Various anatomical variants in pulmonary veins can have a serious effect on patients undergoing lung surgery. We present a case of a patient with an unusual pulmonary vein variation. Preoperative review of the patient's three-dimensional 64-row multidetector computed tomography imaging allowed us precise simulation and good orientation of the patient's vascular variant anatomy during surgery. Upper lobectomy through thoracoscopic approach was performed successfully in the case where the middle lobe vein might have been divided without preoperative anatomical evaluation by 3D CT images.

Papillary fibroelastoma (PFE) is a benign, rare, gelatinous tumour derived from the endocardium, primarily the cardiac valves, which is usually diagnosed by high resolution echocardiography. Although rarely clinically symptomatic, PFEs have a potential for coronary ischaemia, systemic embolisation with neurologic symptoms, and sometimes valvar dysfunction. There are reports of coronary occlusion and even sudden cardiac death due to a ball valve phenomenon on the coronary ostia. Leptospirosis is a zoonotic infection caused by the Leptospira interrogans. Although it is endemic in tropical countries, global incidence has increased in several temperate and developed regions. Here, we present a cardiac magnetic resonance (CMR) and multidetector computer tomography (MDCT) chest features of active systemic leptospiral infection in a 19-year-old male. The MDCT appearances of lungs and CMR appearances of myocardium in icteric leptospirosis are described. Early diagnosis and prompt treatment is important to manage the cardiothoracic complications.
