How to design a therapeutic study in patients with the post-polio syndrome. Methodological concerns and status of present therapies.

The innervation of muscles in patients with the post-polio syndrome (PPS) may differ from limb to limb or even within the same limb because of the segmental nature of the initial involvement and the varying degree of subsequent recovery. Consequently, the compensatory effort of the neighboring muscles varies even in the same limb. Clinicohistological studies have confirmed that in PPS the various muscle groups differ according to whether they were affected during the acute polio and have recovered (partially or completely), or whether they were clinically spared during the original disease, in

Fatigability, relaxation properties, and electromyographic responses of the human paralyzed soleus muscle.

1. The goal of this study was to characterize the fatigability, contractile relaxation properties, electrophysiological responses, and histochemical properties of the human paralyzed soleus muscle to determine its relative plasticity. 2. Acute (< 6 wk, n = 3) and chronic (> 1 yr, n = 10) paralyzed individuals had the tibial nerve activated with a 20-Hz square wave delivered for 330 ms every second for 4 min. The soleus muscle peak torque, one-half relaxation time (1/2RT), normalized maximum rate of relaxation (nMRR), and mass muscle action-potential amplitude (M wave) were computed every 30 s.

The athlete with muscular cramps: clinical approach.

Muscle cramps are involuntary, painful, spasmodic contractions of the skeletal muscle. Although cramps are a common clinical complaint, their etiology and management have not been well established. Exercise-associated muscle cramps occur during or immediately following exercise, and they are associated with muscular fatigue and shortened muscle contraction. The main challenges for treating physicians are to identify whether the complaint represents a true muscle cramp as well as to rule out the presence of an underlying serious clinical condition. Muscle cramps may be a symptom of any of sever

The effects of forearm fatigue on baseball fastball pitching, with implications about elbow injury.

This study investigated the contribution of flexor muscles to the forearm through fatigue; therefore, the differences in forearm mechanisms on the pitching motion in fastball were analysed. Fifteen baseball pitchers were included in this study. Ultrasonographical examination of participants' ulnar nerve in the cubital tunnel with the elbow extended and at 45°, 90° and 120° of flexion was carried. A three-dimensional motion analysis system with 14 reflective markers attached on participants was used for motion data collection. The electromyography system was applied over the flexor carpi ulnari

Effect of acetazolamide and methazolamide on diaphragm and dorsiflexor fatigue: a randomized controlled trial.

Acetazolamide, a carbonic anhydrase (CA) inhibitor used clinically and to prevent acute mountain sickness, worsens skeletal muscle fatigue in animals and humans. In animals, methazolamide, a methylated analog of acetazolamide and an equally potent CA inhibitor, reportedly exacerbates fatigue less than acetazolamide. Accordingly, we sought to determine, in humans, if methazolamide would attenuate diaphragm and dorsiflexor fatigue compared with acetazolamide. Healthy men (dorsiflexor: n = 12; diaphragm: n = 7) performed fatiguing exercise on three occasions, after ingesting acetazolamide (250 mg…

Increased catalase expression improves muscle function in mdx mice.

It has been well established that oxidative stress contributes to pathology associated with Duchenne muscular dystrophy (DMD). I hypothesized that overexpression of the antioxidant enzyme catalase would improve muscle function in the mdx mouse, the mouse model of DMD. To test this hypothesis, neonatal mdx mice were injected with a recombinant adeno-associated virus driving the catalase transgene. Animals were killed 4 or 6 weeks or 6 months following injection. Muscle function was generally improved by catalase overexpression. Four weeks following injection, extensor digitorum longus specific

Integrating electromyography into the physiology curriculum.

Muscle physiology is included in the core curriculum in the biomedical, health, and exercise science fields. It is always challenging to understand the dynamic nature of motor unit (MU) functioning, neuromuscular activity, or muscle contractions. Different teaching instruments can be used during a didactic lecture to make it more engaging for the students, including the use of electromyography (EMG) and dynamometry techniques. EMG serves as a clinical and educational tool to evaluate skeletal muscle activity, enhancing the understanding of muscle physiology. This activity aims to describe seve

Effects of loading on upper airway and respiratory pump muscle motoneurons.

The functional outcomes of respiratory muscle loading by chemical (e.g. hypercapnia), mechanical (i.e. external mechanical loading) or ventilatory (e.g. exercise) factors can be either positive, such as through an increase in pressure-generating capacity of the inspiratory muscles or detrimental, such as by fatigue. Neurophysiological responses to respiratory muscle loading can occur at one or more points along the pathway from motor cortex to muscle. This paper describes the respiratory pump and upper airway motoneuron responses to the imposition of acute loads including processes of pre-acti

Exhaustive exercise slows inspiratory muscle relaxation rate in chronic obstructive pulmonary disease.

The excessive load placed on inspiratory muscles when patients with COPD exercise could lead to fatigue and contribute to exercise limitation. Slowing of maximal relaxation rate (MRR) of skeletal muscle is an early index of the fatiguing process. We investigated whether inspiratory muscle MRR slows when patients with COPD walk to exhaustion. We studied nine well-trained and motivated patients with stable severe COPD (mean FEV1: 0.7 L, 28% predicted). Each subject performed sniff maneuvers before and after walking on a treadmill until they were forced to stop because of dyspnea. Esophageal (Pes

Delayed-onset muscle soreness does not influence occlusal sensitivity and position sense of the mandible.

Masticatory muscle-pain patients often complain about sensorimotor changes, but the effects of pain on the psychophysical properties remain unclear. This study aimed to investigate the effects of delayed-onset muscle soreness (DOMS) on the jaw's position sense (PS) and occlusal sensitivity (OS). In all, 12 participants underwent intense concentric-eccentric jaw exercises. Self-reported muscle fatigue and pain, pain-free maximum mouth opening (MMO), pain pressure thresholds (PPTs) at right and left masseter and right and left anterior temporalis, maximum voluntary bite force (MVBF), PS and OS w…

Relationship between daily training load and psychometric status of professional soccer players.

We studied the relationship between daily training load (TL) experienced by professional soccer players and the Hooper questionnaire reflecting their perceived quality of sleep, fatigue, stress and delayed onset muscle soreness. During a 16-week training period, the rating of perceived exertion and duration were collected after each training session, and daily TL was calculated from 14 professional soccer players. The Hooper questionnaire was completed every day before the first training session and the Hooper's score (HS) was then calculated. The daily TL and HS were 379.9 ± 198.3 AU and 16.2

Effects of different inspiratory muscle training protocols on functional exercise capacity and respiratory and peripheral muscle strength in patients with chronic kidney disease: a randomized study.

BACKGROUND: Pathological changes were observed in the diaphragm due to abnormal renal function in chronic kidney disease (CKD). Inspiratory muscle training (IMT) has been suggested for patients with CKD; however, the most appropriate intensity for IMT has not been determined. Therefore, this study aimed to investigate the effects of different IMT protocols on respiratory muscle strength, quadriceps femoris muscle strength (QMS), handgrip muscle strength (HGS), functional exercise capacity, quality of life (QoL), pulmonary function, dyspnoea, fatigue, balance, and physical activity (PA) levels
