Communication at work: A survey to explore the relationships between healthcare providers' communication competence and professional quality of life in neonatal care.

OBJECTIVE: Providing medical care to preterm infants can be rewarding yet also stressful for healthcare providers in the neonatal care unit (NICU). While the impact of provider-parent communication on parent-related stress and satisfaction is widely accepted, little is known about the provider perspective. Therefore, this study explores the relationships between neonatal care providers' communicative competence and their professional quality of life and job satisfaction. METHODS: Using the NICU Communication Framework, we conducted a cross-sectional survey among N = 300 Dutch pediatricians‑ne

Impact of Task-Oriented Role Assignment on Neonatal Resuscitation Performance: A Simulation-Based Randomized Controlled Trial.

OBJECTIVE: Task-oriented role assignment (TORA) is the assignment of a specific role, a list of tasks, and a location to stand to each resuscitation team member. We performed this study to examine the impact of TORA training during a Neonatal Resuscitation Program (NRP) course on neonatal resuscitation team technical performance and behavioral skills. STUDY DESIGN: Participants were cluster randomized into either a standard NRP course (control) or an NRP course with TORA training. Both technical and behavioral skills were evaluated using standardized simulations. The simulations were video re

Current strategies in the nursing care of infants with hypoplastic left-heart syndrome undergoing first-stage palliation with the Norwood operation.

Hypoplastic left-heart syndrome is a common congenital heart defect that carries a mortality rate of 95% in the first month of life if left untreated. In the past, surgical options were not accepted. However, recent success with the Norwood operation as part of a staged reconstructive approach has renewed interest in operative intervention for these critically ill neonates. Assessment of the balance between systemic and pulmonary blood flow is critical to maintain cardiac output and systemic saturation.

The National Birth Center Study. Part III--Intrapartum and immediate postpartum and neonatal complications and transfers, postpartum and neonatal care, outcomes, and client satisfaction.

This is the final article of the three-part report of the National Birth Center Study. Eight percent of the mothers or infants had serious complications; 16% were transferred, 12% before and 4% after the deliveries. Fifteen percent of transfers were emergencies. Nulliparous women were much more likely than parous women to experience dystocia, be transferred, or have cesarean sections. Seventy-five percent of the nulliparous women gave birth in the centers, compared with 95% of the parous women. Eighty-four percent of the women had at least one postpartum home or office visit. There were 11,814

Delayed Cord Clamping in Preterm Neonates: A Review of Benefits and Risks.

IMPORTANCE: Delayed clamping of the umbilical cord of premature neonates decreases perinatal morbidity. Allowing time for autotransfusion of placental blood before the umbilical cord is clamped represents a simple practice that may have significant impact. In light of many professional societies recommending delayed cord clamping in premature neonates because of its beneficial effects, the topic still holds many unanswered questions. OBJECTIVE: The purpose of this article is to review the most recent evidence available regarding delayed cord clamping in premature neonates. EVIDENCE ACQUISITI

Assessment of the quality of care provided to women with gestational diabetes mellitus by different hospitals in Karachi, Pakistan.

BACKGROUND: Since gestational diabetes mellitus (GDM) prevalence is increasing in Pakistan, the quality of care provided to women with GDM is crucial for improving maternal and neonatal outcomes. Therefore, our study aimed to assess the quality of existing healthcare services related to GDM management across different hospitals in Karachi, Pakistan. METHODS: We used Donabedian's model to evaluate the input, process and output phases. The first two phases were assessed through a cross-sectional design, while the output was evaluated via in-depth interviews. The study included six hospitals, on

Epidemiology of complications of male circumcision in Ibadan, Nigeria.

BACKGROUND: The number of infants managed for neonatal circumcision injuries in our unit has been on the increase over the past 16 years. In our search for the sources and reasons for these injuries, we were unable to identify any previous studies of circumcision injuries from our environment. We therefore decided to carry out this study in order to shed some light on this growing problem. METHODS: The patients were made up of 370 consecutive consented children attending our infant welfare clinic for immunization over a period of 3 months. Information on their demographic data, their age at c

Rickets screening in the preterm infant.

Although rickets in the preterm infant has decreased with improvements in care and nutrition, there continue to be infants at high risk for this disease. Early diagnosis and treatment can prevent fractures and other complications, such as decreased linear growth. Nurses must know which infants are at risk, must be aware of the appropriate screening tests, and must know how to customize care for at-risk neonates.

Management of neonates with hyperbilirubinemia: improving timeliness of care using a clinical pathway.

