[Treating black hypertensive patients: limited applicability of the guideline recommendations].

Self-identified black patients respond better to calcium channel blockers and diuretics, than to renin-angiotensin-system inhibiting agents. This has been translated into sensitive guideline recommendations to treat black patients differently than others. We argue that such recommendations have limited applicability. Studies that shaped these recommendations selected patients on the basis that they self-identify as Black. This self-identification is often considered synonymous to having an African ancestry, but ancestry is but one of the many factors that constitutes one's self-identification.

Oesophageal manometry and video-radiology in patients with systemic sclerosis: a retrospective study of its clinical value.

The aim of this retrospective study was to evaluate the initial video-radiology and manometry in 40 consecutive patients with systemic sclerosis (n=21) or suspected systemic sclerosis (n = 19) in relation to oesophageal symptoms; and, furthermore, to evaluate the consequence of radiographic or manometric findings in the oesophagus on diagnosis and treatment. Evaluating oesophageal abnormalities in relation to diagnosis and treatment has, to our knowledge, not been reported before. Video-radiology together with manometry demonstrated oesophageal dysfunction in 80% of patients. Thirteen patients

Development of Artificial Intelligence-Supported Automatic Three-Dimensional Surface Cephalometry.

OBJECTIVE: Surface-based three-dimensional (3D) cephalometry provides detailed clinical information for the analysis of craniofacial structures. This study aimed to develop an automated 3D surface cephalometry system using mesh fitting based on landmarks identified by artificial intelligence (AI) and to evaluate its accuracy. METHODS: A total of 185 CBCT images from adult Japanese patients (system training, n = 152; evaluation, n = 33) were used in this study. Cranial and mandibular images were generated via surface rendering of CBCT images. An experienced orthodontist manually recognised 19

Acute respiratory distress syndrome: causes, pathophysiology, and phenotypes.

Acute respiratory distress syndrome (ARDS) is a common clinical syndrome of acute respiratory failure as a result of diffuse lung inflammation and oedema. ARDS can be precipitated by a variety of causes. The pathophysiology of ARDS is complex and involves the activation and dysregulation of multiple overlapping and interacting pathways of injury, inflammation, and coagulation, both in the lung and systemically. Mechanical ventilation can contribute to a cycle of lung injury and inflammation. Resolution of inflammation is a coordinated process that requires downregulation of proinflammatory pat

Impaired Odor Perception in Autism Spectrum Disorder Is Associated with Decreased Activity in Olfactory Cortex.

Autism Spectrum Disorders (ASDs) are characterized by atypical sensory functioning in the visual, tactile, and auditory systems. Although less explored, olfactory changes have been reported in ASD patients. To explore these changes on a neural level, 18 adults with ASD and 18 healthy neurotypical controls were examined in a 2-phase study. Participants were first tested for odor threshold and odor identification. Then, (i) structural magnetic resonance (MR) images of the olfactory bulb were acquired, and (ii) a functional MR imaging olfaction study was conducted. ASD patients exhibited decrease

Science, medicine and the future: healing chronic wounds.

Greater interest in wound healing is needed to ensure higher standards of basic care. Precise identification of the systemic, local, and molecular factors underlying the wound healing problem in individual patients should allow better tailored treatment. Allogeneic skin grafting and bioengineered skin equivalents are being used successfully in patients with venous leg ulcers and diabetic patients with foot ulcers.

Reducing time to identification of positive blood cultures with MALDI-TOF MS analysis after a 5-h subculture.

Speeding up the turn-around time of positive blood culture identifications is essential in order to optimize the treatment of septic patients. Several sample preparation techniques have been developed allowing direct matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) identification of positive blood cultures. Yet, the hands-on time restrains their routine workflow. In this study, we evaluated an approach whereby MALDI-TOF MS identification without any additional steps was carried out on short subcultured colonies from positive blood bottles with the obj

Bilateral astrocytoma involving the limbic system precipitating disabling amnesia and seizures.

Astrocytomas involving the limbic system are usually unilateral in nature. We report a very unusual case where a low-grade astrocytoma originating in the left temporal lobe spread to the right hippocampus through the hippocampal commissure to cause disabling amnesia and seizures. Some improvement in the memory deficit was facilitated by identification of complex partial status epilepticus. EEG should be performed in all patients with lesions of the limbic system and neuropsychological problems if ongoing seizure activity is not to be missed.

Intraoperative Parathyroid Autofluorescence Detection in Patients with Primary Hyperparathyroidism.

BACKGROUND: Intrinsic near-infrared (NIR) autofluorescence of the parathyroid gland may improve intraoperative gland identification without the need for contrast agent injection. Compared with patients undergoing surgery for thyroid disease, identification of pathologic parathyroid tissue in patients with hyperparathyroidism is essential. This study analyzed the utility of a novel real-time autofluorescence imaging system in patients with primary hyperparathyroidism enrolled in a prospective feasibility clinical trial. METHODS: Data on patients undergoing surgery for primary hyperparathyroidi

Assays for Sm and RNP antibodies: pitfalls and technical considerations.

