An 82-year-old man presented with a two-week history of three painful, inflamed nodules on his lower extremities with symmetric arthritis of multiple joints. He was under the care of hospice for end-stage acinar cell carcinoma of the pancreas. His serum amylase and lipase levels were markedly elevated. An incisional biopsy revealed lobular inflammation of subcutaneous fat, focal fat necrosis with saponification/ghost cells and scattered foreign-body type giant cells consistent with pancreatic fat necrosis/pancreatic panniculitis. This is hypothesized to be initiated by autodigestion of subcuta…

The authors studied the effect of ligation of the femoral and saphenous veins on the evolution of lipase injection-induced subcutaneous fibrosis and inflammation in the rats' hindlimbs. The superficial muscular fascia was thickened and the number of veins was increased 3 days and 3 weeks after vein ligation; both abnormalities disappeared 6 weeks postoperatively. Vein ligation did not quantitatively affect fascial thickening or fibrosing panniculitis in hindlimbs injected with lipase 6 weeks prior to sacrifice. The results contradict the proposition that lipase-induced injury of rats' subcutan…

Subcutaneous panniculitis-like T cell lymphoma (SPTCL) is a very rare cutaneous T cell lymphoma that has been reported to be associated with autoimmune disorders but is most commonly associated with systemic lupus erythematosus. We herein report a 26-year-old man thought to have lupus panniculitis (LP) treated for 10 years with corticosteroids and cyclosporine. After several relapses with panniculitis, he was finally diagnosed with SPTCL, which was confirmed to have a HAVCR2 mutation for p.Tyr82Cys. We emphasize that rheumatologists should be aware of the possibility of SPTCL, despite its rare…

AIMS: Both lupus panniculitis (LP) and subcutaneous panniculitis-like T-cell lymphoma (SPTCL) are characterized by subcutaneous lobular lymphocytic infiltrates, and they are sometimes difficult to differentiate. Recently, plasmacytoid dendritic cells (PDCs) were found to be present in various types of cutaneous lupus erythematosus lesions, including LP, and are supposed to play important pathogenetic roles. The aim of this study was to investigate whether PDCs are differentially present in these two diseases and can be utilized to differentiate them. Conventional histopathological features wer…

Mesenteric panniculitis is an uncommon, chronic inflammatory disorder involving the adipose tissue of the mesentery. It can present with non-specific abdominal pain, fever, or gastrointestinal symptoms, and may occasionally progress to abscess formation. We report a case of a middle-aged patient who presented with intermittent left lower abdominal pain for five days, associated with fever and loose stools. Clinical examination revealed abdominal distension and tenderness in the right iliac fossa and umbilical regions. Imaging suggested inflammatory changes in the jejunal mesentery. Exploratory…

Pancreatic panniculitis is an inflammation of subcutaneous adipose tissue associated with acute or chronic pancreatic diseases. Two-year-old girl presented with pain abdomen, fever, and multiple tender erythematous nodules over legs, abdomen and back for 1 week. Computed tomography scan suggestive of acute necrotizing pancreatitis. Skin biopsy revealed typical features of septal panniculitis. The child improved with supportive treatment with regression of skin nodules. Although rare, physicians must be aware of this condition closely mimicking vasculitis to avoid delay in diagnosis.

Pancreatitis, panniculitis, and polyarthritis (PPP) syndrome is a rare manifestation (prevalence 2%-3%) of underlying pancreatic pathology which is characterized by fat necrosis in subcutaneous tissue and joints. The widely accepted etiological hypothesis is systemic dissemination of pancreatic lipase. It has a mortality rate approaching 25%. We present a case of pancreatitis, panniculitis, and polyarthritis syndrome in a 52-year-old man with chronic pancreatitis complicated by pancreatico-pericardial fistula.

Here, a case of Sputnik-V vaccine-induced panniculitis was reported. The patient developed erythema, induration, and local tenderness at the injection site after 13 days of the injection. Ultra-sonography imaging showed inflammation in subcutaneous layers including fat tissue compatible with panniculitis. She received ibuprofen and warm compress, and all symptoms resolved.

Alpha-1 antitrypsin deficiency, although one of the most common genetic diseases, is a very rare and often undiagnosed cause of panniculitis. The authors present a case characterized by an acute involvement of several areas in the thorax, abdomen, and limbs, occurring after repetitive trauma of the perineal area caused by a long period of cycling. After performing the differential diagnosis and establishing etiology, the patient was started on augmentation therapy with plasma-derived synthetic human alpha-1 proteinase inhibitor and the disease has been under control since then. We recommend li…

Palisading granulomatous dermatitis and panniculitis is recognized in various cutaneous inflammatory lesions secondary to presumed collagen damage. Cutaneous nodules with a palisading arrangement of histiocytes surrounding foci of collagen degeneration have been clinically termed palisading granuloma in dogs. Study aims were to characterize the cellular infiltrate of canine palisading granuloma and document salient clinical features. Inclusion criteria were met for 36 dogs and encompassed nodular dermal and subcutaneous histiocyte-predominant cellular infiltrates targeting and enveloping colla…
