Immunogenicity and induction of immunological memory of the heptavalent pneumococcal conjugate vaccine in preterm UK infants.

Data on the immunogenicity and memory induction of pneumococcal conjugate vaccines in very preterm infants is limited. We vaccinated 69 full term and 68 preterm infants (median gestational age (GA) 30 weeks) with a 7-valent pneumococcal conjugate vaccine (PCV7) at 2/3/4 months of age, followed by a plain polysaccharide booster at 12 months of age. IgG-GMC (ELISA) was significantly lower in preterm infants to six vaccine serotypes (ST) at 2 months and 5 months of age, to five ST at 12 months of age and to three ST at 13 months of age. A significantly lower proportion of preterm infants achieved

Penicillin-resistant pneumococcal meningitis: high antibiotic exposure impedes new vaccine protection.

The frequency of meningitis due to penicillin-resistant Streptococcus pneumoniae (PRP) has increased in recent years, making treatment failure more likely. It is currently expected that pneumococcal conjugate vaccines might curb this trend. We investigated this issue using a mathematical model applied to the current prevalence of resistance and antibiotic exposure in the United States and in France. Our main finding was that the level of antibiotic exposure may limit the effect of the vaccine. In relatively low antibiotic exposure environments such as the United States, large-scale vaccination

Characteristics of long-term-care facility residents associated with receipt of influenza and pneumococcal vaccinations.

BACKGROUND: Studies have found residency in long-term-care facilities (LTCFs) a risk factor for influenza and pneumonia and have demonstrated that vaccinations against these diseases reduce the risk of disease. However, rates are below Healthy People 2010 goals of 90% for LTCFs. During 1999-2002, a multi-state demonstration project was conducted in LTCFs to implement standing orders programs for immunizations. OBJECTIVE: Identify nursing home resident-specific characteristics associated with vaccination coverage at baseline. METHODS: Facility-level data were collected from self-reported surv

Updated recommendations on the use of pneumococcal conjugate vaccine: suspension of recommendation for third and fourth dose.

On February 13, 2004, CDC recommended that health-care providers temporarily suspend routine use of the fourth dose of 7-valent pneumococcal conjugate vaccine (PCV7) when vaccinating healthy children. This action was taken to conserve vaccine and minimize the likelihood of shortages until Wyeth Vaccines, the only U.S. supplier of PCV7 (marketed as Prevnar), restores sufficient production capacity to meet the national need. Since that recommendation, PCV7 production has been much less than expected because of continuing problems with the PCV7 vial-filling production line. Shipments have been de

Safety and efficacy of the seven-valent pneumococcal conjugate vaccine: evidence from Northern California.

UNLABELLED: Pneumococcal disease remains a significant cause of morbidity among young children. A large-scale efficacy trial in the Northern California Kaiser Permanente system (the KP trial) demonstrated that a seven-valent conjugate vaccine (PCV) is safe and immunogenic in young children and effective in preventing both invasive pneumococcal disease caused by vaccine serotypes (97.4% efficacy) and episodes of otitis media (7.0% efficacy). Since the publication of the results of the KP trial in 2000, we have performed an additional analysis on the safety, immunogenicity, and efficacy of the v

Differential immune response to influenza and pneumococcal vaccination in immunosuppressed patients after heart transplantation.

INTRODUCTION: Patients after solid organ transplantation are at an increased risk for microbial infections. Due to therapeutic immunosuppression, the response to active immunizations may be reduced. The serological efficacy of pneumococcal and influenza vaccination was studied in heart transplant recipients. PATIENTS AND METHODS: Sixteen patients over 1 year after heart transplantation and control patients were immunized with a 23-valent pneumococcal vaccine and a triple-split influenza vaccine. Pre- and postvaccinal antibody titers were serologically determined, including quantitation of spe

Pneumococcal vaccination in Belgian adults: a practical translation of the 2025 Superior Health Council guidelines.

BACKGROUND: Pneumococcal disease remains a significant burden in Belgian adults, with 2,120 cases of invasive pneumococcal disease (IPD) reported in 2024, the highest in a decade. Despite clear indications, vaccination uptake remains unacceptably low (13-24% in risk groups). Complex sequential vaccination schedules involving pneumococcal conjugate vaccines followed by the 23-valent polysaccharide vaccine (PPV23), combined with limited awareness among healthcare providers and the public, and incomplete reimbursement policies, have contributed to implementation barriers and poor adherence. Disease modulation by TIV vaccination during secondary pneumococcal infections in influenza-infected mice.

