Premenstrual syndrome.

Premenstrual syndrome (PMS) affects the quality of life of millions of women. The complexity and variety of clinical presentation together with the cyclic recurrence of affective and somatic symptoms increase the difficulty in understanding and treating the disease. The precise pathophysiology of PMS is still unknown, but it is increasingly believed that, in women with PMS, the sensitive equilibrium between sex-steroids and central neurotransmitters is altered. Several studies have been carried out to understand the origin of the syndrome and to discover new ways of treatment. This review summ

Sudden death in a 21-year-old man caused by thrombosed coronary aneurysm: late sequelae or a very late onset of Kawasaki disease?

Kawasaki disease (KD; mucocutaneous lymph node syndrome) is an infantile febrile illness of unknown origin that occurs in worldwide epidemics. The main clinical features of the disease, such as fever, mucositis, cervical lymphoadenopathy and skin rash, associated with coronary aneurysms or arteritis are diagnostic. Late sequelae of coronary arteritis are very rare in the second decade of life; we describe the case of a 21-year-old male who died suddenly. The autopsy revealed a large thrombotic coronary aneurysm, coronary arteritis and myocarditis, supporting the diagnosis of a late fatal seque

Congenital spinal lipomas--tumor versus hamartoma.

Congenital spinal lipomas as a form of dysraphism are frequently reported in literature. A few studies have highlighted their histologic features and found them different from lipomas in other parts of the body. In a review of eight cases of spinal lipomas, we found heterotopic tissue in seven. Neural tissue, muscle, bone and cartilage were frequently encountered. The presence of meningothelial cells in one case was an unusual finding. These findings support the concept of their hamartomatous origin. Malignancy, occurring later in life in one of the heterotopic elements is known, though uncomm

[Penis synovial sarcoma: a rare case report].

Malignant neoplasms of the penis are infrequent, with sarcomas accounting for 5% of all cases. Synovial sarcoma is a specific subtype that has been reported in the scientific literature in this location on four occasions with clinically aggressive behavior, requiring oncologic resection of the lesion and management with complementary chemotherapeutic agents. The origin of these penile lesions is unknown and has been associated with predisposing factors such as previous exposure to radiotherapy and the presence of the SS18-SSX1/2/4 fusion gene. We present an unusual case of monophasic penile sy

[Incidental discovery of a patent ductus arteriosus in adults].

The patent ductus arteriosus is a vascular structure that connects the proximal descending aorta to the main pulmonary artery near the origin of the left pulmonary artery. The persistence of ductal patency after the first weeks of life is abnormal. In adults, it is most often an isolated finding but it may be associated with other congenital anomalies. Its repercussions depend on the size of the duct, varying from the absence of signs and symptoms to Eisenmenger's syndrome. Echocardiography is the keystone of diagnosis. Multimodal imaging allows precise anatomical characterization. Treatment w

Primary lung carcinoma in cystic fibrosis: A case report and literature review.

Patients with cystic fibrosis (CF) have increased life span because of improved care over last 50 years. With increasing survival, predisposition of cancer may become evident. We have observed increase risk of gastrointestinal tract cancer, testicular cancer and lymphoid leukemia. Lung cancer in patients with CF is rare. Our patient developed chronic respiratory failure due to CF related bronchiectasis. Patient had progressive disease despite optimum treatment, requiring lung transplantation. Pathology of explant lung showed focus of invasive adenocarcinoma of lung origin. Patient had no evide

Accessory soleus muscle.

Accessory soleus muscle is an uncommon anatomical variant that may present as a soft-tissue mass in the posteromedial region of the ankle. It is congenital in origin but usually presents in the second or third decade of life. Although it is a rare entity, accessory soleus muscle should be included in the differential diagnosis of soft-tissue swelling of the ankle. Awareness of the clinical presentation and specific findings of computed tomography, magnetic resonance imaging, and electromyography help with diagnosis without surgical exploration. We describe a 30-year-old patient with accessory

Prenatal detection of a tetralogy of Fallot with origin of the left pulmonary artery from the ascending aorta in a familial 22q11 microdeletion.

Here we report a case of prenatal diagnosis of anomalous origin of the left pulmonary artery from the ascending aorta associated with a tetralogy of Fallot in a familial form of 22q11 deletion. The mother, who had a normal heart and a velo-cardio-facial syndrome, had a first child with a pulmonary atresia plus ventricular septal defect associated with a 22q11 deletion. Prenatal diagnosis during the second pregnancy identified the above-described cono-truncal anomaly and FISH study showed a recurrent 22q11 deletion. This case illustrates the intrafamilial variability of cardiac involvement in 2

[Sleep and sleep disorders].

Disturbance of sleep is a symptom of heterogeneous origin. Hypersomnia (excessive sleep during daytime) must be carefully distinguished from hyposomnia. Hypersomnic patients should be referred to a neurologist for further diagnostic workup; hypnotics may be dangerous in these patients. Medical, psychiatric and other cases of hyposomnia should be identified and treated as part of a comprehensive therapy plan. Counselling and application of the rules of sleep hygiene should be considered. When drug therapy is indicated, benzodiazepines are preferable. Because chronic insomnia causes serious impa

A sizeable aneurysmal bone cyst of the mandibular ramus on 15-year-old patient. Intraoral approach.

Aneurysmal bone cysts (ABCs) are benign, non-neoplastic bone lesions, which contain blood and demonstrate a destructive pattern. They rarely occur in the head and neck region, with the mandible being the most common site of craniofacial origin. They develop in the second decade of life and their etiology is obscure. ABCs can sometimes demonstrate distinct radiological features and imaging is also necessary for preoperative planning. Surgery is the mainstay of treatment. The ABC should be excised in toto and usually osteotomies and primary reconstruction are necessary. We present a case of size

Melatonin production in the sea star Echinaster brasiliensis (Echinodermata).

