Management of Antiplatelet and Anticoagulant Agents before and after Polypectomy.

Antithrombotic medications, including antiplatelet drugs and anticoagulants, are widely prescribed to prevent thromboembolic disease. There is limited evidence informing gastroenterologists of the management of patients on antithrombotic medications undergoing colonoscopy and polypectomy. A patient's risk of thromboembolism versus postpolypectomy bleeding should be carefully considered, incorporating patient preferences concerning benefits and harms of temporary antithrombotic interruption. We will review the available consensus guidelines, current literature, and strategies to mitigate the ri

Leveraging Large Language Models for Personalized Parkinson's Disease Treatment.

Parkinson's Disease (PD) treatment is challenging due to symptom heterogeneity and the lack of a definitive cure. Lifelong medication requires personalized treatment plans developed by physicians, but such approaches are constrained by high costs and limited physician capacity. Although deep learning (DL) methods have been explored, they lack interpretability and are restricted to numerical data inputs. In this study, we propose a novel framework that leverages large language models (LLMs) to design personalized PD treatment strategies, integrating both patient information in natural language

Reinterpretation of pharmacogenomic phenotypes after combinatorial psychiatric testing.

AIM: Providers can use combinatorial pharmacogenomic panels to aid psychiatric medication prescribing. Results are typically documented in static documents which list the genotype and predicted phenotype (interpretation). However, genotype-to-phenotype translations can differ between laboratories and change as scientific consensuses evolves. Here, we describe the implications of reinterpreting phenotype after combinatorial psychiatric pharmacogenomic testing in a real-world setting. PATIENTS AND METHODS: 143 patients underwent testing from 2014 to 2021. Reported genotypes and phenotypes were

Comparison of tomographic reports by radiologists and non-radiologists in trauma and interferences in management in a trauma reference center.

OBJECTIVE: diagnostic errors during the interpretation of an imaging test by the physician can lead to increased mortality and length of hospital stay for patients. The rate of divergence in the report given by a radiologist and an Emergency Physicians (EP) can reach over 20%. The objective of this study was to compare the unofficial tomographic reports issued by EP with the official reports issued by radiologists. METHODS: a cross-sectional study, in which interpretations of the exams (documented in the medical records by the EP) of all patients undergoing computed tomography (CT) of the che

HTAADVar: Aggregation and fully automated clinical interpretation of genetic variants in heritable thoracic aortic aneurysm and dissection.

PURPOSE: Early detection and pathogenicity interpretation of disease-associated variants are crucial but challenging in molecular diagnosis, especially for insidious and life-threatening diseases, such as heritable thoracic aortic aneurysm and dissection (HTAAD). In this study, we developed HTAADVar, an unbiased and fully automated system for the molecular diagnosis of HTAAD. METHODS: We developed HTAADVar (http://htaadvar.fwgenetics.org) under the American College of Medical Genetics and Genomics/Association for Molecular Pathology framework, with optimizations based on disease- and gene-spe

Thymoma: Challenges and Pitfalls in Biopsy Interpretation.

The interpretation of biopsy specimens in the diagnosis of thymoma is a subject that is generally not addressed in the literature. Even though the diagnosis of thymoma may seem to be an easy step in the assessment of these tumors, in reality, it is the biopsy specimen interpretation that will be use to determine course of action in any particular patient. It may determine whether a patient is a surgical candidate or on the contrary whether a patient may be benefited the most by medical therapy. In addition, there may be conditions in which all that is required is surgical resection without any

Translating pediatric urodynamics from clinic into collaborative research: Lessons and recommendations from the UMPIRE study group.

Pediatric patients present unique challenges in the performance and interpretation of urodynamic studies. Interpretation of urodynamics to guide clinical management at an institutional level is accepted as reliable. Challenges arise however when multi-site collaborations incorporate urodynamics into study design to determine primary or secondary outcomes or to direct decision-making. Although standardized terminology has been established by ICCS, the application of this shared language to performance and interpretation of pediatric urodynamics to across multiple sites may not be intuitive or r

Mood volatility with rumination but neither attentional nor interpretation biases in chronic fatigue syndrome.

OBJECTIVES: This study tested whether (1) chronic fatigue syndrome (CFS) individuals have a bias in the initial orientation of attention to illness-related information, which is enhanced by rumination. (2) CFS individuals have an illness interpretation bias (IB) in their early automatic processing of ambiguous information. (3) CFS individuals experience a greater degree of mood fluctuation following rumination and distraction inductions. DESIGN: Thirty-three CFS participants who had received a medical practitioner's diagnosis of CFS were compared to 33 healthy matched controls on an exogenous

A model of teaching and learning in the operating theatre.

INTRODUCTION: This paper extends the work of an earlier publication by the same author which reported the findings of a case study designed to investigate how medical students learn and are taught in the operating theatre. The earlier paper was descriptive in nature, examining the challenges students face as learners in theatres. These were conceptualised around 3 key domains: the challenge posed by the physical environment; the challenge of the educational task, and the challenge of managing and negotiating a role as a participant in the professional workplace of theatres. This paper focuses

The experiential meaning of eating, handicap, adaptedness, and confirmation in living with esophageal dysphagia.

