The use of intravenous immunoglobulin (IVIg) in the management of patients with neuroimmune disorders has shown a progressive trend over the last few years. Despite the wide use of IVIg, consensus on its optimal use is deficient. The European Federation of Neurological Societies (EFNS) guidance regulations offer consensus recommendations for optimal use of IVIg. The effectiveness of IVIg has been proven in Guillain-Barré syndrome (level A), chronic inflammatory demyelinating polyradiculoneuropathy (level A), multifocal mononeuropathy (level A), acute exacerbations of myasthenia gravis and shor

OBJECTIVE: The objective of this study was to investigate the effectiveness of splinting, ultrasound (US), and low-level laser (LLL) in the management of carpal tunnel syndrome (CTS).
BACKGROUND DATA: CTS is the entrapment mononeuropathy most frequently seen in clinical practice, caused by compression of the median nerve at the wrist. Although several treatment modalities are routinely in use, there is no consensus about the best way to manage CTS.
MATERIALS AND METHODS: In our study, patients were randomly allocated to three groups that received the following treatment protocols: splinting

Despite high-dose intravenous immunoglobulin (IVIG) is widely used in treatment of a number of immune-mediated neurological diseases, the consensus on its optimal use is insufficient. To define the evidence-based optimal use of IVIG in neurology, the recent papers of high relevance were reviewed and consensus recommendations are given according to EFNS guidance regulations. The efficacy of IVIG has been proven in Guillain-Barré syndrome (level A), chronic inflammatory demyelinating polyradiculoneuropathy (level A), multifocal mononeuropathy (level A), acute exacerbations of myasthenia gravis (

A variety of peripheral neuropathies are associated with human immunodeficiency virus (HIV) infection. Although the incidence of certain forms of neuropathy is increased in HIV infection, in other cases, the association may be fortuitous. Different forms of peripheral neuropathy occur with increased frequency at particular stages of HIV disease. For example, inflammatory demyelinating neuropathy (IDP) is often the first manifestation of HIV disease, when CD4 lymphocyte counts are relatively high. As immunosuppression progresses and HIV viral load becomes uncontrolled, the incidence of distal s

OBJECTIVE: To assess the viability of sympathetic sudomotor fibers in carpal tunnel syndrome (CTS).
METHODS: We recorded sympathetic skin response (SSR) with a multichannel recording system. Forty-four patients with CTS (51 hands), 7 patients (7 hands) with asymptomatic median mononeuropathy at the wrist (MMW), and 20 normal subjects (20 hands) were studied. We classified the patients into 4 grades of increasing severity.
RESULTS: In the hands of normal subjects, SSR was evoked easily at all recorded sites. SSR at the wrist in patients with asymptomatic MMW decreased in amplitude. SSR was ma

INTRODUCTION: Cyclical sciatica due to implantation of endometrial tissue in the sciatic nerve in the region of the sciatic notch is a very unusual cause of sciatica. It occurs in women of childbearing age, as episodes of pain in the distribution of the sciatic nerve, which present in a cyclic manner and coincide with menstruation. If it is not treated, a sensomotor mononeuropathy of the sciatic nerve develops.
CLINICAL CASE: The patient had complained of right-sided sciatic pain from the age of 36 years. Over the years a motor deficit had slowly and progressively appeared causing foot drop.

Criteria for the classification of polyarteritis nodosa were developed by comparing 118 patients who had this disease with 689 control patients who had other forms of vasculitis. For the traditional format classification, 10 criteria were selected: weight loss greater than or equal to 4 kg, livedo reticularis, testicular pain or tenderness, myalgias, mononeuropathy or polyneuropathy, diastolic blood pressure greater than 90 mm Hg, elevated blood urea nitrogen or serum creatinine levels, presence of hepatitis B reactants in serum, arteriographic abnormality, and presence of granulocyte or mixed

BACKGROUND: Cytomegalovirus (CMV) reactivation with neurological involvement in patients with acquired immunodeficiency syndrome (AIDS) is increasingly rare since the introduction of antiretroviral therapy (ART). Manifestations include encephalitis, myelitis, polyradiculopathy and, less commonly, mononeuritis multiplex (MNM). We report a case of disseminated CMV disease with gastrointestinal and peripheral and central nervous system involvement in a patient with AIDS, manifesting primarily as MNM.
CASE PRESENTATION: A 31-year old woman with AIDS presented with a clinical picture of MNM. Elect

A 76-year-old female patient presented with a progressive motor-sensory multiplex mononeuritis (MM). Combined muscle and nerve biopsy showed the typical findings of a polyarteritis nodosa (PAN). Despite treatment with corticosteroids paresthesias increased and purpura of the legs newly appeared. Hepatitis screening revealed chronic hepatitis C-infection associated with cryoglobulinemia Type II (IgM-kappa Ig A). Finally, we diagnosed a hepatitis C-associated cryoglobulinemic vasculitis based on clinical and laboratory findings.