The primary hormone of the vertebrate pineal gland, melatonin, has been identified broadly throughout the tree of life, in animals, plants, and fungi, supporting a deep evolutionary origin for this signaling molecule. However, some key groups have not been studied. Echinoderms, deuterostome animals, are one of these groups. Herein we study the presence of melatonin and enzymes of its pathway in the sea star Echinaster brasiliensis. We demonstrate that E. brasiliensis produces endogenous melatonin, in the gonads, under a circadian pattern with a nocturnal peak of production. We also show that t

[Management of onychocryptosis in primary care: A clinical case].

Onychocryptosis (ingrown toenail) is a condition commonly seen in Primary Care clinics. It is uncomfortable and restrictive for patients and has a high incidence in males between second and third decades of life. It is of unknown origin, with a number of predisposing triggering factors being involved. Treatment depends on the stage of the ingrown nail and the procedures may range from conservative to minor surgery that can be performed by the Primary Care physician in the health centre. We report the case of a 25-year onychocryptosis that did not respond to conservative management, and was ext

[Postnatal diagnosis and prognosis of 2 cases of triploidy].

Triploidy is one of the most common chromosomal aberrations in spontaneous abortions characterized by a 69-chromosome karyotype. This chromosome abnormality is rare in live-born children. Prevalence is lower than 1/50,000. We report on two premature newborns, male and female, born at 35 and 37 weeks of gestation, who presented with severe intrauterine growth retardation, facial dysmorphy, myelomeningocele, and syndactyly. They died during the first hours of life due to respiratory distress syndrome. Analysis of the karyotype showed a homogeneous triploidy on all mitoses: 69 XXY and 69 XXX. The

Congenital aneurysm of the muscular interventricular septum.

Congenital aneurysms of the muscular interventricular septum are rare. We report the diagnosis of such an aneurysm by fetal echocardiography at 19 weeks' gestation and the subsequent follow-up through early neonatal life, with the ultimate progression to a dilated cardiomyopathy requiring cardiac transplantation. This case highlights the early diagnosis of this rare finding and the unusual clinical course to include the confounding features of endocardial fibroelastosis, left ventricular noncompaction, and a lymphocytic infiltrate demonstrated on pathologic examination. Most infants with conge

Ischemic cardiomyopathy, left coronary trunk hypoplasia and subsidiary coronary supply from the descending thoracic aorta.

Clinical symptoms of acute or chronic myocardial ischemia due to congenital coronary anomalies occasionally develop during adult life. While several types of coronary anomalies have been already reported, origin of the coronary arteries outside of the ascending aorta and pulmonary trunk is exceedingly rare, and has indeed been described to date only in a 6-day-old newborn. We hereby report to the best of our knowledge the first and unique case of an adult patient with ischemic cardiomyopathy, in whom coronary angiography and aortography disclosed both left main trunk hypoplasia and subsidiary

Severe mitral regurgitation due to mitral valve prolapse associated with Bland-White-Garland syndrome.

Bland-White-Garland syndrome refers to the rare congenital cardiac abnormality whereby the left coronary artery arises from the pulmonary artery. The natural history of this condition is highly variable, ranging from death in early infancy to asymptomatic adult survival. It is sometimes diagnosed in adults with mitral regurgitation thought to be of ischaemic origin. We report a case of a 29-year-old man with Bland-White-Garland syndrome and concomitant mitral valve prolapse, and review the literature on the appropriate investigations and management of this abnormality. [A case of association of cyst of the urachus and Meckel's diverticulum].

Meckel diverticulum and residual urachus have a common embryological origin: the yolk sac at the eight day of life. Their course is independent but the underlying mechanisms are identical. Meckel diverticulum occurs in 2 to 4% of the population. Clinical signs are often absent and complications occur in 4% of the cases (haemorrhage, occlusion, inflammation, and rarely tumoural formation). Residual urachus occurs as a cystic formation, a sinus or a fistulization and may degenerate into adenocarcinoma of severe prognosis. The reported association has not apparently been reported previously in th

[Prolymphocytic leukemia, description of 2 cases].

We present the clinical and laboratory features of 2 patients with B prolymphocytic leukemia. Both are females of the fifth and seventh decade of life. One had the classical clinical picture of massive splenomegaly and a high white cell count, with characteristic prolymphocytes, and the other was asymptomatic, with a low white cell count. The cells were positive to B cell lineage reagents with strong surface immunoglobuline (Ig) and unreactive to T cell antibodies. Analysis of Ig genes at the DNA level demonstrated that both cases had heavy-chain gene rearrangements, confirming the B-cell orig

Among 64 patients who presented with glomerulonephritis of infectious origin, 17 cases (26%) of crescentic glomerulonephritis were studied retrospectively. Five patients had bacterial endocarditis and the identified primary infections were cutaneous or oropharyngeal. At the time of diagnosis, 15 patients had acute nephritic syndrome, 3 were anuric and only 2 had normal renal function. Despite symptomatic treatment, the prognosis in these patients was poor: 5 died of infectious disease, 3 are definitively on dialysis and 7 suffer from chronic renal failure. Meningioma: a nursing perspective.

A meningioma is an intracranial tumor arising from the coverings (meninges) of the brain. One of the first intracranial tumors successfully removed, the meningioma is considered the most common of all nongliomatous tumors, accounting for approximately 15 to 20% of all intracranial tumors. Occurring more than twice as often in women than men, meningiomas usually present between the third and sixth decades of life. It is imperative for nurses who work with neurosurgery patients to easily recognize and accurately assess specific characteristics of different brain tumors. Discussion in this articl
