[Fibrodysplasia ossificans progressiva. Anesthetic management of a 2-year-old child].

Fibromyalgia ossificans progressiva (FOP) is a severely disabling disorder of connective tissue characterized by congenital malformation of the toes, fingers and vertebrae associated with progressive ossification of striated muscles. Anesthetic management of these patients involves preferably general anesthesia as local or regional anesthesia should be avoided due to possible heterotopic ossification. Airway management is determined by the age of the patient and the progression of the disease. Only a few cases in the literature have reported the anesthetic management of FOP patients and to our

A giant radiopaque mass in the masticatory space.

A 2-year-old boy presented with a giant mass in the masticatory space. The mass exhibited a lobulating ossification, with no attachment to the adjacent normal bone. An enucleation was performed under the tentative diagnosis of extra-articular synovial chondromatosis, benign ossifying neoplasm, non-neoplastic heterotopic ossification, or low-grade malignancy. Upon microscopic examination, the excised mass was composed of multiple osteocartilaginous areas. We hereby present detailed clinicopathological findings.

Back to the Basics: When Radiographs Solve the Case.

Myositis ossificans circumscripta is a benign, self-limiting heterotopic ossification usually occurring after a traumatic event. Our case describes a rare etiology given the occurrence after vaccination in a child. Indeed, to date, only two cases of myositis ossificans circumscripta after COVID-19 vaccination have been reported, both in adults. In this first pediatric case, radiographs performed at appropriate time allowed for a conclusive tissue characterization and hence diagnosis. CT and MR scans performed early after clinical manifestation showed only progressive, severe edema of the soft

Fibrodysplasia ossificans progressiva in Brazil: challenges and strategies to create assistance and educational networks.

Fibrodysplasia ossificans progressiva (FOP) is an ultrarare condition and one of the most impactful disorders associated with progressive heterotopic ossification events. It is estimated that there are 120-150 patients in Brazil; however, currently, fewer than 100 patients have been identified, and the role of a FOP advocacy group (FOP Brazil) has been instrumental for the identification and follow-up of these individuals and families. The aim of this article is to summarize the current status of FOP in Brazil and describe strategies proposed to approach this challenge in a continental size co

First evidence of pathology in the forelimb of the late Miocene saber-toothed felid Promegantereon ogygia (Machairodontinae, Smilodontini).

We examined the first evidence of pathology in the forelimb of the primitive saber-toothed felid Promegantereon ogygia, observed in a radius from the late Miocene (Vallesian, MN 10) site of La Roma 2 (Teruel, Spain). This fossil is the first evidence of a member of the Machairodontinae in this locality, and the first fossil of this species found in the Miocene basin of Teruel. The radius shows an exostosis shaped as a rough and wide bony crest probably caused by the lesion and posterior ossification of part of the tendon of the muscle abductor pollicis longus, an important extensor and abducto

[Ossification of pulmonary tissue].

Two cases of pulmonary ossification found in postmortems were described. The first case, a 67-year-old stop-smoker with systemic hypertension died of a ruptured atherosclerotic aneurysm of the abdominal aorta. A well-defined subpleural mass (52 mm in max. dimension) was composed of monotonous mature trabecular osseous tissue with two focuses of cartilage up to 500 microns. There was no evidence of occluded blood supply and chronic passive congestion in the pulmonary parenchyma. A diagnosis of mesenchymoma (osteohamartoma) with the predominance of osseous tissue was made. The second case, a 72-

Epiphyseal clefts.

Defects or clefts may be seen in the growing epiphysis and are usually observed just before puberty. The basal epiphysis of the proximal phalanx of the great toe is the most common site but similar defects have been observed in a number of other epiphyses. At least some of these defects develop within a single normal appearing epiphysis and are not associated with signs or symptoms suggestive of fracture. The mechanism of formation of these defects is not clear. The defects probably close spontaneously sometime around late puberty.

A Case of Progressive Ossifying Fibrodysplasia of Tracheobronchial Respiratory Muscles.

The authors report a case of progressive ossifying myositis (POM) in a 13-year-old boy, revealed by dry cough and dyspnea. Conventional chest x-rays and whole-body CT showed extraskeletal ossification that seems to affect the left bronchial strain and trachea. This lesional topography, if established, not yet described to our knowledge, contrasts with the observations of all the authors, including Munchmeyer, for whom smooth muscles and muscles attached to the skeleton by a single end are spared by the heterotopic ossifications characteristic of the disease. Therefore, this observation raises

Long-term vaginal sequelae secondary to pediatric pelvic fracture.

