Infection of a pancreatic pseudocyst due to Candida albicans.

A 40-year-old man with diabetes mellitus, congestive heart failure, alcoholic cirrhosis, and chronic pancreatitis had an exacerbation of pancreatitis due to alcohol abuse. His condition deteriorated rapidly with development of apparent sepsis; cultures were negative. He slowly improved with multiple antibiotic therapy and total parenteral nutrition. Serial imaging of the pancreas revealed edematous pancreatitis that evolved initially into a phlegmon and later into multiple pseudocysts. Intermittent fever prompted computed-tomography-directed percutaneous aspiration of the largest pancreatic fl

Diagnosis and treatment of muscle contraction (tension-type) headaches.

Acute tension-type headache is a very common condition that rarely is a problem in treatment. Chronic tension-type headache, however, is often a difficult therapeutic problem. The pathogenesis is not well understood, but both peripheral muscle contraction and central pain-modulating systems are probably involved. Therapy usually works best when multiple techniques are used, including pharmacologic, psychological, and physiologic modalities.

Sensitization, subjective health complaints, and sustained arousal.

The purpose of this presentation is to discuss the possibility that sensitization is a psychobiological mechanism underlying not only multiple chemical sensitivity (MCS), but a much more general cluster of illness, referred to as "subjective health complaints". Sustained arousal, or sustained "stress" responses, may be an important factor for the development of these conditions. Patients with subjective complaints without objective changes are sometimes referred to as having "fashionable diagnoses" or "unexplained symptoms". They may be given diagnoses like MCS, epidemic fatigue, chronic fatig

Arthritis and increased risk of cardiovascular disease: is there an interaction with other chronic diseases? A nationwide cohort study.

BACKGROUND: The relationship between arthritis and cardiovascular disease (CVD) remains controversial. This study aimed to investigate whether arthritis constitutes an independent risk factor for CVD and to evaluate its potential synergistic or antagonistic interactions with other chronic conditions in predicting CVD onset. METHODS: We used China Health and Retirement Longitudinal Study (CHARLS) data (2011-2020). Multivariate Cox regression models were employed to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) for associations between arthritis and self-reported physician-dia

A technique for re-utilizing catheter insertion sites in children with difficult central venous access.

Maintenance of central venous access in patients with chronic medical conditions such as short bowel syndrome demands forethought and ingenuity. We describe an innovative technique for re-utilizing central venous access sites in patients who have chronic central venous access needs. Records of patients undergoing this technique were reviewed between August 2012 and December 2015. The technique involves "cutting-down" to the sterile fibrous tunnel that naturally forms around tunneled catheters. The fibrous sheath is then isolated and controlled much as would be done for a venous "cut-down." A s

Medial septal GABAergic neurons reduce seizure duration upon optogenetic closed-loop stimulation.

Seizures can emerge from multiple or large foci in temporal lobe epilepsy, complicating focally targeted strategies such as surgical resection or the modulation of the activity of specific hippocampal neuronal populations through genetic or optogenetic techniques. Here, we evaluate a strategy in which optogenetic activation of medial septal GABAergic neurons, which provide extensive projections throughout the hippocampus, is used to control seizures. We utilized the chronic intrahippocampal kainate mouse model of temporal lobe epilepsy, which results in spontaneous seizures and as is often the

[Effect of deep brain stimulation on glottal phonation in patients with Parkinson's disease and multiple sclerosis].

The present study examines the effect of neurostimulatory operations on glottal phonation of 3 parkinsonian patients and 3 patients with multiple sclerosis. With the help of two voice analysis programs (MDVP from Kay Elemetrics and EEG Program by Marasek) for the acoustic and electroglottographic definition of voice characteristics, vowel productions of the patients, which were recorded under two conditions (with and without stimulation), were analysed. In a first step, significantly different intrasubject means in the two conditions indicate the effect of neurostimulation. The strength of the

The Interaction of Pain Catastrophizing and Chronic Pain Acceptance in Sickle Cell.

PURPOSE: Chronic pain in sickle cell disease is more complex and challenging to manage than in other conditions. To better inform chronic pain assessment and management, this study evaluated the role of chronic pain acceptance in the context of pain catastrophizing and their combined association with chronic pain intensity and chronic pain disability in adults with sickle cell disease. DESIGN: This was a secondary analysis of cross-sectional survey data from 170 adults with sickle cell disease (mean age of 28.05 years and 53.5% female, with moderate to severe chronic pain and high levels of c

[Research progress in association of neurological disorders and periodontal diseases].

Periodontal diseases are inflammatory diseases caused by oral pathogens around the periodontal supporting tissues, leading to systemic and chronic inflammatory conditions. The continuous chronic systemic inflammation may be a trigger of neuroinflammation, which is the prominent feature of a variety of neurological disorders. It implies that there may be a causal link between periodontal diseases and neurological disorders. This article presents epidemiological and biological evidences that periodontal diseases can induce or exacerbate neurological disorders, including Alzheimer's disease, Park

Laryngeal Sarcoidosis in a Child: Case Report.

