Aneurysmal subarachnoid hemorrhage diagnosis with computed tomographic angiography and OsiriX.

PURPOSE: Recent advances in computed tomographic angiography (CTA) have resulted in its replacing digital subtraction angiography (DSA). However, CTA requires a powerful workstation and experienced technicians for image postprocessing. OsiriX, a free open-source medical imaging software with powerful three-dimensional (3D) capability, enables neurosurgeons to perform 3D rendering without extensive training. In this study, we examined the sensitivity and specificity of CTA with OsiriX as the primary diagnostic tool for intracranial aneurysms. METHOD: From May 2006 to March 2009, 121 patients w…

Controversy pertaining to therapeutic modalities for tumors of the pineal region: a worldwide survey of different patient populations.

The management of tumors of the pineal region differs between Western countries and Japan. This paper reports on a worldwide survey of individual experience and regimens for treating pineal region tumors in different patient populations. Fifteen pediatric neurosurgeons from nine different countries participated in the survey, and a total of 408 pineal region tumors were evaluated. Determination of tumor histology as an initial procedure was strongly supported by the majority of neurosurgeons in North and Central America and Europe (group A), whereas all but one from Asia and Egypt (group B) em…

Neurosurgical Evaluation and Management of Children with Achondroplasia.

Achondroplasia is the most common of skeletal dysplasias and is caused by a defect in endochondral bone formation. In addition to skeletal deformities, patients with achondroplasia possess significant abnormalities of the axial skeleton, including small skull base with a narrowed foramen magnum and small vertebral bodies with shortened pedicles. Consequently, patients with achondroplasia are at risk of several severe neurologic conditions, such as cervicomedullary compression, spinal stenosis, and hydrocephalus, which frequently require the attention of a neurosurgeon. This article provides an

[Complications of sinusitis].

The complications of sinusitis are essentially secondary to ethmoidal and frontal sinusitis, occurring in patients weakened, particularly in case of immunodeficiency or anatomical defects. The gravity is due to the risk of spreading infection in intracranial tissues and orbital cavity. The diagnosis is always to discuss any symptomatology resistant to treatment and the appearance of orbital or neurological signs. The scanner and MRI contribute greatly to the orientation and diagnostic evaluation of the extent of the lesions. Treatment is based on prolonged antibiotic therapy and surgery and re

Managing the patient with transsphenoidal pituitary tumor resection.

Patients who undergo transsphenoidal pituitary tumor resection require a multidisciplinary team approach, consisting of a neurosurgeon, an endocrinologist, and nurses. Successful transsphenoidal surgery needs expert nursing care for early identification and prompt treatment of pituitary dysfunction and neurosurgical complications. Pituitary dysfunction includes adrenal insufficiency, diabetes insipidus, syndrome of inappropriate antidiuretic hormone, and cerebral salt wasting syndrome. Neurosurgical complications may include visual disturbance, cerebrospinal fluid leak, subdural hematoma, and

Anatomy and clinical significance of the trigeminocerebellar artery.

The trigeminocerebellar artery (TCA) is a unique branch of the basilar artery supplying both the trigeminal nerve root and the cerebellar hemisphere. In this study, we describe and demonstrate the microanatomy of the TCA in 45 brainstems and discuss the neurological, neuroradiological and neurosurgical significance. This is the largest series of cadavers in the literature. The close relationship of the TCA to the trigeminal nerve root may have clinical implications including for the etiology of trigeminal neuralgia, thus the neurosurgeon must be aware of the vasculature of the trigeminal nerve

Transient quadriplegia and return-to-play criteria.

There will always be an inherent risk to participation in contact sports. Appropriate education of coaches, players, and parents is important. Recurrence rate is high in the athlete who has experienced transient quadriplegia or any neck injury. The athlete and his family should be made aware of this risk. Counseling is even more important, both from a medical and legal standpoint, in the patient with underlying spinal stenosis, found either incidentally or following an episode of transient quadriplegia. In these situations, consideration of consultation with a neurologist or neurosurgeon is pr

Basilar trunk occlusion during endovascular treatment of giant and fusiform aneurysms of the basilar artery.

