[Medication-related osteonecrosis of the jaw : a rare but preventable complication].

Medication-related osteonecrosis of the jaw is a rare but severe complication of antiresorptive or antiangiogenic treatments. Prevention relies on patient education, dental screening prior to therapy, and avoidance of invasive procedures. Diagnosis is based on clinical and radiological criteria, with classification into four stages. Management is multidisciplinary and adapted to disease stage: strict oral hygiene, antibiotics, sequestrectomy, or reconstructive surgery. Regular follow-up and close communication among healthcare providers are key to controlling disease progression and maintainin

A Multifaceted Approach to Seizure Management in a Patient With Down Syndrome, Obstructive Sleep Apnea, and Hypothyroidism: A Case Report.

In this case study, a 16-year-old male with Down syndrome (DS) faced persistent nocturnal seizures despite anti-seizure medications and treatment for concurrent hypothyroidism. Obstructive sleep apnea (OSA), a common issue in patients with Down syndrome, was revealed as a trigger of the seizures. The implementation of continuous positive airway pressure (CPAP) therapy along with medication adjustments led to a significant decrease in seizure frequency, highlighting the importance of a comprehensive approach to seizure management in patients with complex medical conditions.

[Phenylketonuria: new treatments].

Low phenylalanine diet has been the key treatment of phenylketonuria for more than 50years, allowing efficient management of thousands of PKU patients to date. However, non-compliance exists, mainly after adolescence. A medication for PKU received approval in Europe in 2009 (sapropterine dihydrochloride or Kuvan(®)) and can benefit to patients responsive to this drug. Other treatment options are available in some countries (glycomacropeptides, large neutral amino acids) or are currently under investigation (phenylalanine ammonia lyase, chaperones molecules, gene therapy).

Sulphasalazine in the treatment of juvenile rheumatoid arthritis: a preliminary open trial.

Sulphasalazine was administered to 18 patients with juvenile rheumatoid arthritis (JRA). The mean duration of therapy was 7.8 +/- 2.6 months. There was significant improvement in all clinical variables and erythrocyte sedimentation rate. Five children developed minor and transient side effects. Medication was discontinued in 3 patients: one because of leukopenia, 2 for lack of effect. These results suggest that sulphasalazine is an effective and relatively safe drug which may prove to be a useful second line agent in the management of JRA.

Management on tsunami causing posttraumatic stress disorder: a case report.

On December 26, 2004, tsunamis hit Southeast Asia and caused serious damage and loss of lives. In Thailand, six provinces (Ranong, Phang-Nga, Phuket, Krabi, Trang, and Satun) were impacted. The present study reports the psychiatric assessments such as Thai GHQ-60 and IES. It also reports management techniques of both cognitive behavior therapy and medication. Those were provided to a Thai female patient who was 54 years old. The patient responded to treatment quickly because of early management. The tsunami victim with Posttraumatic Stress Disorder (PTSD) is not an individual. A mass of people

Overcoming adherence issues and other barriers to optimal care in gout.

PURPOSE OF REVIEW: This review presents research published over the last year examining use of urate-lowering therapy (ULT) as well as trends over time in adherence to this class of agents. Additionally, it explores factors associated with nonadherence to ULTs for chronic gout and interventions to improve chronic gout management. RECENT FINDINGS: New literature suggests prescriptions of ULTs for prevalent and incident gout patients remains lower than expected based on the burden of the disease in the population. Overall ULT adherence remains suboptimal, in part related to inadequate patient e

Bone Health ECHO Case Report: Significant Elevation in Bone Turnover Markers and Progression of Vertebral Fractures After Denosumab Discontinuation Followed by a PTH-Analog.

Bone Health ECHO (Extension for Community Healthcare Outcomes) is a virtual community of practice, where healthcare professionals have met via videoconferencing weekly since 2015. This model of learning is focused on short didactics and the presentation of real but de-identified patient cases followed by highly interactive discussions. These are often clinical situations with diagnostic and therapeutic dilemmas that are not readily addressed by randomized placebo-controlled clinical trials and clinical practice guidelines. Here we present the case of a woman with postmenopausal osteoporosis wi

Innovative Use of Limberg Flap for MRONJ Reconstruction.

Medication-related osteonecrosis of the jaw (MRONJ) is a serious complication associated with bisphosphonate therapy, characterized by necrotic bone exposure in the maxillofacial region. This case report details the surgical management of a 68-year-old female patient with a history of cancer and bisphosphonate-related osteonecrosis. After the initial sequestrectomy, the patient developed a purulent discharge, necessitating soft tissue reconstruction using a Limberg flap. The surgical procedure involved meticulous debridement and flap placement, resulting in successful healing and resolution of

[Xerostomia: etiology, diagnosis and treatment].

Salivary hypofunction caused by salivary gland disease, medication, or radiation may predispose for secondary oral mucosal disease. In these patients the protective coating of saliva is reduced or absent, leaving the oral mucosa more vulnerable. Candidiasis, burning mouth syndrome, and white lesions of oral mucosa are increased in frequency. The aim of management is to prevent oral pathological changes. The management procedure may include proper oral hygiene, saliva-stimulating agents, or saliva substitutes, depending on the severity of the salivary dysfunction. Treatment includes antifungal

[Key points of the International consensus guidelines on the implementation and monitoring of vosoritide therapy in individuals with Achondroplasia].

Achondroplasia (ACH) is a common inherited skeletal dysplasia (inherited dwarfism) that compromises quality of life across the lifespan. In 2021, vosoritide became the first approved precision therapy for ACH and is now available in more than 40 countries. Compared with prior symptomatic measures, vosoritide has demonstrated favorable efficacy and a reassuring safety profile. Nevertheless, existing international ACH guidelines largely emphasize complication management and symptomatic care, and there is no unified consensus on pharmacologic therapy. To address this gap, an international expert

Protocol of the randomized control trial: the WiseApp trial for improving health outcomes in PLWH (WiseApp).

BACKGROUND: Poor adherence to antiretroviral therapy (ART) is one of the primary barriers to viral load suppression. mHealth technology can help overcome challenges with ART adherence. This paper outlines the protocol for the WiseApp randomized control trial. The WiseApp contains real-time medication monitoring linking an electronic pill bottle and fitness tracker to the app, helping persons living with HIV (PLWH) self-manage their medication adherence and improve their overall quality of life. The primary objective of the trial is to test the effect of the WiseApp's medication adherence featu

Rebound-withdrawal headache (medication overuse headache).

Rebound-withdrawal headache (medication overuse headache) is a problem that affects 1% of the population or perhaps more. Work from many countries has shown this to be a worldwide problem. It appears that the rebound-withdrawal headache often occurs on the background of pre-existing migraine or tension headache and may assume the phenotype of chronic migraine or chronic tension-type headache. The key feature of this entity is that the patient initially uses a symptomatic treatment for headache with good result. Use of this medication may increase over time, but as the use increases to more tha
