Labor Intervention and Outcomes in Women Who Are Nulliparous and Obese: Comparison of Nurse-Midwife to Obstetrician Intrapartum Care.

BACKGROUND: Women who are obese have slower labors than women of normal weight, and show reduced response to interventions designed to speed labor progress like oxytocin augmentation and artificial rupture of membranes. The optimal labor management for these women has not been described. METHODS: This retrospective cohort study compared 2 propensity score-matched groups of women (N = 360) who were healthy, nulliparous, spontaneously laboring, and obese (body mass index ≥ 30 kg/m2 ). Labors were managed by either a certified nurse-midwife (CNM) or an obstetrician at one hospital from 2005 thro

Three-year follow-up for virtual reality exposure for fear of flying.

Thirty participants who had been treated for aviophobia with virtual reality graded exposure therapy with physiological monitoring and visual feedback (VRGETpm), virtual reality graded exposure therapy with physiological monitoring only (VRGETno), or imaginal exposure therapy (visualization) with physiological monitoring only (IET) between January 1998 and January 1999 were contacted in January 2002 for a 3-year posttreatment follow-up assessment. Of the participants in the VRGETpm group who had flown successfully by the end of treatment, all had maintained their ability to fly at follow-up. O

Muscle-Specific Differences in Neuromuscular Block and Quantitative Neuromuscular Monitoring: A Narrative Review.

Neuromuscular blocking agents are an essential component of contemporary anesthesiology. Adequate intraoperative neuromuscular monitoring is recommended to ensure sufficient depth of blockade and prevent postoperative residual neuromuscular block. Several quantitative techniques have been developed to assess neuromuscular transmission at the neuromuscular junction, and the number of clinically relevant monitoring sites has expanded beyond conventional peripheral muscles. In routine practice, train-of-four (TOF) stimulation combined with acceleromyography is most commonly used; the adductor pol

Implantable Intracranial Pressure Sensor with Continuous Bluetooth Transmission via Mobile Application.

Hydrocephalus is a clinical disorder caused by excessive cerebrospinal fluid (CSF) buildup in the ventricles of the brain, often requiring permanent CSF diversion via an implanted shunt system. Such shunts are prone to failure over time; an ambulatory intracranial pressure (ICP) monitoring device may assist in the detection of shunt failure without an invasive diagnostic workup. Additionally, high resolution, noninvasive intracranial pressure monitoring will help in the study of diseases such as normal pressure hydrocephalus (NPH) and idiopathic intracranial hypertension (IIH). We propose an i

Feasibility and Safety of Integrating Extended TCD Assessments in a Full Multimodal Neuromonitoring Protocol After Traumatic Brain Injury.

OBJECTIVE: Targeting single monitoring modalities such as intracranial pressure (ICP) or cerebral perfusion pressure alone has shown to be insufficient in improving outcome after traumatic brain injury (TBI). Multimodality monitoring (MMM) allows for a more complete description of brain function and for individualized management. Transcranial Doppler (TCD) represents the gold standard for continuous cerebral blood flow velocity assessment, but requires high levels skill and time. In TBI, the practical aspects of conducting extended TCD monitoring sessions have yet to be evaluated. METHODS: Pa

A methodological framework for validating a multi-domain physiological sensor for divers using a scalable data fusion platform.

Divers face numerous physiological hazards that can lead to diving disorders, yet the underlying mechanisms remain poorly understood due to limitations in monitoring technologies capable of functioning underwater. To bridge this gap, it is necessary to critically evaluate novel monitoring technologies and develop approaches to determine their suitability for near real-time physiological assessment. Gold standard devices are typically used to validate new physiological sensors under controlled normobaric conditions, where their accuracy benefits from stable, low-interference environments. Howev

Personalized treatment approaches in neurocritical care.

Acute brain injuries-including traumatic brain injury, subarachnoid hemorrhage, and intracerebral hemorrhage-exhibit profound pathophysiological heterogeneity, yet are often managed using standardized treatment protocols. While evidence-based guidelines have improved outcomes at a population level, they frequently overlook patient-specific variations in cerebral compliance, autoregulation, and metabolic reserve. This review explores the evolving paradigm of personalized neurocritical care, which integrates dynamic multimodal monitoring, individualized intracranial pressure management strategie

Glucose monitoring and insulin administration in the pregnant diabetic patient.