Twelve consecutive patients with long thoracic nerve palsy, who underwent transfer of the pectoralis major muscle for dynamic stabilization of the scapula, were reviewed. Direct transfer of the tendon to the lateral margin of the inferior scapular angle was performed with a bony chip from the tendon's insertion. All patients were followed up clinically by evaluation of the Constant score and radiographically with magnetic resonance imaging for an average of 92.5 months (range, 60-136 months). The mean Constant score improved from 41 to 85.4 points. Mean active flexion increased from 89 degrees

BACKGROUND: Pure neuritic variety of leprosy (PNL) presents as peripheral neuropathy with absent skin lesions and negative skin smears. Diagnosing PNL is an uphill task as most of these patients have nonspecific changes on nerve biopsy. In such circumstances, additional molecular diagnostic tools like polymerase chain reaction (PCR) has proven to be useful in diagnosing leprosy. The present study was planned to evaluate the role of PCR in nerve biopsy specimens of patients with PNL.
METHODS: Patients attending the neuromuscular clinic from January 2013 to June 2014 with mononeuropathy multipl

Local tumor compression is the main mechanical cause of posterior interosseous nerve (PIN) palsy. The reported cases of these tumors do not include that of pigmented villonodular synovitis (PVNS). Here, we report a case of a 53-year-old male with a 9-year history of painless swelling in his left elbow and a few months of progressive weakness in his left hand. Imaging identified the mass, and histological examination of the biopsy specimens revealed PVNS. The mass was compressing the nerve at the arcade of Frohse, and we performed a complete resection of the mass. Following removal of the mass,

INTRODUCTION: Hypereosinophilic syndrome is produced by what is usually a multiple infiltration of eosinophils into tissues, and may be secondary or idiopathic, depending on whether it is related to a specific aetiology or not. It is not uncommon for it to include nerve disease, but it is unusual for it to do so in the form of multineuritis. Exceptionally, pathogenesis into multiple mononeuritis appears to be related with neurotoxicity due to products derived from eosinophils rather than with infiltrating or inflammatory phenomena. This study describes the case of a female patient with hypereo

Neurological complications frequently affect chronic kidney disease patients. They are important causes of morbidity and mortality. We present a review of neurological complications affecting hemodialysis patients, focusing on classical and new aspects. Neurological complications can be classified as central or peripheral, but they also include other conditions such as muscle and autonomic disorders. Neurological complications in hemodialysis patients are often underdiagnosed and undertreated. Dialysis treatment can modify the clinical pattern and the course of neurological complications, but

Cancer polyarthritis is a very rare condition. Here, we present a case of cancer polyarthritis, who also had mononeuritis simplex as a manifestation of paraneoplastic neuralgic syndrome. A 71-year-old man, who initially presented symmetrical polyarthritis and unilateral posterior interosseous nerve palsy, was subsequently diagnosed to have gastric cancer. Total gastrectomy was performed, and his polyarthritis and the palsy simultaneously disappeared within 2 weeks after the resection. His gastric cancer was found to be metastasized to his liver 16 months after the total gastrectomy; however, t

Acute pancreatitis stemming from IgG4-Related Disease (IgG4-RD) seldom coincides with Systemic Lupus Erythematosus (SLE), highlighting the importance of investigating autoimmune conditions in patients with IgG4-RD. We present the case of a 57-year-old male with a medical history notable for hypertension, photosensitivity, arthritis, and malar rash, who presented with 6 weeks of persistent epigastric pain. Computed Tomography (CT) of the abdomen revealed hallmark features such as fat stranding around the pancreatic tail and gallbladder wall thickening, confirming the diagnosis of acute pancreat

Intraneural ganglion cysts (IGCs) are benign, mucinous lesions that are often found within the epineurium of peripheral nerves, causing mononeuropathy due to nerve compression. These cysts arise from synovial fluid tracking from an adjacent synovial joint along an articular nerve branch to the parent nerve, as proposed by the unifying articular theory. This theory has transformed the understanding of IGCs from their being considered as isolated nerve lesions but as joint-related pathologies. The emphasis of identifying the cyst's origin and its connection to the neighboring joint has shifted t