Sarcoidosis is a chronic, multisystemic, granulomatous disorder. Our patient was a 2-year-old girl with multiple airway conditions and a partial response to inhaled and systemic steroids. She was positive for acute phase reactants and negative for antibodies. Polymerase chain reaction revealed atypical Mycobacteria and she was negative for Mycobacterium tuberculosis. Laryngeal sarcoidosis was diagnosed by histopathology in a biopsy of larynx that revealed a chronic granulomatous inflammatory process with Langhans giant cells and acute and ulcerated areas with changes compatible with tuberculos

Associations of Reproductive Health Concerns, Health-Related Quality of Life, and Stress Among Adolescents and Young Adults with Chronic Illness.

OBJECTIVE: Adolescents and young adults (AYA) living with chronic illness are as sexually active and interested in having children as healthy peers. Few studies have examined the intersection of managing chronic illness and concerns about realizing reproductive goals, including fertility, pregnancy, childbirth, and parenting. Even less is understood regarding how these concerns might be associated with psychological distress. This study examines associations between the reproductive health concerns (RHC) and health-related stress of AYA patients living with chronic conditions including rheumat

[Musculoskeletal Complications in Rheumatic Diseases].

Rheumatic diseases comprise a heterogeneous group of disorders that affect not only the joints but also multiple organ systems throughout the body. Because of chronic inflammation and immunosuppressive therapies, these conditions are often associated with a wide range of musculoskeletal complications. Herein, we discuss common musculoskeletal complications in patients with rheumatic diseases, including osteoporosis, non-traumatic tendon rupture, osteonecrosis, infection, growth retardation, and macrophage activation syndrome. As these complications may be clinically indistinguishable from dise

Efficacy of Addiction Pharmacotherapy in Alcohol Use Disorder and Their Effects on Liver Health.

Both alcohol-associated liver disease (ALD) and metabolic dysfunction-associated steatotic liver disease are leading contributors to chronic liver diseases. These conditions often coexist, exacerbating disease progression. Despite ALD being a leading cause of liver transplantation, many individuals with alcohol use disorder (AUD) do not receive treatment. In this review, we discussed the epidemiology of ALD in AUD, various treatment options for AUD, and their efficacy on liver health. Our critical analysis of current evidence underscores the need for integrated models involving multiple stakeh

Recurrent Multiple Squamous Cell Carcinomas on the Scalp in a Patient with Juvenile Dermatomyositis.

Both long-term administration of immunosuppressive agents and chronic inflammatory conditions, such as autoimmune disease, could be risk factors for the development of cutaneous squamous cell carcinoma (cSCC). In this report, we present a case of recurrent multiple cSCC on the scalp in a patient with juvenile dermatomyositis who had been administered cyclosporine and Predonine since she was a 1-year-old infant. Interestingly, immunohistochemical staining revealed IL-17-producing cells adjacent to IL-17R-expressing atypical keratinocytes. Our present case suggested that IL-17/IL-17R signaling m

[Rheumatoid arthritis and bone -periarticular and systemic bone loss-].

In rheumatoid arthritis (RA) , the osteoclast pathway is activated by an abnormal immune condition accompanied by chronic inflammation, resulting in periarticular osteoporosis and local bone destruction around joints. In addition, multiple factors including pharmacotherapies such as steroids, and reduced physical activity, lead to systemic osteoporosis. These conditions expose patients to increased fracture risk. In RA treatment, it is important to achieve suppression of fracture risk by controlling inflammation, which is associated with periarticular osteoporosis and bone destruction, using d

Management of severe pediatric tourette syndrome resistant to drug treatment.

Tourette syndrome (TS) is a neurodevelopmental disorder with chronic and disabling impacts on multiple domains of functioning in children. Treatment of TS is often complicated by comorbid conditions. We present a 12-year-old boy with severe symptoms of Tourette syndrome (TS), with prominent and markedly disabling vocal tics, who failed to respond to substantial doses of risperidone and haloperidol, given for a prolonged period. Satisfactory outcome was achieved with exposure and response prevention, adjunct to medication. Comorbid ADHD was treated with stimulants with no exacerbation of tics.

[Alopecia mucinosa].

Epithelial mucinosis (follicular mucinosis) is a reactive histological pattern characterized by the accumulation of mucin in the infundibular, follicular, and sebaceous epithelium, that may occur in multiple skin conditions and cannot be considered a specific disease. Alopecia mucinosa is a clinicopathological entity with distinctive skin lesions, mucin deposits in the hair follicles and periadnexal lymphoid infiltrates. Three types of mucinous alopecia have been described: idiopathic, persistent or benign chronic, and associated to lymphoma. For some authors, these types of mucinous alopecia

Occurrence of IgA subclasses (IgA1 and IgA2) in the human nervous system. Correlation with disease.

The occurrence of IgA subclasses in pathological conditions of the nervous system was studied by means of monoclonal antibodies and an indirect immunofluorescence technique. IgA1- and/or IgA2-positive lymphoid (plasma) cells were found in demyelinating diseases comprising multiple sclerosis, Guillain-Barré syndrome, and adrenoleukodystrophy, in various inflammatory diseases, and in tumors, some of which exhibited labeling of tumor cells. Demyelinating and inflammatory diseases with chronic course displayed some prevalence of IgA2-, and tumors some prevalence of IgA1-positive cells. This is the