Secondary bacterial infections can significantly worsen the clinical course of influenza virus infections and are a leading cause of morbidity and mortality during seasonal influenza epidemics. Despite being a vaccine-preventable disease, influenza-related complications from secondary bacterial infections are an important cause of death, particularly among the elderly population. Streptococcus pneumoniae (Spn) is the most common agent responsible for influenza-related secondary bacterial infections. Influenza virus vaccination serves as an effective prophylactic strategy for preventing influen

Cross-reactive immunity within pneumococcal serogroups: Implications for next-generation vaccine development.

OBJECTIVES: This study aims to investigate within-serogroup cross-reactive immunity among pneumococcal serotypes using inhibition opsonophagocytic assays (OPA). The findings provide immunologic insights into serotype-replacement dynamics with implications for the design of future pneumococcal vaccines. METHODS: Pooled sera were prepared from 24 adults aged ≥40 years: Pool 1 (PCV13/PPSV23 recipients), Pool 2 (PCV20 recipients), and Pool 3 (PCV21 recipients). Cross-reactivity was examined across five major serogroups (6, 9, 15, 19, 23) by measuring residual OPA titers after adsorption with homo…

Identification of age-group reservoirs for persistent vaccine-type pneumococcal carriage in rural Gambia.

BACKGROUND: Although pneumococcal conjugate vaccines (PCVs) have been used widely in many low and middle-income countries, residual vaccine-type (VT) carriage persists in these settings. We examined the role of specific age groups in transmission and as reservoirs of VT pneumococcal infection in The Gambia. METHODS: Between January and November 2022, we conducted a nested, population-based, cross-sectional social contact and pneumococcal carriage survey in the Central and Upper River Regions of The Gambia. Participants completed questionnaires on carriage risk factors and social contacts. Nas

Multisite infection with Austrian syndrome caused by non-vaccine pneumococcal serotype-case report.

BACKGROUND: Although rare, Austrian syndrome remains a life-threatening triad consisting of pneumonia, endocarditis, and meningitis caused by Streptococcus pneumoniae. CASE SUMMARY: We present a case of Austrian syndrome, further complicated by spondylodiscitis and endophthalmitis, in an immunocompromised male receiving methotrexate treatment for rheumatoid arthritis. The infection was caused by a non-vaccine pneumococcal serotype (23B). Susceptibility testing showed reduced susceptibility towards benzylpenicillin, an uncommon finding in Denmark. CONCLUSION: This case illustrates some of the

Changes in composition of respiratory bacteria colonizing pneumococcal conjugate vaccinated children, 2006-2023.

We hypothesized that the introduction of 13-valent pneumococcal conjugate vaccine (PCV13) would alter patterns of nasopharyngeal colonization and their relationship to acute otitis media (AOM) in children. We prospectively followed 1537 children from age 6-36 months across three periods: 2006-2009 (PCV7 era), 2010-2014 (early-PCV13 era), and 2015-2023 (late-PCV13 era), collecting nasopharyngeal cultures at healthy and AOM visits, with paired tympanocentesis when appropriate. Colonized children had more frequent daycare attendance and less breastfeeding. Children with no AOM differed from those

The impact of implementing the 10-valent pneumococcal conjugate vaccine on hospitalizations for pneumonia among children.

Pneumonia is a major cause of morbidity and mortality in children, with pneumococcus as the main etiologic agent. In Brazil, the 10-valent pneumococcal conjugate vaccine (PCV-10) was introduced into the childhood immunization schedule in 2010. The aim of this study was to assess the impact caused by implementing PCV-10 on the hospitalizations of children with pneumonia, between 2005 and 2015, in the state of Pernambuco, Brazil. An ecological time series study and a forecasting analysis were conducted. A comparison was made between the hospitalizations of children aged between seven months and

Cost-effectiveness of the 15-valent pneumococcal conjugate vaccine for high-risk adults in Switzerland.

BACKGROUND: Vaccination against pneumococcal disease (PD) has shown a favorable cost-effectivenessprofile for many national immunization programs. While vaccination efforts have concentrated on children, many adults with underlying illnesses face elevated risks of PD and death. A 15-valent pneumococcal conjugate vaccine (V114) is currently available offering protection against 15 different serotypes and can be used in adults. RESEARCH DESIGN AND METHODS: We examined the cost-effectiveness of V114 vaccination in high-risk adults, aged 18+, in Switzerland. To this end, a Markov model was constr

A case of vaccine-associated myocarditis following pneumococcal immunization leading to acute mitral regurgitation.

Vaccine-associated myocarditis (VAM) is a rare entity but can result in potentially serious sequelae if left untreated. However, the mechanisms of the complications of VAM and its treatment remain unclear. Herein, we report the first case of VAM related to pneumococcal immunization, presenting as a local and systemic inflammatory reaction, in which the patient developed significant secondary mitral regurgitation, resulting in acute heart failure. Finally, the patient recovered completely following corticosteroid treatment. This case highlights the value of cardiac magnetic resonance and the pi
