BACKGROUND: Maxillofacial fractures can lead to massive oronasal bleeding; however, surgical hemostasis and packing procedures can be challenging owing to complex facial anatomy. Only a few studies investigated maxillofacial fractures with massive oronasal hemorrhage. However, thus far, no studies have reported a protocolized management approach for maxillofacial trauma from a single center. This study aimed to evaluate the effectiveness of protocolized management for maxillofacial fractures with oronasal bleeding.
METHODS: Patients were identified from the National Cheng University Hospital

PURPOSE: The initial evaluation and treatment of trauma victims should follow a planned approach, as delineated by the Advanced Trauma Life Support protocol, with the main concern securing a patent airway. When trauma has been associated with maxillofacial injury, it can complicate airway management owing to aspirated avulsed teeth or dental prosthetic devices. In such cases, endotracheal intubation can be life-threatening, if the foreign bodies are pushed into the upper respiratory tract. The objective of the present report was to illustrate the diagnostic and management problems related to f

The Chosin Reservoir campaign of the Korean War represents one of the most extreme environments in which modern military medicine was tested. Among its casualties was First Lieutenant John Yancey of the US Marine Corps, who sustained a devastating open facial gunshot wound with orbital destruction while defending Hill 1282 in December 1950. Remarkably, Yancey survived the initial injury, remained conscious, retained command, and continued combat operations before eventual evacuation and surgical care. This paper revisits Yancey's injury as a historical and clinical lens through which to examin

UNLABELLED: Diagnostic imaging is pivotal in clinical maxillofacial evaluation. This study evaluated and compared the use of conventional radiographs (CR) with two-dimensional (2D) and three-dimensional (3D) computed tomography (CT) in patients requiring maxillofacial surgery. Pre-surgical records of sixty five patients who had undergone routine CR, 2D CT and 3D CT were selected and classified into the categories of traumatic injury, pathology and deformities. The accuracy of image analysis, diagnosis and treatment planning were scored and the data statistically analysed. 2D CT scanning was mo

Background: Disruption of peripheral branches of the trigeminal nerve in the field of maxillofacial surgery is a known risk due to the close connection of these branches with the bony structures of the maxilla and mandible. As a result, injuries of the lingual nerve and inferior alveolar nerve take place within routine maxillofacial surgery procedures, including local anesthetic injection, wisdom tooth surgery, and dental implant placement, resulting in paresthesia and dysesthesia. During the last three decades, low-level lasers (LLL) have been frequently used in various medical fields. Lately

BACKGROUND: The submental route for endotracheal intubation has been proposed as an alternative to tracheotomy in the surgical management of patients with maxillofacial trauma. The purpose of this study was to review our experience with this procedure.
METHODS: Medical records of 25 patients who had surgical reduction of midfacial or panfacial fractures while securing their airway with submental intubation were reviewed. After standard orotracheal intubation, a passage was created by blunt dissection with a hemostat clamp through the floor of the mouth in the submental area. The proximal end

The use of mouthguards in contact sports effectively prevents oral injury and preserves oral structures. We investigated oral injuries and awareness concerning mouthguards among Japanese high school soccer and rugby players. Athletes were asked a series of questions concerning their history of oral injury while participating in sports, and their pattern of mouthguard use was determined. The data were evaluated statistically using chi-square analysis. The incidence of oral injuries was 32.3% for soccer athletes and 56.5% for rugby athletes, with 0.8% and 24.1% of the respective groups having mo

At present, there are no objective testing modalities available for evaluation of iatrogenic injury to the terminal branches of the trigeminal nerve, making such clinical diagnosis and management complicated for the oral and maxillofacial surgeon. Several imaging modalities can assist in the preoperative risk assessment of the trigeminal nerve as related to commonly performed procedures in the vicinity of the nerve, mostly third molar surgery. This article provides a review of all available imaging modalities and their clinical application relative to preoperative injury risk assessment of the

Blast trauma is the primary cause of maxillofacial injury sustained by British service personnel on deployment, and the mandible is the maxillofacial structure most likely to be injured in combat, but there are few reports about the effect of blast trauma on it. The Joint Theatre Trauma Registry identified all mandibular fractures sustained by British servicemen secondary to blast injury between 1 January 2004 and 30 September 2009. These were matched to corresponding hospital notes from the Royal Centre for Defence Medicine (RCDM) for those evacuated servicemen and autopsy records for those w

The mandibular condyle is a commonly involved site of maxillofacial trauma in children. However, this injury is often overlooked on initial physical examination in the emergency department. In cases involving blunt trauma to the chin it is important to suspect possible injury to the condyle of the mandible, as this type of facial injury can result in a spectrum of facial growth disturbances during the patient's later years. General practitioners and pediatric dentists are often the first clinicians to recognize the late complications of condylar trauma years after the injury. With a good clini

Vessel sealing systems (VSSs) are medical devices that use bipolar electrothermal energy to seal blood vessels by denaturing collagen and elastin in the vessel wall, successfully occluding the vessel and reducing blood loss during surgical procedures. These devices eliminate the need for conventional ligatures or surgical clips in many situations and have become widely adopted in both human and veterinary surgery. Reported advantages include reduced operative time, improved hemostasis, decreased blood loss, and the ability to perform dissection and vessel sealing with a single instrument. The

The temporal bone anatomy is complex, with many critical structures in close association with one another. The temporal bone region comprises cranial nerves V, VI, VII, and VIII; vascular structures such as the internal carotid and middle meningeal arteries; sigmoid sinus; jugular bulb; and sensorineural and membranous structures of the inner ear. Most temporal bone fractures are a result of high-energy blunt head trauma. Multidetector computed tomography (CT) plays a fundamental role in the initial evaluation of patients with polytrauma in the emergency department. Multidetector CT may help i