Giant and fusiform aneurysms of the basilar artery (BA) are difficult to manage surgically, and they still present a challenge for the neurosurgeon despite progress in posterior fossa surgery. Guglielmi detachable coils have provided an alternative approach to treatment, but they have their own difficulties. We present a series of three patients with BA aneurysms treated with endovascular occlusion of the aneurysm and basilar trunk. Our experience suggests that, in select cases, BA occlusion may be not only safe but also necessary for the effective treatment of these aneurysms.

Herniated nucleus pulposus with radiculopathy in an adolescent: successful nonoperative treatment.

The following report describes a 17-year old girl with a central herniated nucleus pulposus (HNP) at L4-5. The diagnosis of lumbar disk herniation was based on findings from magnetic resonance imaging (MRI). A decision to use conservative treatment was unanimously agreed upon by a team of orthopedists, a neurosurgeon, and an internist. This report follows the patient's course from the onset of pain through the completion of the nonoperative treatment and shows that HNP of a lumbar intervertebral disk with radiculopathy can be treated successfully with aggressive nonoperative care.

Correction of nonsyndromal craniosynostosis.

Correction of craniosynostosis requires close collaboration between the craniofacial surgeon and the neurosurgeon. Typically, nonsyndromal craniosynostosis patients will require only one operation to correct the cranial vault deformity. The procedures usually are undertaken between 3 and 6 months of age. Any gaps are filled in with new bone because the dura is highly osteogenic. The early correction of these deformities can avoid future facial deformities as a result of restricted skull base growth causing maxillary and secondary mandibular deformities.

[Surgery of benign tumors of the anterior base of the skull].

Our surgical techniques for the complete removal of large tumours of the anterior skull base are described. Neurosurgeon and otolaryngologist should perform this surgery together. However, only in few cases a combined transfrontal and subfrontal approach is indicated. In cases where the sinuses of both sides are involved the approach of Schloffer offers some advantages in combination with modern techniques of reconstruction. Special attention is directed to the reconstruction of large defects of the skull base with ceramics (e.g. highly pure aluminium-oxide-ceramic, porous tricalciumphosphate

AutoPathNet: a patient-specific automated 3D trajectory planning framework for minimally invasive evacuation of hypertensive intracerebral hemorrhage.

PURPOSE: Rapid and reproducible trajectory planning is important for minimally invasive evacuation of hypertensive intracerebral hemorrhage (ICH). We developed AutoPathNet, a patient-specific automated 3D trajectory-planning framework that generates geometrically favorable candidate trajectories from preoperative imaging. METHODS: AutoPathNet reconstructs patient-specific anatomical models from CT-derived inputs, generates candidate entry-target trajectories, filters candidates according to predefined anatomical constraints, and ranks feasible trajectories using a composite geometric planning

Exploring patient satisfaction of a joint-consultation clinic for trigeminal neuralgia: Enabling improved decision-making.

BACKGROUND: Trigeminal neuralgia (TN) is a relatively rare condition which has a profound impact not only on the patient but also on those around them. There is no cure for TN, and the management of the condition is complex. The most effective forms of treatment are either through medication, neurosurgery, or combination of the two. Each option has risks and implications for the patient. As with all clinical decisions, it is important for patients to understand and be fully informed of the treatments available to them. A London UK unit adopted a joint-consultation clinic approach where the pat

Histological evaluation of thermal damage to Osteocytes: A comparative study of conventional and ultrasonic-assisted bone grinding.

This paper addresses an important issue faced by neurosurgeons during surgical skull bone grinding, a common process used to remove bone in skull base tumour removal surgery to enable the neurosurgeon to reach the target region. The heat generated during bone grinding could harm the soft tissues and can lead to osteonecrosis and cell death. In the present study, a novel process of rotary ultrasonic bone grinding (RUBG) was proposed for osteotomy to limit the temperature to a safe level. A systematic investigation was conducted to determine the effect of varying process parameters on osteonecro…
