Outcome of redo orchidopexy after previous laparoscopic orchidopexy.

PURPOSE: Testicular reascent is a recognised complication of orchidopexy, and redo surgery may be required. In this report, we present our experience of redo orchidopexy after initial laparoscopic surgery. METHODS: Patients who had undergone redo orchidopexy following an initial vessel-sparing (VS) or non-vessel sparing (NVS) laparoscopic orchidopexy between 2005 and 2019 were identified. Outcome data, including complications and testicular size, were recorded. RESULTS: The series comprised 23 patients (5: initial bilateral surgery with reascent on one side only; 18: unilateral surgery) with

A Case of Conservative Management of Intra-abdominal Polyorchidism in a Child.

Polyorchidism is defined as the presence of 2 or more testes on the same side. A 7-year-old boy (46 XY) with multiple congenital abnormalities and nonpalpable bilateral gonads is presented. He underwent diagnostic laparoscopic surgery at 8 months old which revealed duplicated right testes and contralateral presence of persistent left Mullerian duct structures. A successful Fowler-Stephens orchidopexy in 2 stages was performed on both testicles. Postoperative ultrasonography showed orthotopic right testes of 0.15 and 0.28 cc. We propose testicular preservation in these patients to prevent early

The undescended testis in children and adolescents. Part 1: pathophysiology, classification, and fertility- and cancer-related controversies.

Undescended testis (UDT) is defined as failure of a testis to descend into the scrotum. It is one of the most common reasons for consultation in pediatric surgery and urology with an incidence of 3% in live-born male infants. Decades ago, classical studies established that the failure of a testis to descend alters the development of its germ cells increasing the risk of infertility and testicular cancer in adulthood. More recent publications have rebutted some of the myths and raised controversies regarding the management of these patients, which, far from being limited to surgical treatment,

The undescended testis in children and adolescents part 2: evaluation and therapeutic approach.

Undescended testis (UDT) is defined as failure of a testis to descend into the scrotum and it is a common reason for consultation in pediatric urology. As extensively discussed in "The undescended testis in children and adolescents: part 1", the failure of a testis to descend alters testicular germ-cells development, increasing the risk of infertility and testicular cancer in adulthood. Here, we present the second part of our review and analysis of this topic with the aim to propose an updated and well-informed approach to UDT together with a treatment flow chart that may be useful to guide pe

Acquired cryptorchidism: an unexpected consequence of inguinoscrotal lymphatic malformation treated with Eppikajutsuto.

Lymphatic malformation (LM) that causes inguinoscrotal swelling is extremely rare. Surgery, sclerotherapy and pharmacotherapy have been reported as possible treatment options for LM. Recently, Eppikajutsuto (TJ-28), a traditional Japanese herbal medicine has emerged as therapeutic option for LM. We report the case of a 2-year-old boy who presented with a left inguinoscrotal swelling, which was diagnosed as retroperitoneal LM extending into the left scrotum. The surgical approach was less favourable, given the risk of damaging the testicular vasculature or the spermatic cord. Therefore, the pat

Modern management of and update on prune belly syndrome.

Prune belly syndrome (PBS) is characterized by the triad of abdominal flaccidity, a variable degree of urinary tract involvement and cryptorchidism. Most cases of PBS are sporadic and have a normal karyotype, with 95% patients being male. In the last decade, mutations in known genes that regulate embryonic genitourinary myogenesis have been identified and with increasing knowledge of these critical genes involved in bladder maldevelopment, advances can be made in genetic counseling. A multidisciplinary approach is necessary and individualization of care is recommended according to phenotypic s

Managing traumatic testicular dislocations: what we know after two centuries.

Traumatic testicular dislocation (TTD) is a rare consequence of blunt scrotal trauma. A 21-year old gentleman presented with inguinal pain following a motorcycle accident and physical examination revealed absence of both testes within a well-formed scrotal sac with bilateral inguinal swellings. Ultrasonography confirmed viability and location of the testes at the superficial inguinal pouch. He underwent emergent surgical reduction with orchidopexy and was discharged the next day. No evidence of testicular dysfunction or atrophy was noted at follow-up. We reviewed reports of TTDs reported in En

Modified stepwise mini-incision microdissection testicular sperm extraction: a useful technique for patients with a history of orchidopexy affected by non-obstructive azoospermia.

