FOCAL OR INSULAR NECROSIS PRODUCED BY THE BACILLUS OF TUBERCULOSIS.

1. It must be accepted from a comparison between the histological changes described in the focal necroses due to abrin and ricin, diphtheria, eclampsia, blood-serum intoxication, typhoid fever, lobar pneumonia and glanders, and those described here in three cases of tuberculosis, that the last are also instances of focal necrosis. 2. It may be claimed that the areas of necrosis in the three cases of tuberculosis had their origin in ischaemia, for the reason that they were numerous in the spleen and were not found in the kidney. Against such an explanation of their causation are the facts: that…

Scaphotrapeziotrapezoid Fusion for Chemotherapy-induced Capitate Avascular Necrosis.

The capitate's retrograde vascular supply and limited intraosseous anastomoses predispose its proximal pole to avascular necrosis, although this remains rare. Although typically linked to trauma, connective tissue disorders, or corticosteroid use, avascular necrosis may also arise secondary to chemotherapy. We present the case of an 18-year-old woman with a history of childhood leukemia who developed capitate avascular necrosis. This case emphasizes the importance of considering avascular necrosis in chemotherapy-exposed patients presenting with atraumatic wrist pain and evaluates outcomes of …

Acute esophageal necrosis in end-stage renal disease.

Acute esophageal necrosis is a rare but often fatal condition characterized by diffuse black discoloration of the esophageal mucosa. While commonly associated with hemodynamic compromise, or infection, the pathogenesis is often multifactorial. We report a fatal case of acute esophageal necrosis in a man in his 50s with untreated long-standing hypertension and newly diagnosed end-stage renal disease. The patient presented with fatigue, loss of appetite, and respiratory distress and was admitted with pneumonia, pulmonary edema, and advanced renal dysfunction. After the initiation of hemodialysis…

Tacrolimus-Induced Acute Esophageal Necrosis Postorthotopic Liver Transplantation.

Acute esophageal necrosis, or "black esophagus," is a rare but severe condition characterized by circumferential black necrotized esophageal mucosa with an abrupt transition at the gastroesophageal junction. The "two-hit" hypothesis suggests that an initial ischemic insult predisposes the esophageal mucosa to further injury from luminal acid exposure or medications. We present the first documented case of tacrolimus-induced acute esophageal necrosis without preceding hemodynamic instability in a liver transplant recipient, which resolved with discontinuation of tacrolimus and transition to cyc…

NICOLAU SYNDROME: CUTANEOUS NECROSIS FOLLOWING DICLOFENAC INTRAMUSCULAR INJECTION.

BACKGROUND: Nicolau syndrome, although it is quite rare, often occurs following Intramuscular injections of different medications, especially diclofenac and pencillin's. Accordingly, its symptoms usually begin with severe pain during injection, leading to ulceration and necrosis of the local tissue over time. Immediate diagnosis and treatment in the case of the syndrome are of great Importance. Diclofenac is class of NSAID's act as anti-inflammatory drug through inhibiting the Cycloxygenase-I and Cycloxygenase-II (COX-I AND COX-II) enzymes. CASE REPORT: In our case report we report the case o…

Spontaneous falciform ligament necrosis: A rare case report.

Spontaneous and isolated necrosis of the falciform ligament is a rare condition. A 55-year-old lady presented with sudden onset of epigastric pain, fever and vomiting. Laboratory studies revealed less than three-fold increase in serum amylase and lipase levels. Ultrasound examination and contrast enhanced computed tomography (CECT) revealed features suggestive of inflammation and necrosis of the falciform ligament involving ligamentum teres hepatis and extending to the lesser omentum with an adjacent minimal ill-defined collection. Then, conservative management was started. However, the sympto…

Unusual complication of pancreatic adenocarcinoma: massive liver necrosis.

Pancreatic adenocarcinoma is a malignant and aggressive disease, whose diagnose is achieved in many cases at advanced stage. We present the case of a 63-year-old woman diagnosed with adenocarcinoma of the pancreatic head and body, which invaded hepatic artery and presented with portal vein thrombosis. She consulted for melena and upper endoscopy showed varicose lesions in the second part of duodenum. The patient developed acute worsening of anemia with hemodynamic inestability. Urgent contrast enhanced computed tomography revealed a massive hepatic necrosis without identification of the hepati…
