Buprenorphine maintenance therapy hinders acute pain management in trauma.

Buprenorphine is a mixed opiate receptor agonist-antagonist growing in popularity as an office-based treatment for opioid-dependent patients … and through the early course of admission. Discontinuation of buprenorphine allowed for appropriate pain management and successful analgesia. Further education of acute care clinicians about buprenorphine pharmacology and careful selection of patients for buprenorphine maintenance therapy are needed to avoid delays of pain control in trauma patients.

Psychosocial treatments in pain management of sickle cell disease.

The principal symptom of sickle cell disease (SCD) is pain. Many studies have been conducted on pain management strategies for this illness. There is recognition that psychosocial factors influence clinical disease outcomes; therefore, more attention is being provided to behavioral interventions that address psychosocial problems. This review examines the psychosocial interventions that have been researched for children and adults with SCD, the limitations of these studies, and barriers to implementing the treatments. The intervention receiving the most empirical support was cognitive-behavior…

Malpractice claims associated with medication management for chronic pain.

BACKGROUND: Medication management is an integral part of chronic pain management. Prompted by an increase in the role of medication management in anesthesia chronic pain liability, we investigated the characteristics of malpractice claims collected from 2005 to 2008. METHODS: After Institutional Review Board approval, we compared medication management claims with other chronic pain claims from the American Society of Anesthesiologists Closed Claims Database of 8,954 claims. Claims for death underwent in-depth analysis. RESULTS: Medication management represented 17% of 295 chronic non-cancer …

Current issues in sickle cell pain and its management.

Pain is the insignia of sickle cell disease and the acute painful crisis is the number-one cause of hospital admissions. Tissue damage due to vaso-occlusion releases numerous inflammatory mediators that initiate the transmission of painful stimuli and the perception of pain. The acute sickle cell painful crisis evolves along four distinct phases coupled with changes in certain markers of the disease. Hospital readmission within 1 week occurs in about 16% of discharged patients. Failure to treat acute pain aggressively may lead to chronic pain syndrome. Management of sickle pain is primarily ph

Complicated pain management in a CYP450 2D6 poor metabolizer.

We describe the case of a patient with significant adverse effects from posttraumatic analgesic therapy with opioid analgesics who was found by microarray analysis to have a CYP2D6 genotype predictive of a poor metabolizer phenotype. In addition to her poor tolerance and limited response to opioid analgesics, she developed further discomfort when the antiemetic promethazine was administered to treat her gastrointestinal adverse effects. In our discussion we review the literature about the clinical impact of CYP450 2D6 polymorphisms in treatment with the commonly used opioid analgesics codeine,

Chiropractic management of musculoskeletal pain secondary to tardive dyskinesia.

Tardive dyskinesia (TD) is an umbrella term typically used to describe a movement disorder associated with the use of neuroleptic (anti-psychotic) medication. It is characterized by abnormal, repetitive and involuntary movements. The movements may be around the mouth and face (orofacial dyskinesia) and less frequently, in the trunk and limbs (trunk and limb dyskinesia). TD occurs in over 20% of those using neuroleptic medication continually for longer than three months. A case report is presented of a patient affected by TD who suffered mechanical musculoskeletal pain secondary to its effects,

Chronic scrotal pain syndrome: management among urologists in Switzerland.

INTRODUCTION AND OBJECTIVES: Management of patients presenting with chronic or recurrent pain located in the scrotum is often very challenging. Evidence-based literature and clinical practice guidelines for the management of chronic scrotal pain syndrome (CSPS) are not available. We assessed the current perception and management of chronic scrotal pain syndrome by urologists in Switzerland. METHODS: In July 2004, all the members of the Swiss Society of Urology received a questionnaire focusing on diagnostic and treatment practices for the management of chronic scrotal pain syndrome. The quest

Management of lower back pain in young Turkish recruits.

The aims of this study were to determine the appropriate management strategy for lower back pain in the young male population of the Turkish Army and to analyze the results of surgical treatment of such a common health problem versus medical treatment. Participants in the study included 865 recruits who underwent basic military training between June 2001 and June 2002. All were admitted to our department with the complaint of lower back pain and were evaluated by neurological and radiological examinations. We operated on 23 of them and followed-up by 3 months. We compared the data of surgicall

Pharmacologic management part 1: better-studied neuropathic pain diseases.

Neuropathic pain impacts millions of people in the United States and around the world. Patients experience one of many symptoms, such as pain, paresthesia, dysesthesia, hyperalgesia, and allodynia, for many years because of unavailable or inadequate treatment. One of the major challenges in treating patients with neuropathic pain syndromes is a lack of consensus concerning the appropriate first-line treatment options for conditions associated with neuropathic pain, including postherpetic neuralgia, diabetic peripheral neuropathy, and trigeminal neuralgia. This review summarizes the published r

[Thoracic pain of oesophageal origin. Diagnostic management and treatment].

