Surgical repair of pectus excavatum and carinatum.

The author discusses different forms of pectus deformities and presents appropriate surgical methods he developed for their correction. For pectus excavatum, the surgical technique includes conservative sub-perichondral resection of deformed costal cartilages and detachment of the xiphoid process. A transverse sternotomy is performed at the upper level of the deformed sternum, which is then bent forward. The corrected sternal position is secured by a "hammock" of synthetic mesh, spread behind the sternum, and attached to the respective cartilage remnants. The pectoralis muscles are then united…

[Poland's syndrome].

Poland's syndrome consists of the variable clinical features, but always includes unilateral aplasia of the chest wall muscles and ipsilateral anomalies of upper extremity. The incidence of Poland's syndrome, reported by different authors ranges from 1:10,000 to 1:100,000 and is observed more frequently in males than in females with the right side of the body affected more often than the left. The etiology of this syndrome is still discussed. However most of described cases were sporadic, rare familial incidence of Poland's syndrome were also presented. Therefore different etiologic factors of

Tendon transfers to restore elbow flexion after traumatic paralysis of the brachial plexus in adults.

Loss of elbow flexion after traumatic brachial palsy produces significant functional and cosmetic problems. Although a direct approach to the neurological lesion with an attempt to reinnervate the biceps has given some encouraging results, these can be incomplete and for this reason tendon transfers still have an important role. We report the results of our series of 60 patients (54 men and 6 women) who have undergone tendon transfer between 1984 and 1994. The transfers were performed during or after nerve surgery, and we used the muscles arising from the medial epicondyle, the pectoralis mino

Impact of various types of heat treatment on the content of selected B vitamin and their profile in goose breast meat.

The objective of this study was to (1) investigate the impact of commonly used heat treatment methods - water bath cooking (WBC), convection oven roasting (OCR), grilling (G), and pan frying (PF) - on the concentrations and retention of various forms of selected B vitamins (B1, B2, B3, B5, B6, and B12) in goose breast muscle, and (2) assess the extent to which a 100 g portion of goose meat, both with and without skin, contributes to meeting the Nutrient Reference Values-Requirements (NRV-R) for these vitamins in the adults. The material used in the study comprised 36 breast muscles of 17-week-

Anatomical Basis of a Posterior Intralamellar Plane in Breast Tissue-Preservation Surgery.

Breast tissue-preservation surgery relies on the identification of anatomical dissection planes that allow surgical access while minimizing disruption of glandular and fascial structures. Although a plane between the mammary gland and the pectoralis major fascia is increasingly utilized in aesthetic procedures, its anatomical basis remains incompletely defined. The aim of this study was to describe the anatomical and histological foundations of a posterior intralamellar dissection plane located between the corpus mammae and the pectoralis major fascia and to characterize its relevance for brea

Molecular and gene expression analyses of chicken oncomodulin and their association with breast myopathies in broilers.

Oncomodulins (OCMs), also known as non-α-parvalbumins, are small molecules known for their high-affinity binding of Ca2+ ions. They play crucial roles as Ca2+ buffers and participate in signaling pathways within muscle and neuron cells. In chickens, 3 oncomodulin molecules have been identified at the protein level and are named chicken oncomodulin 1 (OCM1), -3 (OCM3), and alpha-parvalbumin (PVALB). OCM4 was newly assigned by genome annotation. A gene cluster containing OCM1, OCM3, and OCM4 is located in chromosome 14, while a single gene of PVALB is on chromosome 1. The Ca2+ signaling pathway

Distal positioning of the skin paddle of pectoralis major myocutaneous flap in head and neck reconstruction.

BACKGROUND: To assess outcomes of pectoralis major myocutaneous flap (PMF) wherein the skin paddle (SP) was positioned with its distal portion extending beyond the lower border of pectoralis major by ≥2 cm (PMF-d). METHODS: Consecutive head and neck reconstructions with PMF-d (n = 110). SP dimensions l2 (distal extent below the lower border of pectoralis major), l1 (proximal extent above lower border of pectoralis major), and b (breadth) were recorded. Endpoints were SP necrosis, recipient dehiscence, early fistula, and persistent fistula. RESULTS: Median values of l2 , l1 , and b were 3.0,

Complex Shoulder Girdle Stabilization Using Allograft Capsular Reconstruction and Pectoralis Major Transfer: A Case Report.