Over the past decade, it has been apparent that the degree of glycemic control achieved in the pregnant patient with diabetes mellitus significantly affects perinatal outcome. While several studies have documented excellent perinatal outcome with good glucose control, the incidence of neonatal morbidity still remains substantially greater than that observed in the general population. Physiologic or "tight" control appears to further reduce the incidence of macrosomia, hypoglycemia, and other indices of neonatal morbidity. The application of home glucose-monitoring techniques to pregnancies com

How to integrate physiological data from wearables in treatment of personality disorders: a narrative review.

In recent years, stress-monitoring innovations using wearable technology have entered the market. One innovation is biocueing, a process where patients receive real-time feedback on passive monitoring of significant changes in their physiological data, such as (additional) heart rate, heart rate variability or skin conductance. This technology offers potential for patients with borderline personality disorder, as they often report severe stress, difficulties in emotion regulation and low levels of emotional- and body awareness. Yet, currently there is no clear direction on when and how to fit

Home care artificial heart monitoring system via internet.

The availability of a remote management system, which provides both physiological-related information about the patient and device-related information about the implanted device, would be helpful during in vivo experiments or clinical trials involving artificial heart implantation. In order to be able to monitor the course of the in vivo experiment continuously regardless of the patient's location, an internet-based remote monitoring system was developed, which can monitor physiological-related information such as pressure (AoP, LAP, RAP, PAP) and flow data, as well as device-related informati

Advanced hemodynamic monitoring of the patient with acute heart failure and cardiogenic shock.

PURPOSE OF REVIEW: To provide a practical, physiology-driven framework for advanced hemodynamic monitoring in patients with acute heart failure (AHF) and cardiogenic shock, highlighting and supporting clinicians in integrating monitoring tools into an individualized bedside approach. RECENT FINDINGS: Cardiogenic shock is increasingly recognized as a heterogeneous syndrome involving not only impaired cardiac output, but also systemic inflammation, vasoplegia, tissue hypoperfusion and multiorgan dysfunction. Recent studies have emphasized phenotype-based approaches for both diagnosis and treatm

Fetal heart rate monitoring during fetoscopic repair of open spinal neural tube defects: a single-centre observational cohort study.

BACKGROUND: During fetal surgery, the haemodynamic stability of the fetus depends on the haemodynamic stability of the mother. The primary objective of this study was to assess changes in fetal heart rate (FHR) throughout the different stages of surgery. The secondary objective was to assess potential changes in maternal physiological parameters and their association with FHR. METHODS: This was a single-center observational cohort study conducted between 2015 and 2019 in 26 women undergoing intra-uterine fetoscopic repair of open spina bifida. The primary outcome was FHR. Maternal physiologic

Monitoring patients receiving dopamine agonist therapy for hyperprolactinaemia.

The surveillance strategy for patients taking low dose cabergoline for hyperprolactinaemia is controversial. As more evidence has emerged that the risks of cardiac valvulopathy in this population of patients are low, fewer and fewer endocrinologists adhere strictly to the original medicines and healthcare products agency MHRA guidance of "at least" annual echocardiography. Strict adherence to this guidance would be costly in monetary terms (£5.76 million/year in the UK) and also in resource use (90,000 extra echocardiograms/year). This article reviews the proposed pathophysiological mechanism

Management of a parturient with a history of local anesthetic allergy.

The management of parturients with a strong history of allergy to local anesthetics poses significant challenges to the obstetric anesthesiologist. We recommend that when such patients have a strong desire to receive labor analgesia with local anesthetics, they undergo provocative challenge testing with preservative-free bupivacaine performed in labor and delivery with preparations for emergent cesarean delivery after 24-wk gestation.

HOPFIT: A neuro-cardiovascular monitoring framework for precision sedation, anaesthesia, and resuscitation.

The increasing availability of advanced monitoring technologies in anaesthesia and critical care has enabled continuous assessment of neurological, haemodynamic, and oxygen delivery parameters. Despite this, integrating multiple data streams into coherent, physiology-guided clinical decisions remains challenging, and unified interpretive frameworks remain limited. To introduce the HOPFIT framework - a conceptual model linking continuous neuro-cardiovascular monitoring to structured, physiology-guided clinical reasoning across six interdependent domains. HOPFIT organises six therapeutic domains

Monitoring Sleep and Nightly Recovery with Wrist-Worn Wearables: Links to Training Load and Performance Adaptations.

Previous studies on the effects of intensified training on sleep quality/quantity have been somewhat contradictory. Moreover, recreational athletes often track various sleep metrics, and those metrics' actual connections to training adaptations are unknown. This study explored the effects of intensified training on sleep and nightly recovery along with their associations with training adaptations. A total of 24 participants (10 females) performed a 3-week baseline training period (BL), a 2-week overload period (OL), and a 1-week recovery period (REC), which were followed by test days (T1-T3).
