Unilateral pulmonary artery agenesis is a rare congenital anomaly that is commonly associated with additional cardiovascular abnormalities. We report a case of intrapulmonary hemorrhage and hemoptysis in a patient with left-sided pulmonary artery agenesis as well as the first description of this anomaly in a patient with a diverticulum of Kommerell. The patient was successfully treated by performing a left pneumonectomy.

Thirty-one of 256 cases of chronic destructive pneumonia who were judged, by the severity of illness and haemoptysis, to have lung gangrene, were managed with intravenous sodium cephalothin. Five patients died, two in relation to emergency pneumonectomy. In comparison with results of management of this disease in previous, similar patients, the results are encouraging.

BACKGROUND: Neoadjuvant chemoimmunotherapy has improved resectability in locally advanced non-small cell lung cancer (NSCLC); however, its impact on complex bronchioplastic procedures remains controversial. Bacteremia and meningitis due to Streptococcus pneumoniae developed in a 52-year-old man 28 hours after an otherwise uncomplicated fiberoptic bronchoscopy. The patient responded to antimicrobial therapy and supportive care and later underwent pneumonectomy for carcinoma. This report reinforces previous observations that bacteremia may occasionally be associated with the performance of fiberoptic bronchoscopy.

Thymomas are the most common tumours of the anterior mediastinum, with most cases presenting incidentally. We present a case of extensive pleural thymoma initially thought to be unresectable, which was treated with neoadjuvant chemotherapy and pleuro pneumonectomy. We discuss the multimodality approach to management of this tumour, in particular the use of neoadjuvant chemotherapy prior to surgical resection.

We report two infants who underwent right pneumonectomy in infancy and developed postpneumonectomy syndrome with obstruction of the left main bronchus causing severe airway obstruction in one patient and gastrointestinal reflux due to a displaced and grossly dilated oesophagus in the other patient. Pulmonary gangrene is an extremely rare and severe complication of pneumonia. It is very rarely seen in community-acquired pneumonia. A 49-year-old immunocompetent man was admitted with community-acquired pneumonia caused by Klebsiella pneumonia. His condition rapidly deteriorated with ensuing pulmonary gangrene and septic shock. He was successfully managed by emergency pneumonectomy.

We present a novel case of a patient with nephrotic syndrome and previous left pneumonectomy who had a massive pulmonary embolism of his remnant right pulmonary artery. He underwent surgical embolectomy and veno-arterial extracorporeal membrane oxygenation (ECMO). Early embolectomy using retrograde pulmonary perfusion and post-operative ECMO helped the patient survive this catastrophic event.
