Nutrition following gastric operations for morbid obesity.

Nutritional status after 238 gastric operations designed to reduce caloric intake and body weight to within 30% of ideal was assessed by measuring body composition using the multiple isotope dilution technique. Body cell mass (BCM) and body fat were quantitated before and at 24 months after operation. Malnutrition was defined as a total exchangeable sodium (Nae) to total exchangeable potassium (Ke) ratio greater than 1.22. Data were collected on 96 patients. All had lost a mean of 26% of preoperative weight by 24 months. Significant malnutrition occurred in 47 patients whose Nae/Ke ratio range

Surgical treatment of morbid obesity with reflux esophagitis.

Experience with six morbidly obese patients with symptomatic reflux esophagitis associated with hiatal hernia, the diagnoses confirmed by upper GI x-ray contrast studies and endoscopy, has been reported. Truncal vagotomy with Heineke-Mikilicz pyloroplasty and gastric bypass with Roux-en-Y gastrojejunostomy has controlled all symptoms of esophagitis and produced effective weight loss. It is suggested that if the proximal gastric pouch is no larger than 25 ml there is no need to perform truncal vagotomy and pyloroplasty.

The mortality and morbidity of young survivors of myocardial infarction.

The mortality and morbidity of 150 young (less than 45 years) survivors of myocardial infarction was studied prospectively over a 10-year period. Thirty-seven deaths (36 due to coronary artery disease) occurred from one month to 10 years after myocardial infarction and the cumulative 10-year mortality was 27 per cent compared with 5 per cent in the general population. High initial lactate dehydrogenase levels and continued cigarette smoking after infarction were associated with a poor long-term survival. Angina was present at some time during follow-up in 60 per cent of patients. Severe limita…

Gastrointestinal bleeding in the elderly. Morbidity, mortality and cause.

One hundred thirty-six consecutive patients with upper gastrointestinal bleeding were divided by age into three groups, young, middle aged and elderly, and evaluated for the causes and complications of the bleeding episode. Hemorrhagic gastritis was the most frequent source of bleeding in the young, while gastric ulcer was more common in the middle aged and elderly groups. The high mortality in the young (20 percent) was often attributable to associated liver functional abnormalities secondary to alcoholism. The elderly fared better than the young when the source of bleeding was hemorrhagic ga

Tuberculosis after intestinal bypass for morbid obesity.

Tuberculosis of organs other than the lung may occur after an intestinal bypass operation for morbid obesity, with an incidence varying from 1% to 4%, a value rather higher than that of the general population. As its clinical symptoms (fever and chills, abundant sweating and an increase or return of weight loss) appear during the period of greatest weight loss, it is probably caused by malnutrition and malabsorption. In most cases lymphadenopathy (usually cervical) also appears. Tuberculosis occurring after bypass operation should be treated with the classic antitubercular therapy; this always

[Analysis of the trends in typhoid-paratyphoid morbidity in the USSR].

Mathematico-statistical analysis of typhoid-paratyphoid infection morbidity in the USSR during 1900--1940 revealed a regular many-year cyclic recurrence of the changes in the morbidity level. The intensity of the morbidity level reduction at the periods of decline--in 1900--1917, 1920--1928, and in 1932--1939--was similar. A successive reduction of morbidity of this group of infections, by 6.45% on the average, annually, occurred in 1930--1976. The character of reduction of typhoid-paratyphoid infections morbidity level in the USSR during that period is well described by the exponential functi…

Morbid changes of the mandible in familial polyposis of the colon.

The study was made to ascertain whether the radiographs of the mandible of patients with familial polyposis of the colon show osteomas. Orthopantomography of the mandible was taken in 10 cases of familial polyposis of the colon and 6 cases of the unaffected family members. And radiographic and histological studies were performed in another autopsied case. As a result, radiographic studies revealed sclerotic masses of the mandible in 10 of the 11 cases. In some cases, the changes were also found in the maxilla. The number of sclerotic masses ranged from 2 to 11, and they were associated with od

The role of chronogenetics and of gemellology in the prevention of genetic morbidity.

The prevention of hereditary diseases after birth is essentially based on the detection of these diseases at their preclinical level. This may be made possible through a careful pedigree examination and possible identification of a "genealogical alarm", as well as a detailed clinical examination of the subject and possible identification of preclinical signs of the pathogenesis, i.e., of a "clinical alarm". The respective roles of chronogenetics and of gemellology in this approach are reviewed.

Perinatal and neonatal mortality and morbidity in Lusaka, 1976.

A prospective study of perinatal and neonatal mortality and morbidity at the University Teaching Hospital (UTH), Lusaka for 1976 is presented. The early neonatal mortality of the babies born in hospital was 28.7 per 1000 live-births; it was 239.96 per 1000 admissions of those born outside the hospital. The still-birth rate in the hospital-born babies was 25.2 per 1000 deliveries; the perinatal mortality was 53.3 per 1000 deliveries; and the neonatal mortality in the hospital-born was 31.1 per 1000 live-births. The cause of death were asphyxia, infections could injury, respiratory distress synd…

Morbidity and mortality of mandibular resection for malignant disease.

Twenty-nine per cent of patients undergoing mandibular resection had an entirely uncomplicated postoperative course. Local complications occurred in 54 per cent and systemic complication in 35 per cent. Increasing the extent of resection or an additional procedure, such as radical neck dissection, increased the local complication rate. Operation in fully irradiated tissue produced an increase in the proportion of life-threatening complications. Systemic complications were responsible for 80 per cent of the postoperative deaths.
