OBJECTIVE: We have used three different approaches, namely, the infratentorial lateral supracerebellar approach, the lateral suboccipital infrafloccular approach, and the transcondylar fossa approach, for microvascular decompression for treatment of trigeminal neuralgia, hemifacial spasm, and glossopharyngeal neuralgia, respectively. Each approach is a variation of the lateral suboccipital approach to the cerebellopontine angle (CPA); however, each has a different site of bony opening, a different surgical direction, and a different route along the cerebellar surface.
METHODS: The infratentor…

OBJECTIVE: This study is to systematically evaluate the efficacy of acupuncture for PHN and provide a visual overview of treatment landscape.
METHODS: A systematic search was conducted in PubMed, Embase, Web of Science, China National Knowledge Infrastructure (CNKI), Chinese Scientific Journals Database (VIP), and Wanfang Database for systematic reviews (SRs) on acupuncture for PHN up to Apr 18, 2025. Studies were included if they were SRs of randomized controlled trials (RCTs) assessing traditional Chinese acupuncture interventions for PHN, and excluded if they involved non-traditional acupu

INTRODUCTION: Radiosurgical treatment of trigeminal neuralgia (TN) is the most common functional radiosurgery procedure used to modulate refractory pain. The aim of this study is to describe the clinical results and dosimetric parameters of the first series of patients with TN treated with ZAP-X gyroscopic frameless radiosurgery (ZAP Surgical Systems, Inc., San Carlos, CA), an innovative, frameless radiosurgical device.
METHODS: A total of 30 patients with TN received gyroscopic frameless radiosurgery (GRS) between February 2023 and January 2025. Treatment plans were developed delivering a ma…

OBJECTIVE: Trigeminal neuralgia (TN) is a debilitating neuropathic pain disorder that affects the trigeminal nerve, often causing intense facial pain. The prevalence and epidemiology of TN remain under-researched, particularly in China. This study aimed to estimate the lifetime prevalence of TN in Jincheng City, central China, and to provide an initial epidemiological insight into its occurrence within the Chinese population.
METHODS: A cross-sectional, descriptive epidemiological study was conducted from September to December 2024. A total of 1,350 individuals were surveyed using a 16-questi…

Tic douloureux, also known as trigeminal neuralgia, is distinguished by recurrent episodes of severe, lancinating pain that affects one or more branches of the trigeminal nerve, representing a prevalent pain syndrome. This condition has an annual incidence rate of 27 per 100,000 individuals. Nevertheless, direct compression caused by vertebrobasilar dolichoectasia (VBD) represents a considerably less frequent etiology of trigeminal neuralgia, with an estimated overall incidence of about 1%. A 65-year-old female patient with a history of diabetes mellitus and hypertension presented with a sever…

PURPOSE OF REVIEW: Discuss the current understanding of the pathophysiology and management of refractory trigeminal neuralgia (TN). This includes a discussion on why TN can recur after microvascular decompression and a discussion on "outside of the box" options when both first- and second-line management strategies have been exhausted.
RECENT FINDINGS: This review discusses second- and third-line oral medication options, botulinum toxin A, repeat microvascular decompression, repeat ablative procedures, internal neurolysis, trigeminal branch blockade, and neuromodulation using TMS or periphera…

OBJECTIVE: The goal of this study was to provide a comprehensive overview of the current understanding of molecular and genetic mechanisms underlying the pathophysiology of trigeminal neuralgia (TN).
METHODS: The authors searched PubMed systematically for primary research literature investigating specific molecular mechanisms from samples derived from patients with TN. The genes/molecules of interest from the selected literature were then cross-referenced with corresponding studies in animal models of TN.
RESULTS: From approximately 345 articles, a total of 12 articles were selected and incl…

PURPOSE OF REVIEW: Trigeminal neuralgia is a sudden, unilateral, stabbing pain in the distribution of one or more branches of the fifth cranial nerve, with an overall prevalence ranging between 0.03 and 0.3%. While conservative treatments may offer temporary relief, many patients experience chronic headaches associated with their neuralgia. Invasive treatments are available for patients with intractable neuralgia; however, they may cause permanent tissue damage and often do not provide relief. This article examines pulsed radiofrequency (PRF) ablation (RFA) of the trigeminal nerve as a minimal…

Trigeminal neuralgia is a painful disorder of the face that negatively affects the functionality of those who suffer from it. Microvascular decompression remains the top choice for its short and long-term outcomes. In those who are not candidates for open surgery, ablative options become valuable, including radiosurgery, and radiofrequency, balloon, and glycerol rhizotomies. Radiosurgery can be useful in cases of trigeminal neuralgia associated with multiple sclerosis. A scheme is proposed to facilitate decision-making considering outcomes, predictors of response and complication rates.

BACKGROUND: Occipital neuralgia (ON) is a primary headache disorder characterized by sharp, shooting, or electric shock-like pain in the distribution of the greater, lesser, or third occipital nerves.
AIM: To review the existing literature on the management of ON and to describe our technique of an endoscopic-assisted approach to decompress the GON proximally in areas of fibrous and muscular compression, as well as distally by thorough decompression of the occipital artery from the nerve.
METHODS: Relevant literature on the medical and surgical management of ON was reviewed. Literature on th…

BACKGROUND: Trigeminal neuralgia (TN) is a severe, disabling form of painful cranial neuropathy. Even though TN has a typical clinical picture, diagnosis it is often missed or delayed in clinical practice. In order to investigate the occurrence of diagnostic and therapeutic errors in TN, we studied 102 patients suffering from TN recruited through a multicentric survey.
METHODS: We performed a Pubmed database search on errors and pittfalls in TN diagnosis and management. Then, patients with TN were consecutively enrolled in the period from February 2017 to October 2019, by several European Hea…
