In 6 cases of polymyositis was performed muscle and sural nerve biopsy. All the 6 patients presented typical clinical pictures of simple polymyositis. Sural nerves showed demyelination and axonal changes, under light and electron microscopy. It seems that simple polymyositis accompanied by the alterations in peripheral nerves is not very rare. Sera of 34 patients with progressive systemic sclerosis and of 11 patients with polymyositis/dermatomyositis (PM/DM) were analyzed by the immunoblotting technique for the presence of marker antibodies. A pregnant patient developed fulminant polymyositis with myoglobinuria after terbutaline and magnesium sulfate tocolysis. Her response to prednisone was dramatic. Rapid relapse occurred after inadvertent postcesarean dose reduction. Our patient again responded to increased prednisone. She and her twin babies are well.

Ulcerative ileojejunitis is a rare condition. We describe a patient who developed this on a background of longstanding celiac disease. Long-term remission has been maintained on corticosteroids. Recently polymyositis and an inflammatory alveolitis have developed, both responding to a higher dose of prednisolone. Relationships between celiac disease and ulcerative ileojejunitis, polymyositis and fibrosing alveolitis have been previously described, and it is of interest that an auto-immune pathophysiology has been implicated in each of these conditions.

This report describes an unusual case of pseudotumour of muscle caused by an eosinophilic myositis associated with peripheral blood eosinophilia, systemic illness and a pericardial effusion. The relationship of this case to other reported cases of eosinophilic myositis and the hypereosinophilic syndrome is discussed briefly.

Measurements of maximum respiratory pressures and routine pulmonary function tests were performed in 8 patients with polymyositis (PM) and 2 patients with dermatomyositis (DM). Serial measurements of routine pulmonary function tests in 8 patients remained unchanged. In polymyositis (PM), neither the precise target nor the mechanism of the attack against skeletal muscle have yet been defined. In this study, we report evidence of a pathologic process involving a membrane component of muscle, acetylcholine receptors (AChRs). Our results show that PM patients have significantly reduced AChRs at neuromuscular junctions, averaging 55% below control values (P less than 0.01). The clinical significance of serum aspartate aminotransferase (GOT) isozymes was studied in 18 patients with polymyositis. Abnormally high levels of mitochondrial GOT (mGOT) (6.2 +/- 1.2 IU/L, mean +/- SEM; normal, less than 2.0 IU/L) and cytosol GOT (sGOT) (95 +/- 21.6 IU/L; normal, less than 25 IU/L) were observed in sera. A patient is described who had polymyositis with arthritis, keratodermia blenorrhagica, pulmonary fibrosis, and cardiac failure with a right bundle branch block.
