Primary aldosteronism presenting as myoglobinuric acute renal failure.

Hypokalemia is an uncommon cause of rhabdomyolysis with acute tubular necrosis. We recently treated a patient in whom severe hypokalemia attributed to diuretic therapy antedated acute myoglobinuric renal failure by six months. After recovery, hypokalemia persisted and subsequent evaluation disclosed primary aldosteronism. This case is a unique presentation for primary aldosteronism and illustrates the importance of diagnosis before treatment in hypertension as well as the hazards of hypokalemia.

Hypercalcaemia in acute renal failure.

A case of myoglobinuric acute renal failure complicated by hypercalcaemia is reported. Data on 13 other such cases published in the English literature since 1964 are summarised. Hypercalcaemia appears to be uniquely confined to rhabdomyolysis-myoglobinuric acute renal failure. Serum calcium concentrations should be monitored in such patients.

Cholesterol ester accumulation: an immediate consequence of acute in vivo ischemic renal injury.

BACKGROUND: Cholesterol is a major constituent of plasma membranes, and recent evidence indicates that it is up-regulated during the mainten … rtex after in vivo ischemia (15 and 45 minutes)/reperfusion (0 to 120 minutes) and glycerol-induced myoglobinuria (1 to 2 hours). FC/CE were also measured in (1) cultured human proximal tubule (HK-2) cells three hours after ATP depletion and in (2) isolated mouse proximal tubule segments (PTSs) subjected to plasma membrane damage (with cholesterol oxidase, sphi …

Persistent Hematuria With Late Diagnosis of Cardiac Origin: A Case Report.

Dark-colored urine is often referred to as hematuria; however, this is an assumption that can often be misleading. In the evaluation of dark-colored urine, distinguishing between true hematuria, hemoglobinuria, and myoglobinuria is essential, as each indicates different underlying pathologies and requires targeted diagnostic and management strategies. Visualization of dark-colored urine often leads to an isolated urologic evaluation, and assessment for more infrequent causes of this phenomenon may be obscured. Clinicians tend to anchor on common urologic causes of dark-colored urine or rely on…

Rhabdomyolysis in a young adult following chronic non-prescribed pregabalin use.

Rhabdomyolysis is a serious condition caused by skeletal muscle breakdown and can present with severe generalised pain, elevated creatine kinase and myoglobinuria. We report a rare case of rhabdomyolysis likely induced by chronic pregabalin abuse (15-year history of non-prescribed use) in a physically active man in his 20s. Despite the absence of trauma, infection or other known causes, he presented with typical symptoms and laboratory findings of rhabdomyolysis. He improved significantly with supportive care, including intravenous fluids and analgesia, and was discharged after stabilisation. …

A Unique Case of a Mildly Symptomatic Patient With Severe Hypokalemia Secondary to Furosemide.

Hypokalemia is a common condition that has been well studied. In most patients, mild hypokalemia is asymptomatic, while those with moderate-to-severe hypokalemia tend to show more prominent symptoms. In patients with a potassium level of 2.5 milliequivalents/liter (mEq/L) or lower, symptoms such as muscle cramps or weakness, rhabdomyolysis, and myoglobinuria tend to arise. These manifestations can lead to more debilitating consequences, such as renal failure secondary to rhabdomyolysis or respiratory failure secondary to muscle weakness. Other symptoms such as ileus, nausea, and vomiting, as w…

Cefiderocol-associated brown chromaturia.

Cefiderocol is a novel siderophore cephalosporin antibiotic. In the present case report, a woman in her 70s presented with a 1-week history … was complicated by brown urine discolouration. Investigations were unrevealing for haemoglobinuria, myoglobinuria and bilirubinuria. A side effect from cefiderocol was suspected and subsequently discontinued. Her urine colour returned to its normal colour within 3 days of discontinuation of cefiderocol.

Malignant hyperthermia in a 16-day-old infant with congenital diaphragmatic hernia: a case report.

Malignant hyperthermia (MH) is a severe hypermetabolic disorder associated with dysregulation of calcium homeostasis and is triggered by inh … rred to the pediatric intensive care unit for monitoring and treatment of acute renal injury due to myoglobinuria. Subsequently, two variants of the ryanodine receptor 1 (RYR1) gene were identified in the patient as the mutation point at c.1589G > A p.Arg530His and c.1841G > T p.Arg614Leu, which are known to be associated with MH. This was a rare case of MH in …

Emicizumab Associated Rhabdomyolysis in Hemophilia A.

Emicizumab is increasingly the front-line treatment for patients with Hemophilia A with or without inhibitors. Rhabdomyolysis is a syndrome of muscle necrosis and release of intracellular muscle constituents into the circulation. Creatine kinase (CK) levels are typically markedly elevated, and muscle pain and myoglobinuria may be present. The severity of illness ranges from asymptomatic elevations in serum muscle enzymes to life-threatening disease associated with extreme enzyme elevations, electrolyte imbalances, acute kidney injury and disseminated intravascular coagulation. We present a cas…

McArdle's disease: A differential diagnosis of idiopathic toe walking.

Idiopathic toe walking (ITW) is a pathological gait pattern in which children walk on their tip toes with no orthopedic or neurological reason. Physiological characteristics of the gastrocnemius muscles, the Achilles tendon, and the foot of toe walkers differ from subjects with a plantigrade walking pattern. McArdle's disease is characterized by the inability to break down muscle glycogen. It is an autosomal-recessive condition, characterized by low exercise tolerance, muscular atrophy at the shoulder girdle, episodes of myoglobinuria after vigorous physical activities and the occurrence of th…

Clinical protocol for the management of malignant hyperthermia.

Malignant hyperthermia is a hypermetabolic syndrome that appears in susceptible patients after exposure to certain anaesthetic drugs (succinylcholine, inhalation anaesthetics). Its incidence in Spain is 1 in 40,000 adults, with a 10% mortality rate. It is induced by an abnormal regulation of the ryanodine receptors, producing a massive release of calcium from the sarcoplasmic reticulum in the striate muscle. Clinical manifestations include: CO2 increase, tachycardia, haemodynamic instability, metabolic and respiratory acidosis, profuse sweating, hyperpyrexia, CPK increase, myoglobinuria, kidne…

[Myalgia during warming-up in a 12-year-old boy].

INTRODUCTION: McArdle disease is a hereditary myopathy caused by muscle phosphorylase deficiency. Patients experience painful muscle cramps after strenuous exercise; the condition is sometimes associated with rhabdomyolysis, myoglobinuria and resulting acute renal failure. CASE DESCRIPTION: A 12-year-old boy visited the Paediatric and Neurology outpatients' departments with symptoms of leg myalgia, occurring during the warming-up phase of soccer practice, which disappeared after a short rest. Detailed history-taking revealed that he already experienced pain while walking during early childhoo…
