Metacarpal fractures are a relatively common hand injury that may require operative intervention to ensure adequate reduction and stabilization. The use of permanent hardware, although acceptable, may lead to complications and an increased number of surgical procedures. The use of resorbable hardware such as poly-L-lactic acid and polyglycolic acid copolymer plates and screws may circumvent some of these complications. Dislocated metacarpal V fractures, fixed by percutaneous Kirschner wires or other implants near the metacarpophalangeal joint, often show impaired movement despite excellent radiologic findings. We did a prospective study to compare the results of treatment of subcapital fractures of the fifth metacarpal bone by closed reduction and splinting or by functional treatment. Twenty-nine consecutive patients were randomly divided into the two treatment groups (functional n = 14, and reposition and splinting n = 15). The authors present a short series of 30 displaced fractures of the neck of the fifth metacarpal. All fractures were treated by K-wire nailing according to the method described by Fourcher in 1976. The good results confirm the advantages of this technique, allowing early mobilization and rehabilitation. Open fractures in children with soft tissue injuries of second and third degree generally require osteosynthesis. Minimal osteosynthesis is sufficient. Articular fractures and juxtaarticular fractures are stabilized with Kirschner wires. In shaft fractures of long bones short SAO plates are used in most cases. For shaft fractures of the upper extremity alternatively intramedullary osteosynthesis with Kirschner wires or Rush pins may be applied. Indication, osteosynthesis, and operating technique are discussed.