BACKGROUND: Neonatal hyperbilirubinemia is a common reason for neonates to present to the emergency department (ED). Although clinical practice guidelines provide recommendations for evaluation and therapy, few studies have evaluated ways to apply them effectively in the ED setting. The primary objective of this study was to compare time to phototherapy in neonates presenting to the ED with jaundice before and after implementation of a nursing-initiated clinical pathway. Secondary outcomes included time to bilirubin result and ED length of stay in neonates. METHODS: We performed a retrospecti

Outbreak of Bacillus cereus infections in a neonatal intensive care unit traced to balloons used in manual ventilation.

In 1998, an outbreak of systemic infections caused by Bacillus cereus occurred in the Neonatal Intensive Care Unit of the University Hospital Vrije Universiteit, Amsterdam, The Netherlands. Three neonates developed sepsis with positive blood cultures. One neonate died, and the other two neonates recovered. An environmental survey, a prospective surveillance study of neonates, and a case control study were performed, in combination with molecular typing, in order to identify potential sources and transmission routes of infection. Genotypic fingerprinting by amplified-fragment length polymorphis

The effect of bath applied in two different sequences on stress level and physiological parameters in term babies.

BACKGROUND: The sequence of neonatal bathing may significantly influence an infant's ability to maintain homeostasis. Traditional methods begin with the head, but deferring head washing to the final stage may reduce thermal loss and procedural distress. PURPOSE: This study compared the effects of two swaddle bathing sequences head-first versus body-first on the stress levels and physiological parameters of term newborns. DESIGN AND METHODS: This randomized controlled trial (RCT) included 104 term infants born in a Training and Research Hospital in Istanbul between July 18 and September 20, 2

Preparedness of primary health care workers and audit of primary health centres for newborn resuscitation in Port Harcourt, Rivers State, Southern Nigeria.

INTRODUCTION: Nigeria still has high newborn deaths and birth asphyxia remains a major cause. Birth attendants´ readiness to perform newborn resuscitation depends largely on their competence in basic resuscitation and availability of newborn resuscitation equipment to enable the various steps outlined in resuscitation guidelines to be applied quickly and appropriately. This study aimed to assess primary health care workers´ experience of neonatal resuscitation and audit primary health centres for availability of neonatal resuscitation equipment. METHODS: this descriptive cross-sectional study

Interpreting category II fetal heart rate tracings: does meconium matter?

OBJECTIVE: Category II fetal heart rate (FHR) tracings are considered indeterminate; thus, improved risk stratification of category II FHR tracings is needed. We estimated whether the presence of meconium increased the risk of adverse neonatal outcomes. STUDY DESIGN: This study was conducted within a prospective cohort of 5000 women with singleton pregnancies who were admitted in labor at term. Pregnancies with category II FHR in the 60 minutes before delivery were included. FHR data were extracted by trained nurses who were blinded to clinical outcome. The exposure was the presence of meconi

Twin-to-twin transfusion syndrome: a case study.

Twin-to-twin transfusion syndrome (TTTS) is a disease that affects roughly 15% of monochorionic twins. Although TTTS is not extremely prevalent, the rate of mortality and morbidly approaches 100% without early detection and treatment. The following case study is a triplet pregnancy that included a set of monochorionic twins affected by TTTS. Typically, it is assumed that monochorionicity can identify the zygosity of twins, which was disproven in this case. Research indicates that there has been an increased rate of monochorionic twins with in vitro fertilization. There is great debate about th

Noninvasive ventilation in the neonate.

One of the most common and concerning complications seen in low-birth-weight infants is chronic lung disease. A variety of factors have been implicated in the etiology of chronic lung disease including lung inflammation and injury. Noninvasive ventilation (NIV), a term applied to a variety of devices capable of supporting neonatal ventilation without the use of an endotracheal tube, is receiving increasing attention as means to reduce damage often incurred with mechanical ventilation. This article will review the history of continuous positive pressure ventilation and will provide an overview

Is a twelve-percent cesarean section rate at a perinatal center safe?

OBJECTIVE: Our purpose was to examine the pregnancy and neonatal outcomes at a perinatal center with a consistent cesarean section rate approximately half the national average. STUDY DESIGN: Ten years of vaginal delivery and cesarean section rates (1983 to 1992) and 5 years of mortality and morbidity outcomes (1988 to 1992) were compared with national health statistics and national health objectives. RESULTS: The cesarean section rate during the 10-year period ranged from 10% to 15%, with an average of 12.5%. The cesarean section rate for the 5 years during which maternal and neonatal outcom
