Autosomal dominant microcephaly with mental retardation.

A 3-year-old girl, her mother, and maternal uncle had microcephaly and mental retardation. Their facial appearance is characterized by deep-set eyes, short philtrums, and a "beaked" nose. The mother and uncle live in an adult foster care facility because of mental retardation. The 3-year-old girl has a developmental quotient of 55. Mother has normal phenylalanine level and the child's chromosomes are normal. This appears to be a first report of a autosomal dominant form of microcephaly associated with mild to moderate mental retardation in contrast to absent or mild mental retardation describe

Prevalence and pattern of mental health problems.

Three hundred and forty eight children of age group 5-15 years were studied. A reporting questionnaire was administered to the parents and then affected children were examined in detail by history, physical examination and mental status examination. Fifty out of 348 children were having mental health problems. Male and first born children were affected more. Common problems observed were poor scholastic performance, enuresis, hyperkinetic syndrome, speech disorders and sleep disorders. In males, poor scholastic performance, hyperkinetic syndrome and temper tantrums were common while in females

Classification of mental disorder in primary care.

This monograph describes a study designed to test how far the two major international systems of disease classification, International Classification of Diseases (ICD) and International Classification of Health Problems in Primary Care (ICHPPC), can be consistently applied by General Practitioners (GPs) to mental disorder presenting in primary care, and to identify sources of observer variation occurring at different stages of clinical judgement. A group of 27 senior GPs was exposed to a series of real life general practice consultations, either in the form of videotape or written case-vignett

The mental health of old homeless men.

The recent focus on the young homeless mentally ill has diverted attention from other subgroups of the homeless, particularly the aging, who compose one fourth of the homeless population. A total of 86 street-dwelling and 195 non-street-dwelling (177 flophouse-, 18 apartment-dwelling) men aged 50 and older on the Bowery in New York City were studied. Although 23% evidenced psychosis or had prior psychiatric hospitalization (PPH group), depression was more prevalent with one third of all men categorized as clinically depressed. Only 5% evidenced gross organic mental disease. Several groups were

Sustained attention of adults with mental retardation.

The sustained attention of young adults with mild mental retardation and those without retardation was assessed during three 50-minute tests in which visual cues were presented successively on a computer-controlled monitor. Target cues occurred on 5% of the stimulus presentations in a random fashion. Significant decrements in target detection were observed in both groups of observers during all tests; however, the vigilance decrement was unrelated to intelligence level. The target-detection ability of observers with and without mental retardation did not differ on a recognition task. The obser

[Difficulties in the occupational incorporation of the mentally handicapped].

Psychiatric clinics report great difficulties in motivating patients, on successful completion of medical rehabilitation and vocational therapy, to file an application for vocational rehabilitation. And if they do so, the mentally ill persons more frequently then other disabled persons break off the vocational rehabilitation measures taken. After a two year discussion process, a study group in Berlin (West), which consisted of representatives of different institutions involved in vocational rehabilitation, have published a memorandum stating the central obstacles for mentally ill persons to en

Intrauterine radiation exposures and mental retardation.

Small head size and mental retardation have been known as effects of intrauterine exposure to ionizing radiation since the 1920s. In the 1950s, studies of Japanese atomic-bomb survivors revealed that at 4-17 wk of gestation, the greater the dose, the smaller the brain (and head size), and that beginning at 0.5 Gy (50 rad) in Hiroshima, mental retardation increased in frequency with increasing dose. No other excess of birth defects was observed. Otake and Schull (1984) pointed out that the period of susceptibility to mental retardation coincided with that for proliferation and migration of neur

Mental changes in subcortical arteriosclerotic encephalopathy.

We describe six patients with chronic hypertension, atherosclerotic vascular disease and periventricular leukoencephalopathy (PVL) on CT. PVL was combined with cortical infarcts in one and with lacunar infarcts in five patients. On neuropsychological examination all patients showed the characteristic features of a subcortical type of mental deterioration. Clinico-radiologic correlation suggests a direct association of PVL and subcortical dementia, and it is argued that prominent cortical features in the dementia of patients with vascular disease and PVL suggest the concomitant presence of Alzh

Mental retardation in a patient with Maroteaux-Lamy.

An 8-year-old boy with Hurler-like traits including severe mental retardation excreted in his urine a pathognomonic amount of dermatan sulfate. Less than 10% residual activity of arylsulfatase B was found in his leucocytes and fibroblasts. Hurler fibroblasts corrected an abnormally high 35SO4-incorporation into acid mucopolysaccharides (MPS) in cultured fibroblasts, whereas Maroteaux-Lamy fibroblasts did not. The patient thus has a rare combination of Maroteaux-Lamy and severe mental retardation.

Fragile 19p13 in a family with mental illness.

We describe a family where four brothers show the rare heritable folate sensitive autosomal fragile site (FSAFS) at 19p13 when cells were grown in TC199. Two of the brothers are schizophrenic while one brother is mentally retarded with autistic-like features. The clinical significance of this fragile site however is still unknown.

Mental retardation: a controversial indication for hysterectomy.

Some mentally retarded women, particularly those severely affected, may benefit from hysterectomy. Although the contraceptive effect of hysterectomy frequently may be useful, this procedure should be performed only in those women for whom menstrual hygeine is (or is anticipated to be) a major problem. A recommendation from a pediatrician or other physician who has an established relationship with the patient is a prerequisite to surgery. Likewise, informed written consent from the women's legal guardian must include a review of the risks and benefits of and alternatives to the proposed surgery

Psychiatric and behaviour disturbance in mental handicap.

This study reports on the relationship between behavioural and psychiatric perceptions of mentally handicapped subjects. A Standardised Psychiatric Interview schedule was found to characterise psychiatric problems in the mentally handicapped along eight dimensions: depression, neurasthenia, psychoticism, phobias, histrionic elation, hypochondria, mental retardation and medication effects. A Behavioural Disturbance Scale characterised disturbances along six dimensions: aggressive conduct, mood disturbance, withdrawal, antisocial conduct, idiosyncratic mannerisms and self-injury. The relationshi

Birth injury as the cause of mental retardation.

Mental retardation, on initial impression, may appear to be caused by the birth process, but may be disproved by later clinical or autopsy examination. However, information often is lacking as to the basic pathology that may have been responsible for attributing the cause of the mental retardation to traumatic birth. From a 39-year period (1944 to 1983) 1146 records were evaluated at a state hospital for mentally retarded persons to determine how the intake impression compared with the final clinical and autopsy diagnoses of perinatal damage. Clinical evaluations provided some corrections of t
