with many techniques and materials for obliteration described. This study assesses the effectiveness of SerenoCem granules in providing successful obliteration, by recording the need for subsequent aural care and patient satisfaction using the Glasgow Benefit Inventory. All ears were made dry by the procedure (three after revision). Aural care visits reduced [down 9.2 (95% CI 4.7-13.8) over 2 years] and quality of life benefited [total postoperative Gl…

Reconstruction of the nasal tip including the columella and soft triangle using a mastoid composite graft.

This paper describes the use of a composite graft from the mastoid area consisting of full-thickness skin peripherally and selectively localised fascia-fat tissue underneath the skin centrally for immediate reconstruction of moderate defects of the nasal tip including the columella and soft triangle. Mastoid composite grafting is a simple and safe procedure that avoids partial graft loss and provides adequate augmentation of soft tissue, easy reshaping of the new nostril rim, minimal post-operative shrinkage, and no donor-site morbidity. Then, it results in a satisfactory nasal appearance with…

[Fates of bone defects in the mastoid cavity studied from the findings of two-stage tympanoplasty].

The incidence and prognosis of bone defects occurring in the mastoid cavity were investigated in 205 ears of 197 patients who underwent planned two-stage tympanoplasty by the intact canal wall technique. Bone defects found at the first stage had spontaneously closed by the second stage in 43 of 62 ears at the mastoid tegmen, in 13 of 15 ears at the posterior wall of the mastoid cavity and in 2 of 17 ears at the posterior wall of the external ear canal. Twenty two ears developed bone defects at the ear canal between the first and second operations, despite the canal wall having been primarily p…

Brain herniation into the middle ear: two idiopathic cases treated by a combined intracranial-mastoid approach.

One hundred thirty-nine cases of brain herniation into the middle ear and mastoid have been reported in the world literature during the last 40 years. In 29 cases the herniation was idiopathic. In idiopathic cases, intermittent CSF otorhinorrhea, a history of intermittent "serous otitis media," and a conductive hearing loss were the main clinical findings. Polytomography or CT-scan revealed a tegmen antri or tympani defect in some patients. There are three surgical approaches for treatment of brain herniation into the middle ear: neurosurgical, otological, and combined.

Menière's disease: results of treatment with the endolymphatic subarachnoid shunt compared with the endolymphatic mastoid shunt.

The senior author has performed 346 endolymphatic sac operations since 1971. Questionnaires were sent to these patients to evaluate the results of this surgery. One hundred ninety-six questionnaires were returned and formed the basis for this report. The results of surgery are reported by the new American Academy of Otolaryngology--Head and Neck Surgery criteria as well as by the older method of reporting. There is no significant difference in results between the endolymphatic subarachnoid shunt and the endolymphatic mastoid shunt. Because of the lower morbidity of the latter procedure the aut…

Endolymphatic-mastoid shunt operation: results of the 24 cases and revision surgery with the silastic sheet.

Endolymphatic-mastoid shunt operation due to Kitahara's method was performed on 24 patients with Meniere's disease. In 14 patients, the vertiginous attack was controlled with satisfactory preservation of hearing for longer than one year. In 2 patients, the vertiginous attack subsided but the hearing worsened. In the remaining 8 patients, long-term control of the vertiginous attack was not attained. Of these 8 recurrent cases, revision was performed on 4, revealing shunt obstruction with granulation or scar tissue. After reopening the endolymphatic sac, a large triangular Silastic sheet was pla…

Functional and Aesthetic Outcomes of Trans-Mastoid Decompression for Facial Paralysis After Retroauricular Hyaluronic Acid Injection.

BACKGROUND: Retroauricular hyaluronic acid (HA) injection for aesthetic enhancement can precipitate immediate-onset facial nerve palsy. The underlying mechanism remains unclear, and no consensus exists on the next step when prolonged conservative therapy fails. OBJECTIVE: To investigate the pathogenesis of persistent facial paralysis following retroauricular HA injection and to evaluate the functional and aesthetic outcomes of trans-mastoid facial nerve decompression in affected patients. METHODS: We retrospectively reviewed 17 patients (16 females, 1 male; mean age 33.65±7.91 years) who dev…

Mastoid Reconstruction in Lateral Suboccipital Craniotomy: Autologous Bone Paste Enhances Sealing and Promotes Air-cell Osteogenesis.

Lateral suboccipital craniotomy is frequently used for cerebellopontine angle lesions, during which the mastoid air cells may be opened. This study evaluated whether the autologous bone paste technique prevents postoperative cerebrospinal fluid leakage and anatomically reconstructs the mastoid air cells. Eighty-six patients who underwent lateral suboccipital craniotomy with opened mastoid air cells were included in this study. The opened cells were sealed with various methods, including the autologous bone paste technique, in which bone dust harvested during the craniotomy was mixed with fibri…

Is Mastoid Head Bandage Really Required in Patients Undergoing Middle Ear Surgery? A Randomised Control Trial.

BACKGROUND: Middle ear surgery is a common procedure addressing various pathologies. Postoperative head bandaging has traditionally been used to provide support and prevent complications. However, its necessity remains controversial, with limited empirical evidence supporting its routine use. OBJECTIVE: This randomised controlled trial (RCT) evaluated the necessity of head bandaging in middle ear surgery by comparing postoperative complications between a mastoid head bandage and a simple gauze dressing. METHODS: Eighty patients scheduled for middle ear surgery were randomly assigned to two g…

Different types of mastoid vibrations affect the amount and structure of minimum toe clearance variability differently.

BACKGROUND: The majority of research primarily examines the role of the vestibular system in regulating balance by assessing gait parameters in the transverse plane while neglecting those in the sagittal plane. The present study aimed to examine the impact of various forms of mastoid vibration (MV) on minimum toe clearance (MTC) and its pattern of variability. This study proposed two hypotheses: 1) the application of MV reduced the MTC, and 2) the application of different forms of MV influenced the amount and structure of MTC variability. METHODS: A total of twenty young adults participated i…
