Myelopathy following influenza vaccination in inflammatory CNS disorder treated with chronic immunosuppression.

We report a patient who developed a transverse myelitis with Brown-Séquard syndrome following a prophylactic influenza vaccination, despite being chronically immunosuppressed for a steroid-responsive optic neuropathy. Although influenza vaccination is recommended in patients receiving chronic immunosuppression, its use may on occasion be associated with neurological complications previously reported in immunocompetent individuals.

Mumps hemiplegia.

Neurological involvement of mumps is commonly restricted to aseptic meningitis. However, rarely mumps is associated with more severe encephalitic illness; other known associations described with mumps are cases of transverse myelitis and Gullain Barre like illness. We report a case of hemiplegia due to mumps parotitis probably caused by involvement of carotid artery.

Cranial neuropathy, myeloradiculopathy, and myositis: complications of Mycoplasma pneumoniae infection.

Polymyositis, transverse myelitis, ascending polyneuritis, bilateral optic neuritis, and hearing loss developed in a patient with high complement-fixing antibody titers to Mycoplasma pneumoniae. Each of her three children had primary atypical pneumonia with isolation of the organism. The neurologic disturbance is thought to represent a postinfectious complication of M pneumoniae infection.

Myeloradicular Form of Neuroschistosomiasis in a Six-Year-Old Boy Infected With Schistosoma mansoni.

Neuroschistosomiasis is a severe disease caused by the presence of Schistosoma eggs and/or adult worms in the central nervous system. Schistosomal transverse myelitis represents a rare clinical form with nonspecific clinical findings, and it is thus underdiagnosed, especially in children. In this report, we describe a 6-year-old patient with the myeloradicular form of neuroschistosomiasis.

Neuromyelitis Optica Spectrum Disorders.

Neuromyelitis optica (NMO) is clinically characterized by severe optic neuritis and transverse myelitis, but recent studies with anti-aquaporin-4-antibody specific to NMO have revealed that the clinical spectrum is wider than previously thought. International consensus diagnostic criteria propose NMO spectrum disorders (NMOSD) as the term to define the entire spectrum including typical NMO, optic neuritis, acute myelitis, brain syndrome, and their combinations. NMOSD is now divided into anti-aquaporin-4-antibody-seropositive NMOSD and -seronegative NMOSD (or unknown serostatus). MR imaging and

Neurologic Complications Associated With the Zika Virus in Brazilian Adults.

IMPORTANCE: There are no prospective cohort studies assessing the incidence and spectrum of neurologic manifestations secondary to Zika virus (ZIKV) infection in adults. OBJECTIVE: To evaluate the rates of acute ZIKV infection among patients hospitalized with Guillain-Barré syndrome (GBS), meningoencephalitis, or transverse myelitis. DESIGN, SETTING, AND PARTICIPANTS: A prospective, observational cohort study was conducted at a tertiary referral center for neurological diseases in Rio de Janeiro, Brazil, between December 5, 2015, and May 10, 2016, among consecutive hospitalized adults (>18 y

Serum thyroid-stimulating hormone and anti-thyroglobulin antibody are independently associated with lesions in spinal cord in central nervous system demyelinating diseases.

Transverse myelitis (TM) is associated with neuromyelitis optica (NMO) and multiple sclerosis (MS). Early recognition of useful parameters may be helpful to distinguish their difference. This retrospective study analyzed thyroid parameters from 243 serum samples (relapse = 128; remission = 115) of 178 patients with demyelinating diseases (NMO, n = 25; TM, n = 48; MS, n = 105). The relationship between thyroid and clinical parameters was analyzed. Patients with NMO and TM had a higher frequency of abnormal thyroid-stimulating hormone (TSH), anti-thyroglobulin antibodies (TG-Ab), and antithyroid

Acute monophasic focal demyelinating diseases of the central nervous system: longitudinal clinico-neuroradiological study of two patients.

We report the clinical features and laboratory-instrumental findings in two patients with acute monophasic demyelinating disease with focal clinical expression: one case of incomplete cervical transverse myelitis and one of focal supratentorial encephalitis. We describe the clinical course and evolution of the neuroradiological findings (CT and MRI) and discuss their pathological meaning and diagnostic value.

