[Treatment of distal radius non-union in a three-stage procedure. Case report].

INTRODUCTION: Non-union of the distal radius is an infrequent entity. Recently, it has been identified more often and it has been speculated that external fixation and other techniques used to maintain radial length, may create defects that favor the non-union. In chronic injuries, residual deformity and retraction of tissues are difficulties for treatment. CASE PRESENTATION: We report the case of a 40-year-old male patient with a nonunion after a Frykman VIII distal radius fracture managed unsuccessfully with external fixation with a Green-type cast, and with one year of evolution showing a

Peroneal tendon injuries.

Peroneal tendon injuries are underdiagnosed and should be considered in every patient who presents with chronic lateral ankle pain. Ankle sprains are common, and up to 40% of affected individuals experience subsequent chronic ankle pain. Identifying the source of chronic ankle pain can be difficult because of the large number of possible causes. The peroneal tendons are the primary evertors of the foot and function as lateral ankle stabilizers. A careful physical examination, along with a thorough patient history and imaging studies, are critical in arriving at an accurate diagnosis. Understan

Review of current methods used in the treatment of clubfoot at initial presentation and at recurrence.

Idiopathic clubfoot is a common condition seen by orthopaedic surgeons and is characterized by complex three-dimensional deformity of the foot. It is recognized that clubfoot treatment is a challenging issue in orthopaedics because it is an ongoing process, beginning in early infancy and continuing until the patient has reached skeletal maturity. This review article summarizes two important stages of clubfoot treatment. First, methods of initial correction-including nonoperative, semi-operative, and operative techniques-that have been used in the last 20 years are described. Second, the manage

Slipped capital femoral epiphysis: current concepts.

Slipped capital femoral epiphysis is a common hip disorder in adolescents, with an incidence of 0.2 (Japan) to 10 (United States) per 100,000. The etiology is unknown, but biomechanical and biochemical factors play an important role. Symptoms at presentation include pain in the groin, thigh, or knee. Ambulatory patients also may present with a limp. Nonambulatory patients present with excruciating pain. The slipped capital femoral epiphysis is classified as stable when the patient can walk and unstable when the patient cannot walk, even with the aid of crutches. Because the epiphysis slips pos

Posttraumatic proximal interphalangeal joint flexion contractures.

Normal motion of the proximal interphalangeal joint requires bony support, intact articular surfaces, unimpeded tendon gliding, and uncompromised integrity of the collateral ligaments and volar plate. Deficiency in any one of these structural requirements can lead to a loss of finger joint motion and decreased hand function. Once finger extension is lost, options include nonsurgical or surgical treatment. Nonsurgical treatment such as splinting or serial casting should be tried before attempting surgical intervention. When severe flexion deformity exists or the vascular status of the finger ha

Wound problems in surgery for talipes equinovarus.

The traditional treatment for talipes equinovarus (TEV) has been open surgery in many units around the world. The complication rates for primary corrective surgery are relatively small but may affect outcome significantly. The complications of surgery in relapsed TEV are more frequent than for primary surgery, and problems relating to wound healing present difficult challenges to the treating surgeon. An overview of the approaches to management of this still challenging condition are presented, and the factors involved in wound healing problems and strategies for their prevention and treatment

[The evaluation and treatment of acute anterior shoulder dislocation].

Anterior shoulder dislocation is the most common dislocation in the human body with a high recurrence rate and thus, has been one of the interesting subjects in orthopaedics and traumatology. There is still controversy about its evaluation and treatment. In particular, the type and duration of immobilization after reduction of the shoulder vary among centers. Another matter of debate is the indication of acute repair of the capsulolabral structures to prevent late instability. This review aims to provide an outline of current knowledge about the management and treatment alternatives of anterio

Surgical management of the arthritic wrist.

Wrist destruction by rheumatoid arthritis adversely affects hand function and increases the risk of finger deformities. Surgical management is indicated not only for salvage of end-stage wrist deformity but to alleviate pain and reduce progression of deformity at earlier stages. Available procedures include arthroscopic synovectomy, partial and complete arthrodesis, and implant arthroplasty. Procedure selection is highly dependent on disease characteristics and patient needs.

A new technique for the treatment of syndactyly with osseous fusion of the distal phalanges.

In treating complex syndactyly of the hand, including bony union of varying degrees, how to cover the exposed bone surface is still disputable. The authors adopted a homemade device for the treatment of a case of syndactyly with terminal bony union. External tissue expander theory was applied to their device, and expansion of the cutaneous bridge was followed by division of the fused distal phalanges. Two months later the raw bone could be covered simply with the expanded local skin, and no skin graft was needed except for at the base of the affected fingers. This technique produces good cosme

[Distal transfer of the greater trochanter among other surgical procedures for the treatment of hip deformity].

Distal greater trochanteric transfer is a rarely used procedure for treatment of hip deformity. Surgical technique is described. Potential for changing morphology of proximal end of the femur is underlined and pitfalls indicated. The main advantage of the procedure is reconstruction of the femoral neck. It is biomechanically more sound than valgus osteotomy done frequently in similar cases. Advantages of trochanteric transfer are confirmed by other authors who after this procedure achieved clinical and radiological improvement in patients with degenerative disease of the hip.

Advanced Surgical Training Models for Wrist Surgery.

Globally, surgical exposure for the trainee has become challenging due to changes in shift patterns, increasing medicolegal issues and the recognition of burnout. Simulation models are a crucial adjunct for the development of arthroscopic skills and open procedures. Advanced anatomically precise 3D-printed wrist models are used in our workshops. The models have pathological lesions within, allowing a range of arthroscopic and open procedures to be performed. Pathology can be ordered on-demand. Arthroscopic and open procedures can be performed, including fracture fixation and arthroplasty. For