FROM AN ETIOLOGICAL POINT OF VIEW: Thoracic pain is a frequent symptom. Before confirming the oesophageal origin of the pain, a coronary disease must be excluded. Two principle causes are source of thoracic pain of oesophageal origin: gastro-oesophageal reflux disease and oesophageal motility abnormalities. THE DIAGNOSTIC APPROACH: This must include the questioning of the patient and the usual paraclinical examinations. To confirm the diagnosis, these examinations must establish a chronological relationship between the symptoms and the abnormalities. For economic reasons, following a normal ga

Hemiplegic shoulder pain: defining the problem and its management.

PURPOSE: Hemiplegic shoulder pain can affect up to 70% of stroke patients and can have an adverse impact on rehabilitation outcomes. This article aims to review the literature on the suggested causes of hemiplegic shoulder pain and the therapeutic techniques that can be used to prevent or treat it. On the basis of this review, the components of an optimal management programme for hemiplegic shoulder pain are explored. METHOD: English language articles in the CINAHL and MEDLINE databases between 1990 and 2000 were reviewed. These were supplemented by citation tracking and manual searches. RES

Postoperative pain management in infants and children: new developments.

The elimination of pain should be of outstanding importance for all people caring for children. A concept of balanced analgesia including non-steroidal anti-inflammatory drugs, opioids and local anaesthetics is widely accepted. This review focuses on extending analgesia beyond the immediate postoperative period, the understanding of pharmacokinetic-pharmacodynamic interactions of paracetamol, the side-effects of opioids during patient-controlled administration, and the position of ropivacaine in paediatric pain management. For the majority of clinical situations, however, we already have estab…

Management of complicated gastroesophageal reflux disease: atypical chest pain.

Although most patients with gastroesophageal reflux disease (GERD) present with the classic symptoms of heartburn and acid regurgitation, many complain of atypical chest pain suggestive of cardiac disease. Once cardiac ischemia has been excluded, it is important to consider GERD because this may be established as the cause of pain in 10% to 50% of such patients. If GERD is suspected or documented, vigorous antireflux treatment, preferably with proton pump inhibitory therapy, is indicated.

Laparoscopic management of testicular pain after embolotherapy of varicocele.

We report a patient who underwent embolization of a varicocele for chronic testicular pain with Gianturco coils and developed increased bilateral pain. Complete pain relief was achieved by laparoscopic resection of both spermatic cords. The case provides insights into the pathophysiology of testicular pain and may suggest alternative therapies for chronic orchialgia.

Clinical management of chronic pain in spinal cord injury.

Spinal cord injury (SCI) can cause paralysis; sensory impairment; autonomic nervous system dysfunction; and bowel, bladder, and sexual dysfunction. These impairments may lead to immobility, physical dependence, and alterations in lifestyle and self-esteem. The addition of chronic, intractable pain to these impairments can be truly devastating. Chronic pain superimposed on spinal cord injury can virtually drain the individual of strength, motivation, and will. For the spinal cord injury survivor who already faces functional loss, severe pain can further restrict even the diversional activities

Nonsurgical management of pain secondary to peripheral nerve injuries.

Physicians and surgeons dealing with peripheral nerve injuries have long been plagued by the chronic intractable pain which often follows these injuries. Early recognition of the associated problems and prompt intervention by a multidisciplinary team approach to the pain is essential. Although the techniques reviewed here were presented as separate choices, the treatment of the patient should employ a combined approach of good surgical repair, positive encouragement, non-addicting medications, appropriately timed nerve blocks, aggressive physical and occupational therapy, and electrical stimul

Historical perspective of causalgia. Management of sympathetically maintained pain.

The plethora of terms included in the broad category of sympathetic dystrophies, and causalgia in particular, has made specific disorders, with unique clinical characteristics, very difficult to isolate into discrete clinical entities. Rather, the sympathetic dystrophies currently are regarded as existing along a continuum of varying severity and as having one basic pathophysiological mechanism, with considerable overlap of terms. The purposes of this article are 1) to review the theories of physiological mechanisms of causalgia and other forms of sympathetically maintained pain, 2) to describ

Management of diskogenic pain using epidural and intrathecal steroids.

The use of methylprednisolone acetate (Depo-Medrol) injected by the epidural or intrathecal route for the relief of diskogenic back pain with or without radiculopathy is an adjunct to conservative management useful when conservative measures fail and surgical treatment is under consideration. This is especially true when symptoms have been present for only a few months. Corticosteroids injected in the same manner seem to have little effect on patients with symptoms persisting for periods longer than 3 months or in patients treated previously by surgical methods.