CASE: A 21-year-old woman with a history of multiple failed surgical and conservative management for recurrent involuntary shoulder instability presented with 1-year history of shoulder pain. Physical examination demonstrated intractable static anterior glenohumeral instability, deficient capsular tissue, with reducible scapular winging secondary to long thoracic nerve palsy. Allograft capsular reconstruction and open split pectoralis major tendon transfer were performed to salvage shoulder motion and stabilize her shoulder girdle. CONCLUSION: This report presents a novel repair option for in

Pedicled pectoralis major flap in head and neck reconstruction - our experience.

INTRODUCTION: The pedicled pectoralis major flaps are still harvested and mainly indicated for reconstruction in the polymorbid patient. Other indications are combinations of pedicled pectoralis major flaps with free microvascular flap, salvage reconstruction following complications, free flap failure and recurrent or extended primary disease. MATERIALS AND METHODS: We describe the pedicled pectoralis major flap in 18 patients operated on at the Department of Oral and Maxillofacial Surgery, Olomouc from 1st January 2014 to 1st December 2016. RESULTS: Fifteen oropharyngeal defect reconstructi

Concomitant aortic root and pectus deformity repair in Marfan syndrome patients.

The role of concomitant aortic and pectus repair in Marfan patients remains controversial. We present our surgical technique for concomitant aortic repair of aortic root pathology and pectus correction. The concomitant surgery can be safely achieved in Marfan patients, thus, avoiding the need for a risky second stage operation.

Pectoralis major tendon rupture in a weight lifter: a rare case.

Pectoralis major tendon (PMT) rupture is a rare event, and it was originally described by Patisser in 1822. The PMT is a thick lamina, which has two origins, one from the clavicle and the other from the ribcage sternum. PMT lesions are classified according to 3 levels of severity: Type 1 is contusions and strains, Type 2 is a partial tear and Type 3 is a complete tear. Diagnosis is made through a clinical examination which comprises a positive response to pain upon pressure over the axilla recess, the presence of bruising and limited motion, and is completed with an MRI. Type 3 injuries are tr

Pectoralis major myocutaneous flap in the reconstructive surgery of the head and neck--our experience.

The goal of this retrospective study is the evaluation of pectoralis major myocutaneous pedicle flap (PMMPF) reliability in clinical practice based on the analysis of the leading indication and postoperative complications. In the period from 2005 to 2011 at the University Department of Otorhinolaryngology and Head and Neck Surgery, Zagreb University Hospital Center, a total number of 27 PMMPF were used in the treatment of 26 patients. Indications for flap use were upper aerodigestive tract and soft tissue defects following resection of head and neck cancer. One-stage reconstructive technique w

Counterclockwise exit of cardiac pacemaker leads: sign of pulse-generator flip.

Clockwise exit of pacing leads from transvenous cardiac pacemakers is generally essential for proper function. The authors report a case in which counterclockwise exit of the pacing leads signified a 180 degree flip of the pulse generator. Three types of pacing system malfunction can result from pacemaker flip.

[Value of the pectoralis major musculocutaneous flap in the repair of the posterior wall of the pharynx and the lateral wall of the hypopharynx].

Two further indications for the use of a musculocutaneous flap from pectoralis major are reported: mucosal reconstruction after posterior pharyngectomy and vertical hemi-laryngopharyngectomy. This method was used in 7 cases, after previous surgery in 6 of these patients. Caution is necessary when interpreting carcinologic results because of the lack of long-term follow up results, but postoperative course was uncomplicated and tube removal and subnormal feeding was possible within 3 to 6 weeks.

[Poland's syndrome: presentation of two cases diagnosed during the neonatal period (author's transl)].

Two cases of Poland syndrome, diagnosed during the neonatal period, are reported. The children exhibited the following malformations: absence of the costosternal head of the pectoralis major, brachysyndactyly, hypoplasia of the upper limb. There were also other associated anomalies such as: Sprengel deformity in one case, hypoplasia of the lower limb and absence of a toe on the same site, in the other case. The authors point out the possibility of associating other multiple abnormalities and they examine the various pathogenic hypothesis presented in the literature.

Reconstruction of congenital amastia with complication.

Subcutaneous insertion of a gel-filled Silastic prosthesis to reconstruct a congenitally absent right breast was complicated by deterioration in skin circulation that necessitated removal of the implant approximately three hours postoperatively. No complications ensued when the same procedure was carried out three months later. Attention is drawn to the risk of interfering with skin circulation in patients with the rare anomaly of congenital absence of breast and nipple. The literature on this subject is reviewed. The author suggests that subpectoral implant may be a safer alternative in such