Non-obstructive azoospermia (NOA), which is defined as the absence of spermatozoa in the ejaculate secondary to impaired spermatogenesis within the testis, may be caused by a variety of etiologies, including varicocele-induced testicular damage, cryptorchidism, prior testicular torsion, post-pubertal mumps orchitis, gonadotoxic effects from medications, genetic abnormalities, chemotherapy/radiation, and other unknown causes currently classified as idiopathic (Cocuzza et al., 2013). The microdissection testicular sperm extraction (micro-TESE) technique involves a meticulous microsurgical explor

Laparoscopic orchidopexy in boys with prune belly syndrome--outcome and technical considerations.

BACKGROUND AND PURPOSE: Cryptorchidism is an ubiquitous feature in prune belly syndrome (PBS). Laparoscopic orchidopexy allows dissection of the spermatic cord with minimal morbidity. We discuss the technical difficulties and outcome of three boys with PBS who underwent two-stage laparoscopic Fowler-Stephens orchidopexy (F-SO). PATIENTS AND METHODS: Three boys, ages 1, 2, and 4, underwent laparoscopic F-SO. All boys had viable testes that were found within 3 cm of the deep inguinal ring. The testicular vessels were either ligated bilaterally with 4/0 polyglactin or monopolar diathermy was use

The concordance of testicular anatomic location in bilateral cryptorchidism.

OBJECTIVE: To determine the frequency of concordance of testicular anatomical location between both undescended testes based on surgical findings. BACKGROUND: Cryptorchidism occurs in approximately 1% of boys over one year of age, and in 10-20% of these cases, the condition is bilateral. We report a series of 168 patients with a preoperative diagnosis of bilateral undescended testes who underwent surgical exploration and in whom two testes were surgically identified. METHODS: The testicular location of both testes was stratified according to six anatomic regions. For each patient the anatomi

The treatment of cryptorchidism.

Medical opinion is quite confused about the age for and the method of treatment of the undescended organ. Histologic studies reveal that the spermatogenic cells of the undescended testis show definite injury by the age of four or five years. It therefore seems necessary to see that the testis is in the bottom of the scrotum before that age. Chorionic gonadotropins or testosterone should be tried first. If medical treatment fails, orchidopexy should be done without delay.

The relation between adult dark spermatogonia and other parameters of fertility potential in cryptorchid testes.

PURPOSE: The fertility potential of boys with cryptorchidism may be related to the number of adult dark spermatogonia per tubular transverse section in testicular biopsies taken at orchiopexy. Placental-like alkaline phosphatase positive gonocytes in testes within year 1 of life indicate preserved ability for germ cell transformation. We related these parameters to the total number of tubular germ cells and other factors associated with fertility potential. MATERIALS AND METHODS: The study comprised 89 boys 0.7 to 3 years old (median age 1.8) who underwent bilateral testicular biopsy at bilat…

Fertility-Preserving Laparoscopic Spermatic Cord Elongation in Bilateral Intraabdominal Testes: A Case of Fused Spermatic Cords.

INTRODUCTION: The optimal surgical approach for bilateral intraabdominal testes remains controversial. The Shehata technique has shown promising results as a fertility-preserving method by elongating the gonadal vessels without vessel division. MATERIALS AND METHODS: We present a case of bilateral intraabdominal testes treated with simultaneous bilateral Shehata technique. SURGICAL TECHNIQUE: At 9 months of age, bilateral testes were identified laparoscopically. After dissection of the spermatic cords, a one-stage bilateral orchidopexy was attempted. Due to significant tension in the testicu

A prospective comparative study of open versus laparoscopic stage II Fowler-Stephens orchidopexy in pediatric patients.

INTRODUCTION: Laparoscopic Fowler Stephens orchidopexy, single stage or two-stage, is now routinely performed in non-palpable testis. We performed second stage orchidopexy as open inguinal approach and compared the outcome of this approach to two-staged laparoscopic orchidopexy. METHODS: We performed a prospective randomized interventional study of two different approaches for intra-abdominal testis. In group A, Laparoscopic stage I (SFO) followed by open inguinal orchidopexy was compared for final outcome in group B cases, who underwent laparoscopic staged SF orchidopexy. The average duratio
