Management of optic neuritis in a developing African country.

AIM: To show the value of sub-Tenon's depo-methylprednisolone acetate followed by oral prednisolone in the management of optic neuritis in African patients. METHODOLOGY: Two female patients who presented to the University of Benin Teaching Hospital, Benin city, Nigeria and one male patient who presented to the Eye clinic of the Nigerian National Petroleum Corporation, Warri, Nigeria with optic neuritis were given a single dose of sub-Tenon's depo-methylprednisolone acetate 40 mg followed by oral prednisolone 60 mg daily and their vision monitored. RESULTS: In all the 3 cases, there was rapid…

Clinical profile of simultaneous bilateral optic neuritis in adults.

AIM: To establish the clinical profile of simultaneous bilateral optic neuritis in adults, the efficacy of steroid therapy, extent of visual recovery, and neurological outcome. METHODS: The authors performed a retrospective review of records of patients referred to a neuro-ophthalmology service with acute bilateral optic neuritis from 2000-4. Exclusion criteria included previous multiple sclerosis or myelopathy, known systemic disorders or medications associated with optic neuropathy, uveitis, or neoplasm. Patients received intravenous methylprednisolone followed by oral prednisone. Visual ac…

Multiple sclerosis in childhood: presenting as bilateral optic neuritis.

Multiple sclerosis is a disease of adolescence and middle age. Though rare, it is being increasingly recognized in younger population. We describe two patients in whom multiple sclerosis was diagnosed before 10 years of age. Both patients had bilateral optic neuritis during the course of their illness with partial recovery; other lesions in the neuraxis recovered reasonably well.

Color Doppler imaging in optic neuritis with multiple sclerosis.

PURPOSE: To evaluate the retrobulbar hemodynamics in eyes with optic neuritis with multiple sclerosis and to compare these values with those of the unaffected fellow eyes and healthy control eyes. METHODS: Thirty-four eyes of 17 patients with unilateral optic neuritis and multiple sclerosis and 16 eyes of 16 normal healthy subjects were recruited from the Departments of Ophthalmology and Neurology. The peak systolic and end-diastolic blood flow velocities and resistivity indices of the ophthalmic artery, posterior ciliary arteries and central retinal artery were measured in each eye with opti…

Optic neuritis: correlation of pain and magnetic resonance imaging.

PURPOSE: To demonstrate whether the magnetic resonance imaging (MRI) localization of the abnormal enhancement of the optic nerve can be related to the pain or pattern of visual field loss associated with acute optic neuritis. DESIGN: Retrospective observational series and MRI review from a referral neuro-ophthalmology service. PARTICIPANTS: Seventy-three women and 23 men with acute optic neuritis who had high resolution gadolinium-enhanced fat-suppressed MRI within twenty days of the onset of visual loss. METHODS: The presence of eye or other fifth cranial nerve (V(1)) pain, and pain with e…

[Optic neuritis as the first sign of multiple sclerosis].

BACKGROUND: Optic neuritis (ON) is as the initial manifestation of multiple sclerosis (SM) a favourable prognostic factor during the subsequent course. OBJECTIVE: To assess the ratio of ON at the onset of SM in the authors patients and to assess according to clinical criteria the form of disease and degree of functional inability, expressed by Kurtzke s EDSS scale. PATIENTS AND METHODS: In a retrospective analysis of patients with clinically defined and probable SM (Poser s criteria)) hospitalized at the neurological clinic in 1987-2000 the authors investigated the incidence of ON in relatio…

[Immunogenetics of optic neuritis in children with multiple sclerosis].

The aim of the work was to study immunogenetic peculiarities of optic neuritis in children with MS. Using PCR-SSP technique genomic typing was performed on HLA DRB1 gene (chromosome 6p21) of 56 unrelated children with ON registered at least once in clinically verified MS. 264 adult MS patients and 328 healthy controls from the same stratum of population were also genotyped. A very strong correlation of MS with DR15 (DRB1*150 ... alleles) was observed in comparison with healthy cases in the above population. In 39 cases of children with ON and MS both parents were also genotyped and the rate of…

Chronic inflammatory demyelinating polyradiculoneuropathy with histologically proven optic neuritis.

Although patient series of clinical, electrophysiological, or magnetic resonance imaging evidence for involvement of the central nervous sys … n revealed severe peripheral neuropathy consistent with CIDP and central involvement with bilateral optic neuritis. This is the first case reporting CIDP and histologically proven optic neuritis.

[Follow-up studies of optic neuritis with lymphocytic adenohypophysitis].

BACKGROUND: We report a 38-year-old female suffering from bilateral optic neuritis with lymphocytic adenohypophysitis. CASE: The initial symptom of the 38-year-old-female was diabetes insipidus. Magnetic resonance imaging showed swollen pituitary stalk and disappearance of the T1 shortening of the neurohypophysis. Immunological tests showed that the serum anterior pituitary antibody was positive. These findings suggested lymphocystic adenohypophysitis. Bilateral optic neuritis also occurred and the pattern visual evoked cortical potential (VECP) demonstrated increased P100 peak latency and re…

Optic neuritis with transient total blindness during lactation(1).

BACKGROUND: Lactation-induced blindness is a rare but devastating puerperal complication. There are few reported cases and no consistent associated disease processes historically identified. This case illustrates lactation-associated optic neuritis as an early identifier of multiple sclerosis. CASE: A nulliparous woman underwent a term vaginal delivery complicated only by chorioamnionitis. She was treated with intravenous antibiotics, which included spontaneous bacterial endocarditis prophylaxis. Her postpartum course was uncomplicated, and she was discharged on postpartum day 2 with her infa…

Optic neuritis in children: clinical features and visual outcome.

BACKGROUND: The purpose of this paper is to study the clinical spectrum of pediatric optic neuritis. We evaluated the presenting features, neuroimaging findings, cerebrospinal fluid abnormalities, associated systemic disease, and visual outcome in patients with this condition. METHODS: A retrospective analysis was performed on all patients who came to Baylor College of Medicine with optic neuritis during a 6-year period from 1991 to 1997. The degree of initial visual loss, subsequent visual recovery, and associated disease were reviewed. Magnetic resonance images and cerebrospinal fluid findi…

mtDNA haplogroup J: a contributing factor of optic neuritis.

Optic neuritis frequently occurs in multiple sclerosis (MS), and shares several similarities with the optic neuritis of Leber's hereditary optic neuropathy (LHON), which is mainly due to maternally transmitted mitochondrial DNA (mtDNA) mutations. Our report shows for the first time that a mitochondrial DNA background could influence the clinical expression of MS. One European mtDNA haplogroup was found only in MS patients with optic neuritis but not in MS patients without visual symptoms. Therefore, we hypothesize that mtDNA haplogroup J might constitute a risk factor for optic neuritis occurr…

Changes in the retinal veins in acute optic neuritis.

OBJECTIVE: To investigate patients with acute optic neuritis (ON) for changes of the retinal veins. MATERIAL AND METHODS: Seventy-six patients with acute ON were extensively neuro-ophthalmologically examined. RESULTS: Multiple sclerosis (MS) was found in 41 patients of whom 1 had periphlebitis retinae (PR) and 2 had venous sheathing (VS). Probable MS was found in 15 patients without prior symptoms of MS. One had PR and VS, and 2 had VS. Twenty patients had mono-symptomatic ON, none had retinal changes. CONCLUSION: Changes of the retinal veins should alert the clinician to a probable diagnos…
