Admission NarxCare Narcotic Scores Are Associated With Increased Odds of Readmission and Prolonged Length of Hospital Stay After Primary Elective Total Knee Arthroplasty.

INTRODUCTION: The association of preoperative narcotic use with postoperative outcomes after primary elective total knee arthroplasty (TKA) has remained poorly characterized. The NarxCare platform analyzes patients' state Prescription Drug Monitoring Program records to assign numerical scores that approximate a patient's overall opioid usage. The present study investigated the utility of admission NarxCare narcotic scores in predicting the odds of adverse events (AEs) after primary elective TKA. METHODS: Elective primary TKA patients performed at a single institution between October 2017 and

Postpartum Hospital Readmissions Among Massachusetts Women Who are Deaf or Hard of Hearing.

Objectives: Deaf or hard of hearing (DHH) women are at a higher risk of adverse pregnancy and birth outcomes compared with other women. However, little is known about postpartum outcomes among DHH women. The objective was to compare the risk of postpartum hospitalizations for DHH compared with non-DHH women and the leading indications for postpartum admissions. Materials and Methods: We analyzed data from the 1998-2017 Massachusetts Pregnancy to Early Life Longitudinal Data System and identified 3,546 singleton deliveries to DHH women and 1,381,439 singleton deliveries to non-DHH women. We use

Fluid Restriction After Transsphenoidal Surgery for the Prevention of Delayed Hyponatremia: A Systematic Review and Meta-Analysis.

OBJECTIVE: Delayed hyponatremia is the primary cause of readmission after transsphenoidal surgery, with a reported incidence of 9% to 30.7%. Studies have failed to identify consistent predictive factors for postoperative hyponatremia; thus, it is difficult to determine patients that are at a high risk. Fluid restriction is one approach for the prevention of hyponatremia. We have performed a meta-analysis and systematic review of the literature to evaluate the impact of fluid restriction on hyponatremia and hospital readmissions. METHODS: Ovid EMBASE, PubMed, Scopus, and Cochrane were searched

Using machine learning to reduce unnecessary rehospitalization of cardiovascular patients in Saudi Arabia.

OBJECTIVES: Patient readmission is a costly and preventable burden on healthcare systems. The main objective of this study was to develop a machine-learning classification model to identify cardiovascular patients with a high risk of readmission. METHODS: Inpatient data were collected from 48 Ministry of Health hospitals (MOH) in Saudi Arabia from 2016 to 2019. Cardiovascular disease (CVD)-related diagnoses were defined as congestive heart failure (HF), ischemic heart disease (IHD), cardiac arrhythmias (CA), and valvular diseases (VD). Hospitalization days, daily hospitalization price, and th…

Incidence and predictors of hospital readmission in children presenting with severe anaemia in Uganda and Malawi: a secondary analysis of TRACT trial data.

BACKGROUND: Severe anaemia (haemoglobin < 6 g/dL) is a leading cause of recurrent hospitalisation in African children. We investigated predictors of readmission in children hospitalised with severe anaemia in the TRACT trial (ISRCTN84086586) in order to identify potential future interventions. METHODS: Secondary analyses of the trial examined 3894 children from Uganda and Malawi surviving a hospital episode of severe anaemia. Predictors of all-cause readmission within 180 days of discharge were identified using multivariable regression with death as a competing risk. Groups of children with s

Readmissions After Bariatric Surgery in France, 2013-2016: a Nationwide Study on Administrative Data.

BACKGROUND: Readmission rate is considered an indicator of quality of care, which is already used in some countries to impose financial penalties on hospital with readmissions in excess of the national average. Nevertheless, this indicator presents some controversial drawbacks. The objective of this study was the assessment of readmission rate after bariatric surgery. METHODS: This is a retrospective observational study on a national administrative claims database, the Information Systems Medicalization Program, PMSI, which is a nationwide billing tool collecting information on all hospital d

Index admission cholecystectomy for acute biliary pancreatitis favorably impacts outcomes of hospitalization in cirrhosis.

BACKGROUND AND AIM: Despite higher rates of gallstones in patients with cirrhosis, there are no population-based studies evaluating outcomes of acute biliary pancreatitis (ABP). Therefore, we sought to evaluate the predictors of early readmission and mortality in this high-risk population. METHODS: We utilized the Nationwide Readmission Database (2011-2014) to evaluate all adults admitted with ABP. Multivariable logistic regression models were used to assess independent predictors for 30-day readmission, index admission mortality, and calendar year mortality. RESULTS: Among 184 611 index adm

Wearable-derived postdischarge mobility as a digital biomarker for 90-day readmission and mortality in metastatic cancer.

PURPOSE: The postdischarge period after unplanned hospitalization is a vulnerable transition for patients with metastatic cancer, often accompanied by functional decline, symptom burden, psychological distress, early readmission, and death. However, short-term risk assessment during this period still relies largely on episodic clinician-rated measures. We evaluated whether wearable-derived postdischarge step counts are associated with 90-day readmission, mortality, and patient-reported outcomes in patients with metastatic cancer. METHODS: In this prospective, two-center cohort study, adults w

National Trends in 30-Day Readmission Following Transoral Robotic Surgery for Oropharyngeal Squamous Cell Carcinoma.

IMPORTANCE: As the incidence of oropharyngeal squamous cell carcinoma (OPSCC) continues to rise in the US, an increasing number of patients are being treated with transoral robotic surgery (TORS). Readmission following surgery can potentially delay initiation of adjuvant treatment and affect survival outcomes. OBJECTIVE: To identify risk factors for 30-day postoperative readmission in patients undergoing TORS for OPSCC. DESIGN, SETTING, AND PARTICIPANTS: This retrospective, population-based cohort study used data from the Nationwide Readmissions Database from 2010 to 2017. All patients under
