Twenty-six pleural biopsies were performed on 23 patients over a 3-year period. Twenty-three biopsies were performed guided with ultrasound; one, with computed tomography; and two, with fluoroscopy. We describe the radiographic, gross morphologic and microscopic appearance of a pleural collagenous nodule presenting as a "solitary nodule." We believe that this lesion is within the spectrum of "pleural plaques," an associated condition present in our patient who had a history of potential asbestos exposure. Awareness of this probably rare variant of a common abnormality may obviate the need for a diagnostic thoracotomy in certain circumstances.

Seventeen patients (ten male and seven female) between the ages of 21 and 76 years underwent thoracoscopy from 1976 through 1978 by the Stanford University University Thoracic Surgical Service. IFN gamma levels and IFN gamma production capability were studied. The results showed that IFN gamma levels, IFN gamma production in the tuberculous pleurisy (n = 30) were significantly higher than those in the malignant pleural effusion (n = 23) and the pleural transudate. However, there was no detectable IFN gamma in the blood, IFN gamma production were lower in the blood than in the pleural effusion. The finding suggests that IFN gamma may play an important role in local anti-mycobacterial infection.

In this article, we evaluated the usefulness of pleural fluid lactic dehydrogenase and isoenzymes in differential diagnosis of pleurisy. Lactic dehydrogenase activities and isoenzymes in pleural fluid were determined in twenty-five patients with tuberculosis and twenty-five patients with malignancy. The results showed that both lactic dehydrogenase level and isoenzymes in pleural fluid between two groups did not correlate significantly (P greater than 0.05).