BACKGROUND: Pediatric pelvic fractures often appear radiographically benign despite significant injury to viscera. Vaginal injuries are rare, and therefore require a high index of suspicion. CASE: A 26-year-old female patient who sustained a pelvic fracture in a motorcycle accident at the age of 10 presented with vaginal stenosis precluding sexual intercourse. Patient menstruated through a fistula in the distal vaginal vault. Radiographic and dye studies showed almost complete vaginal obstruction with heterotopic ossification. An extensive 30-year review of the literature was conducted lookin

Structural basis for the potent and selective binding of LDN-212854 to the BMP receptor kinase ALK2.

Individuals with the rare developmental disorder fibrodysplasia ossificans progressiva (FOP) experience disabling heterotopic ossification caused by a gain of function mutation in the intracellular region of the BMP type I receptor kinase ALK2, encoded by the gene ACVR1. Small molecule BMP type I receptor inhibitors that block this ossification in FOP mouse models have been derived from the pyrazolo[1,5-a]pyrimidine scaffold of dorsomorphin. While the first derivative LDN-193189 exhibited pan inhibition of BMP receptors, the more recent compound LDN-212854 has shown increased selectivity for A

Resection arthroplasty for Mason type III radial head fractures yield good clinical but poor radiological results in the long term.

Long-term clinical and radiographic outcomes of primary resection for Mason type III radial head fractures were evaluated in 13 patients (14 elbows) with a mean age of 38.8 years (range, 20-67 years) at the time of surgery. All patients had isolated radial head fractures without associated injuries. Mean follow-up was 14.7 years (range, 9-26 years). Proximal migration of the radius and the carrying angle were measured, and radiographs were reviewed for degenerative elbow and wrist arthritis and periarticular ossification. Five results were excellent and 9 were good. In 8 cases, the radii had m

Sciatic nerve injury associated with acetabular fractures.

Sciatic nerve injuries associated with acetabular fractures may be a result of the initial trauma or injury at the time of surgical reconstruction. Patients may present with a broad range of symptoms ranging from radiculopathy to foot drop. There are several posttraumatic, perioperative, and postoperative causes for sciatic nerve palsy including fracture-dislocation of the hip joint, excessive tension or inappropriate placement of retractors, instrument- or implant-related complications, heterotopic ossification, hematoma, and scarring. Natural history studies suggest that nerve recovery depen

Progressive Bone Formation After Cervical Disc Replacement: Minimum of 5-Year Follow-up.

STUDY DESIGN: Retrospective review. OBJECTIVE: Our objective was to examine the prevalence, clinical significance, ramifications, and possible etiology of postoperative bone formation at the index level after cervical disc replacement (CDR) with a minimum of 5 years of follow-up. SUMMARY OF BACKGROUND DATA: CDR can be complicated by postoperative ossification and unwanted ankylosis at the index level, which some authors have termed "heterotopic ossification." This terminology may be inaccurate as it assumes the postoperative bone formation is unnatural and a consequence of the CDR surgery. W

Bone formation and inflammation in cardiac valves.

BACKGROUND: For nearly a century, the mechanical failure of calcified heart valves was attributed to a passive degenerative process. Recently, several case reports described bone formation in surgically excised heart valves and suggested an unexpected process of tissue repair. METHODS AND RESULTS: We studied the prevalence and pathology of heterotopic ossification in 347 surgically excised heart valves (256 aortic, 91 mitral) in 324 consecutive patients (182 men, 142 women; mean age 68 years) who underwent cardiac valve replacement surgery between 1994 and 1998. The valves were examined micro

Single-staged treatment using a standardized protocol results in functional motion in the majority of patients with a terrible triad elbow injury.

BACKGROUND: Terrible triad injuries of the elbow, defined as elbow dislocation with associated fractures to the radial head and coronoid, are associated with stiffness, pain, and loss of motion. Studies to date have consisted of small sample sizes and used heterogeneous surgical techniques, which render comparisons difficult and unreliable. QUESTIONS/PURPOSES: In a group of patients treated under a standard surgical protocol, we sought to determine the early dislocation rate, the range of motion in those not undergoing secondary procedures, the frequency and types of secondary surgical interv

Extremity fractures in the patient with a traumatic brain injury.

Extremity fractures are common in patients with traumatic brain injuries (TBIs). These injuries are often inadequately treated and occasionally are completely missed due to the unique problems inherent to the TBI patient. However, appropriate evaluation of the TBI patient allows prompt diagnosis and optimal treatment of extremity fractures. The increased survival rate of these patients has resulted in a greater emphasis on minimizing dysfunction and disability, especially that due to concomitant orthopaedic trauma. Advances in anesthestic technique permit earlier operative fixation of extremit