This article is mainly based on interview studies of pensioners' (n = 62), patients' (n = 19), and students' (n = 87) experiences of living with longstanding esophageal dysphagia. The aim is to describe the experiential meaning of dysphagic patients' lives by interpreting their experiences, problems, and emotions and by specifying these into scales according to the model of goal-directed action by Pörn [43-49]. The experiential meaning will be articulated in terms of attainment or nonattainment of goals in relation to eating, handicap, adaptedness, and confirmation. In the experiential meaning

Medical treatment of Attention Deficit/Hyperactivity Disorder (ADHD) and children's academic performance.

Attention Deficit/Hyperactivity Disorder (ADHD) is negatively associated with a range of academic achievement measures. We use Danish administrative register data to study the impact of medical treatment of ADHD on children's academic performance assessed by student grade point average (GPA). Using administrative register data on children, who begin medical treatment, we conduct a natural experiment and exploit plausible exogenous variation in medical nonresponse to estimate the effect of medical treatment on school-leaving GPA. We find significant effects of treatment on both exam and teacher

A Trust-Based Methodology to Evaluate Deep Learning Models for Automatic Diagnosis of Ocular Toxoplasmosis from Fundus Images.

In the automatic diagnosis of ocular toxoplasmosis (OT), Deep Learning (DL) has arisen as a powerful and promising approach for diagnosis. However, despite the good performance of the models, decision rules should be interpretable to elicit trust from the medical community. Therefore, the development of an evaluation methodology to assess DL models based on interpretability methods is a challenging task that is necessary to extend the use of AI among clinicians. In this work, we propose a novel methodology to quantify the similarity between the decision rules used by a DL model and an ophthalm

Deep learning computer-aided detection system for pneumonia in febrile neutropenia patients: a diagnostic cohort study.

BACKGROUND: Diagnosis of pneumonia is critical in managing patients with febrile neutropenia (FN), however, chest X-ray (CXR) has limited performance in the detection of pneumonia. We aimed to evaluate the performance of a deep learning-based computer-aided detection (CAD) system in pneumonia detection in the CXRs of consecutive FN patients and investigated whether CAD could improve radiologists' diagnostic performance when used as a second reader. METHODS: CXRs of patients with FN (a body temperature ≥ 38.3 °C, or a sustained body temperature ≥ 38.0 °C for an hour; absolute neutrophil count

Predicting infected pancreatic necrosis in acute pancreatitis using machine learning models and feature selection.

Infected pancreatic necrosis (IPN) is a life-threatening complication of acute pancreatitis (AP), and its early prediction remains challenging. This study aimed to develop and externally validate interpretable machine learning models for individualized IPN risk prediction. A total of 728 patients with AP admitted to Xuanwu Hospital, Capital Medical University, between 2017 and 2023 were retrospectively analyzed. Embedded feature selection was incorporated within model training using regularized linear and tree-based algorithms to enhance interpretability and prevent overfitting. Five machine l

Why Most Child Sexual Abuse Examinations Are Normal: Forensic Implications, Misconceptions, and Evidence-Based Communication.

Child sexual abuse (CSA) evaluations frequently yield normal or nonspecific anogenital findings, even in cases later substantiated through investigation. This reality is often misunderstood in clinical, legal, and multidisciplinary contexts, where the absence of physical injury may be incorrectly interpreted as the absence of abuse. The purpose of this narrative review is to explain why most CSA examinations are normal and to provide an evidence-based framework for interpreting and communicating these findings in a manner that is medically accurate and forensically defensible. A targeted revie

A multimodal deep learning framework for clinical nursing assessment in lumbar fusion surgery via representation learning and feature extraction.

Medical image interpretation plays a critical role in lumbar fusion surgery, where accurate analysis of anatomical structures is essential for clinical assessment. However, most existing deep learning approaches rely primarily on visual features and fail to effectively integrate heterogeneous clinical information. This study proposes a multimodal deep learning framework for lumbar spine image interpretation by jointly modeling medical images and associated clinical text. The framework adopts a global-local representation learning strategy to capture both overall anatomical context and fine-gra

Is diagnostic review of radiotherapy-planning CT scans important in the conformal therapy era?

OBJECTIVE: Detailed CT scans are often acquired during the radiotherapy planning process. This study was performed to determine the incidence of important benign and cancer-related CT findings on these scans. SUBJECTS AND METHODS: From December 1998 to December 2000, 162 radiotherapy patients who were to be treated curatively underwent treatment planning CT scans on a helical scanner in the radiology department at Washington Hospital, Fremont, CA. All CT scans were prospectively interpreted relative to diagnoses, and reports were dictated for the medical records. The diagnostic reports and re
