We treated 33 cases of intractable Meniere's disease with endolymphatic sac drainage and steroid-instillation surgery (EDSS), attaining good long-term results in vertigo and hearing. To elucidate how EDSS affects the diseased inner ear, we examined changes in plasma inner ear hormones after EDSS. Among inner ear hormones, plasma vasopressin was significantly decreased after EDSS compared to after mastoidectomy. In cases with good long-term results in vertigo and hearing, postoperative plasma vasopressin remained lower over the long term than in cases with poor results. In cases with negative g…

We present 15 cases of acute mastoiditis diagnosed in previously healthy children. Diagnostic criteria were the presence of acute otitis med … n with i.v. antibiotic therapy. Tympanostomy was performed in 5 of 6 cases with the entire eardrum. Mastoidectomy was performed in 3 patients due to poor results of the treatment. The evolution was successful in all cases.

BACKGROUND: Visual field defects occur from a variety of causes. As a result, the differential diagnosis of visual field defects and their etiology is important.
METHODS: A 64-year old white male was examined in follow up related to the surgical removal of a right temporal lobe abscess secondary to a childhood right side mastoidectomy. The patient underwent ophthalmic examination, which suggested possible changes in his field of vision, and neurobehavioral evaluation related to his visual field defect, a left homonymous superior quadrant defect.
RESULTS: Computed tomography (CT) scan of the …

A 14-year-old girl presented with bilateral sixth nerve palsies and papilledema after a two-week history of ear drainage and progressive rig … repeat computed tomography showed evidence of mastoiditis. Definitive therapy consisted of a right mastoidectomy and antimicrobial therapy, to which the patient responded promptly. This case illustrates an unusual sequela of otitis media--termed otitic hydrocephalus--that resulted from obstruction of the sigmoid sinus. Although otitis media is generally a beni …

Nine patients with tumours located at the petro-clival region were operated upon from June 1985 to June 1988 using a combined supra- and infratentorial approach anterior to the sigmoid sinus. Two patients had petroclival meningiomas. 4 foramen jugulare neurinomas and 3 glomus jugulare tumours. There was no mortality. Total tumour removal was accomplished in all the patients. All patients remained independent postoperatively. The surgical approach used involves a temporal craniotomy, a suboccipital craniectomy, an extensive mastoidectomy and petrous pyramid drilling without entering the bony la…

If homograft tympanoplasty is to be of value, specific instances in which homograft tympanic membrane, malleus, and incus (TMMI) provide significant hearing and anatomical advantages over standard techniques should be identified. This author has performed 305 homograft tympanoplasties limited to the reconstruction of the severely damaged middle ear. Indications include: previous failure with standard tympanoplasty techniques; high risk of anatomic or hearing failure with standard techniques (total perforation with absent malleus, slag burns); reconstruction of radical mastoidectomy; congenital…

Twenty per cent only had to undergo simple mastoidectomy and two cases a decompression. Out of 25 cases of Bell's palsy, 18 recovered totally without surgery and four were decompressed with total or partial recovery. OBJECTIVE: There are many forms of calvarial tuberculosis. In this case report, extensive calvarial tuberculosis caused a large cranial defe … left sigmoid sinus.
INTERVENTION: The encephalocele was surgically excised and combined with left mastoidectomy. Although no mycobacterium had been demonstrated in the excised lesion, the presence of active tuberculous infection in the mastoid led to the conclusion that the bone destruction leading to encephalocele was secondary to the calvarial tuberculosis. …

METHODS: Charts were reviewed for all patients who underwent tympanoplasty with and without mastoidectomy between June 1999 and June 2002 in whom AlloDerm was used as a grafting material. There were 24 patients, 17 with simple perforations (8 revisions) and 7 with chronic otitis media with cholesteatoma (6 revisions). Succe…

A study was undertaken in 43 patients to determine the role of surgery in tuberculous mastoiditis. Cortical mastoidectomy was performed on five patients (Group I). Incision and drainage of a post-auricular abscess, removal of sequestrum and meatoplasty in eight (Group II). Thirty patients had no ear surgery (Group III). A combined retroauricular and preauricular transpetrosal-transtentorial approach is described for the resection of meningiomas arising from the clivus. Via radical mastoidectomy the sigmoid sinus is exposed down to the jugular bulb, and via the transmastoideal-subtemporal approach the retroauricular petrosal bone, 1 cm in depth from the petrosal ridge, and the roof of the internal auditory meatus are removed, the middle ear and fallopian canal being left intact. Additionally, via a transzygomatic-subtemporal approach the preauricular petrosal bone is removed anteriorly up to the petrosal tip a…

Preoperative, operative and postoperative findings in 97 cases of labyrinthine fistula are presented. The majority of patients had had sympt … y. When a labyrinthine fistula is encountered in an only hearing ear, a classified modified radical mastoidectomy is usually recommended. In other instances, the procedure performed will vary with the status of the opposite ear, the extent of the fistula, the sensorineural function of the involved ear and the size of the mastoid.
