Polyneuropathy in late Lyme borreliosis - a clinical, neurophysiological and morphological description.

In a prospective study, detailed clinical and neurophysiological examinations were performed in 17 patients with polyneuropathy associated with the late borrelial manifestation acrodermatitis chronica atrophicans (ACA). Similar clinical and neurophysiological signs were found in most of the patients. The findings were those of a sensory polyneuropathy, mainly affecting large nerve fibres. Marked abnormality of vibration threshold was a common finding and in 4 patients this raised a suspicion of spinal cord engagement, in addition to a polyneuropathy. Sural nerve biopsy, performed in 3 of the p

Facial reflex myoclonus induced by language: a neuropsychological and neurophysiological study.

We studied a 53 year old right-handed patient who presented isolated myoclonus of right facial muscles induced exclusively by language. Twitching significantly hindered speaking and reading performance. MRI and CT-scan revealed no brain lesion. Conventional EEG showed a few spike-waves predominantly in the left hemisphere. Spike-waves increased during drowsiness. An EEG-EMG polygraphic study was performed during stimulation tests which included linguistic tasks and non-verbal/non-linguistic tests. Myoclonus was triggered by speaking and writing but not by non-linguistic tasks. The severity of

Photic reflex myoclonus: a neurophysiological study in progressive myoclonus epilepsies.

PURPOSE: To investigate the neurophysiological features of photic reflex myoclonus (PRM) in patients with progressive myoclonus epilepsies (PMEs) of different types (Unverricht-Lundborg disease, Lafora's disease, cryptogenic). METHODS: All patients underwent computerized video-polygraphic recordings, collecting electromyographic (EMG) activity from several cranial and limb muscles. PRM was elicited by intermittent photic stimulation (IPS). RESULTS: IPS could evoke PRM with a 1:1 relation at frequencies up to 12 Hz. Back-average of the EEG, triggered from the onset of PRM at the upper limbs,

Neurophysiological measures of reading difficulty in very-low-birthweight children.

Twenty-four 8-10-year-old children (13 very low birthweight, 11 control) performed a lexical decision and a semantic classification task while event-related potentials (ERPs) were recorded. Both groups were within normal range on standardized reading tests, but the very-low-birthweight group had lower scores. There were no differences between groups in reaction times or accuracy for ERP tasks. On analyses of P2a (246 ms anteriorly), P2p (336 ms posteriorly), N2a (356 ms anteriorly), and N2p (396 ms posteriorly) peaks and a late positive component, control children showed greater right than lef

[Value of neurophysiological studies in diagnostic verification of carpal tunnel syndrome].

The success of surgical treatment of carpal tunnel syndrome depends largely on correct diagnosis. Nerve conduction studies are usually recommended, especially in cases where history and clinical signs are not very clear. The aim of this study is to show whether nerve conduction studies are helpful in confirming the diagnosis. Fifty-two cases of carpal tunnel decompression (39 patients, 13 male, 26 female, mean age 54 years) were included in the study. Prior to surgery, all patients underwent standardized nerve conduction studies. One patient who developed RSD was excluded. In 33 cases the hist

Reversible motor and sensory neurophysiological abnormalities in cauda equina claudication.

A case of cauda equina claudication with canal stenosis is presented. Neurophysiological studies show reversible changes during symptomatic and asymptomatic phases. The somatosensory evoked potential from the tibial nerve was reduced in amplitude. Central motor conduction time (CMCT) after transcranial magnetic stimulation of the brain was reversibly prolonged in the symptomatic phase. Reversible CMCT changes have not been previously shown. The findings are discussed in the light of the pathophysiology of ischaemic nerve.

Neurophysiological assessment of divers with medical histories of neurological decompression illness.

OBJECTIVE: To examine the possibility that subclinical damage may persist after clinical recovery from neurological decompression illness. METHODS: The neuraxes of 71 divers with medical histories of neurological decompression illness and 37 non-diver controls were examined by recording the somatosensory evoked potentials produced on stimulation of the posterior tibial and median nerves. RESULTS: Although the tests gave some objective support for the presence of "soft" residual neurological symptoms and signs, no evidence was given for the presence of subclinical damage. CONCLUSIONS: The co

Shunting in chronic post-traumatic hydrocephalus: demonstration of neurophysiologic improvement.

The neuronal metabolic state that forms the cellular basis for cognitive functioning and motor behavior is dependent on cerebral perfusion. As intracerebral pressure increases, cerebral perfusion is compromised. The management of acute hydrocephalus after head trauma has been extensively described. In this article, we present evidence of clinical, cerebral perfusion, and neurophysiologic improvement in a 19-year-old patient with chronic post-traumatic hydrocephalus. Lethargy, gaze palsy, torticollis, and triplegia were noted 2 years postinjury in a neurologically stable patient. Computed axial

Neurophysiological diagnosis of the carpal tunnel syndrome: evaluation of neurographic techniques.

Twenty-eight patients with a clinical diagnosis of carpal tunnel syndrome underwent extensive neurographic examination. The results were compared with those in 86 normal subjects who had no signs or symptoms. The widely used methods of measuring distal motor latency from wrist to thenar and the sensory conduction velocity from the first and third fingers to the wrist gave highly significant differences between the carpal tunnel syndrome group and the control subjects. There was, however, a large overlap of values between the two groups and the discriminative power of the tests was not sufficie

Inadequacy of transcranial magnetic stimulation in the neurophysiologic assessment of Bell's palsy.

Transcranial magnetic stimulation is a non-invasive procedure which to stimulate the brain cortex and the peripheral nerve pathways. A new technique was recently introduced to record the muscle action potential of facial muscles by means of transcranial magnetic stimulation of the facial nerve. The experimental data that was obtained indicate that this technique allows to stimulate the facial nerve above the stylomastoid foramen: a greater tract of the nerve can therefore be explored than what was possible with the traditional electrical stimulation at the mastoid. Until now no comparison data

[Neurophysiologic and histopathologic studies in a case of congenital sensory neuropathy with anhidrosis].

A three year-old boy with congenital sensory neuropathy with anhidrosis (CSNA) was described. Sural nerve biopsy specimens revealed an almost complete absence of unmyelinated fibers and a marked decrease of the density of small myelinated fibers with preservation of the density of large myelinated fibers. No evidence of active degeneration of unmyelinated or myelinated fibers was found. Skin biopsy specimens revealed the absence of nerve terminals and fibers innervating sweat glands, although sweat glands seemed to be apparently normal in their morphological findings. Therefore, it was conclud