Sm antibodies are a specific serum marker for the diagnosis of systemic lupus erythematosus (SLE), and a high titer of RNP antibodies in the absence of other antinuclear antibodies (ANA) is highly suggestive of the diagnosis of mixed connective tissue disease (MCTD); therefore, specificity is a very important aspect of the assays for these antibodies. Currently in the clinical laboratory, double immunodiffusion (ID) is the most specific of the assays for anti-Sm and RNP. When 74 sera from patients with various systemic rheumatic diseases were assayed by counterimmunoelectrophoresis (CIE) with

Utilizing Patient-Derived Organoids in the Management of Colorectal Cancer with Peritoneal Metastases: A Review of Current Literature.

INTRODUCTION: Treatment of colorectal cancer-derived peritoneal carcinomatosis (CRC-PC) is challenging due to cellular heterogeneity that exhibits variable degrees of resistance to systemic as well as intraperitoneal chemotherapy. Therefore, it is not a surprise that the majority of patients undergoing cytoreductive surgery with HIPEC will experience recurrence. Patient-derived tumor organoids (PTOs) may be potentially capable of informing clinical treatment decisions at the level of the individual patient. In this study, we review the current landscape of CRC-PC PTO literature. METHODS: PubM

Immunosuppressed patient with suspected systemic Malassezia infection: A case report.

Fungal infections are a significant cause of systemic infections in immunosuppressed patients. Studies indicate that the most prevalent fungal pathogens include Candida albicans (25 %), non-albicans Candida species (25 %), and Aspergillus species (25 %) [1]. Malassezia, a lipophilic, opportunistic pathogenic yeast, primarily causes skin infections, but there are increasing reports of invasive infections. This report describes a Suspected systemic multisystem infection with Malassezia in a bone marrow transplant patient, detailing the diagnosis and treatment process. The article also discusses

Solitary Angiokeratoma: Report of Two Uncommon Cases.

Angiokeratomas are rare benign vascular skin lesions arising in isolation or in groups of multiple lesions, as solitary cutaneous forms or generalized systemic forms. They are ectasias of dermal capillaries with an acanthotic and hyperkeratotic epidermis. They can occur in both healthy individuals and in those with underlying systemic disease due to inherited enzyme deficiency or other acquired predisposing factors. The identification and reporting of these lesions is important as patients with these lesions should be evaluated to rule out underlying pathogenic conditions. We report two rare c

Immunoproteomic Approach of Extracellular Antigens From Paracoccidioides Species Reveals Exclusive B-Cell Epitopes.

Fungi of the Paracoccidioides genus are the etiological agents of paracoccidioidomycosis (PCM), a systemic mycosis restricted to the countries of Latin America. Currently, the Paracoccidioides complex is represented by Paracoccidioides lutzii, Paracoccidioides americana, Paracoccidioides brasiliensis, Paracoccidioides restrepiensis, and Paracoccidioides venezuelensis. Even with advances in techniques used for diagnosing fungal diseases, high rates of false-positive results for PCM are still presented. Additionally, there is no efficient antigen that can be used to follow up the efficiency of p

Management of unilateral temporomandibular joint ankylosis & orthomorphic correction in a patient with Marfan syndrome: A rare case report.

INTRODUCTION: Marfan syndrome (MFS) is a disorder of the connective tissue that is inherited in an autosomal dominant fashion and is caused by mutations in the gene coding for fibrillin-1 (FBN1). This condition commonly affects the skeletal system, pulmonary system, ocular system and the cardiovascular system. Surgical intervention is often needed to correct the various deformities affecting the patients to restore the function, improving the patient's aesthetic appearance and quality of life. PRESENTATION OF CASE: It's a rare case report of a young male patient suffering through unilateral t

Comparative study of complications after primary and revision transsphenoidal endoscopic surgeries.

A preferred treatment for residual/recurrent pituitary adenomas has not been established. The existence of higher complication rates for revision surgeries remains under debate. This study aimed to compare complication rates of primary and revision transsphenoidal endoscopic surgeries and to identify risk factors for complications. Data from 144 primary and 39 revision surgeries were analysed. The surgical complications evaluated were intraoperative and postoperative cerebrospinal fluid (CSF) leaks; meningitis; permanent diabetes insipidus (DI) and hypopituitarism; worsening visual acuity; oph

Reciprocal inhibition becomes facilitation after spinal cord injury: clinical application of a system identification approach.

Alteration in spinal inputs from descending pathways following spinal cord injury (SCI) affects different mechanisms including reciprocal Ia inhibition. However, whether there is a consistent pattern of change in reciprocal inhibition following SCI is uncertain. Typical attempts to evaluate reciprocal inhibition have been restricted to electrophysiological measurements, which may have limited translation to function. Our objective was to address the uncertainty regarding changes in reciprocal inhibition after SCI by quantitatively evaluating reciprocal inhibition of ankle extensors from ankle