Clinical Spectrum, neuroimaging, treatment, and outcomes of dengue-associated spinal cord involvement.

Dengue-associated spinal cord involvement is an uncommon but clinically important neurological complication of dengue infection. Existing evidence is fragmented and largely limited to isolated case reports, case series, and small observational studies. This systematic review evaluated the clinical spectrum, neuroimaging findings, pathogenesis, treatment approaches, and outcomes of dengue-associated spinal cord involvement. A systematic review was conducted according to PRISMA 2020 guidelines. PubMed, Embase, Scopus, and Google Scholar were searched from inception to the final search date. Case

Double hit - A case in point for dual seropositivity to AQP4 and MOG antibodies.

AQP4-IgG NMOSD (anti-aquaporin-4 neuromyelitis optica spectrum disorder) and MOGAD (myelin oligodendrocyte glycoprotein antibody associated disease) are unique disorders among themselves, with rare reports of dual seropositivity being described. Evaluation with cell-based assays reduces the incidence of false positivity. The clinical features of these cases may either have a dominant phenotype or may evolve into one subsequently. We describe a young girl aged 18-year-old who presented with longitudinally extensive transverse myelitis and dual seropositivity to both AQP4 and MOG antibodies.

Vascular Myelopathies.

PURPOSE OF REVIEW: Neurologists should be able to identify clinical and neuroimaging features that distinguish vascular disorders from other causes of myelopathy. RECENT FINDINGS: Although certain clinical features suggest a vascular etiology in acute and chronic myelopathy settings, accurate MRI interpretation within the clinical context is key. Recent studies have shown vascular myelopathies are frequently misdiagnosed as transverse myelitis, and recognition of this diagnostic pitfall is important. Many different vascular mechanisms can cause myelopathy; this article provides a comprehensiv

Mycoplasma pneumoniae-associated myelitis: a comprehensive review.

Myelitis is one of the most severe central nervous system complications seen in association with Mycoplasma pneumoniae infections and both acute transverse myelitis (ATM) as well as acute disseminated encephalomyelitis (ADEM) have been observed. We reviewed all available literature on cases of Mycoplasma spp. associated ATM as well as ADEM with dominant spinal cord pathology and classified those cases according to the strength of evidence implicating M. pneumoniae as the cause. A wide range of data on diagnosis, epidemiology, immunopathogenesis, clinical picture, laboratory diagnosis, neuroima

Diagnosis and management of acute myelopathies.

BACKGROUND: Acute myelopathies represent a heterogeneous group of disorders with distinct etiologies, clinical and radiologic features, and prognoses. Transverse myelitis (TM) is a prototype member of this group in which an immune-mediated process causes neural injury to the spinal cord, resulting in varying degrees of weakness, sensory alterations, and autonomic dysfunction. TM may exist as part of a multifocal CNS disease (eg, MS), multisystemic disease (eg, systemic lupus erythematosus), or as an isolated, idiopathic entity. REVIEW SUMMARY: In this article, we summarize recent classificati

[A case of spinal hemorrhage associated with abdominal aortic coarctation].

We report a 59-year-old male with abdominal aortic coarctation presented as paraplegia due to spinal hemorrhage caused by the rupture of abnormally dilated spinal artery. In coarctation of aorta, coarctation is usually located in the aortic isthmus which could be the cause of cervical and upper thoracic myelopathy. However, there has been no report of abdominal aortic coarctation with hemorrhagic transverse myelopathy. In this case hemorrhage occurred after surgical treatment and prescribed warfarin may have exaggerated the outcome.

Paraparetic Guillain-Barre syndrome: An uncommon diagnosis of acute flaccid paralysis of the lower limbs.

Apart from the usual differentials of transverse myelitis and cord compression, paraparetic GBS should be considered when sudden, flaccid paralysis of the lower limbs occurs, as prompt diagnosis and management can minimize sequel and unnecessary procedures. We do report a case wherein we managed a similar situation without the use of an immunomodulatory therapy.
