[Diagnostic accuracy of STOP-Bang questionnaire on moderate sleep apnoea in primary care].

OBJECTIVE: We aimed to compare the diagnostic utility of the STOP-Bang questionnaire for moderate apnoea against the gold standard (type I polysomnography) in a primary care setting. METHOD: Study of diagnostic utility in primary care. Estimated sample: 85 cases and 85 healthy controls. In convenience sampling, 203 patients were recruited by their physicians at six health centres. Twenty-five were excluded, and 57 women and 121 men, of whom 74 had apnoea-hypopnoea index (AHI) ≥15, were analyzed. STOP-Bang was validated by comparing scores in the same patient with the apnoea-hypopnoea index ob…

Rapid Eye Movement Sleep Behavior Disorder and Other Rapid Eye Movement Sleep Parasomnias.

PURPOSE OF REVIEW: The most common rapid eye movement (REM) parasomnia encountered by neurologists is REM sleep behavior disorder (RBD), and nightmares are so frequent that every neurologist should be able to differentiate them from the dream enactment of RBD. Isolated sleep paralysis is relatively common and is often mistaken for other neurologic disorders. This article summarizes the current state of the art in the diagnosis of RBD, discusses the role of specific questionnaires and polysomnography in the diagnosis of RBD, and reviews recent studies on idiopathic RBD as an early feature of a …

Sleep disorders in neuromuscular diseases.

PURPOSE OF REVIEW: Patients with neuromuscular disorders (NMD) are especially vulnerable to sleep-related dysfunction. Sleep-disordered brea … h nocturnal hypoventilation and daytime normocapnia. In contrast, the timing for investigation with polysomnography and for initiation of NIV is debated. SUMMARY: Patients with chronic NMD should be routinely assessed for sleep-disordered breathing and sleep complaints, because these are treatable complications in an otherwise progressive disease process. Furthe …

8. Investigation and treatment of upper-airway obstruction: childhood sleep disorders I.

Always take a history of snoring and sleep disturbance when reviewing children in primary care, as there is evidence that episodes of hypoxi … y (desaturation episodes) are useful screening tests, but oximetry is best used in conjunction with polysomnography. A negative oximetry test does not exclude OSAS. Polysomnography is the best method for detecting and assessing the severity of OSAS in children, and is especially helpful for prioritising treatment and evaluating the risk of perioperative complicat …

Eight years of follow-up--uvulopalatopharyngoplasty combined with midline glossectomy as a treatment for obstructive sleep apnoea syndrome.

In 22 patients with obstructive sleep apnoea, uvulopalatopharyngoplasty was combined with a midline glossectomy of the dorsum of the tongue. The body mass index (BMI) was normal in 13 cases and abnormal in 9. Daytime polysomnography was performed in all cases both preoperatively and 1 year postoperatively. In 16 patients another daytime polysomnography was performed after 8.4 years. The patients also answered a questionnaire 11, 33, 43 and 98 months postoperatively. The apnoea index (AI) was reduced in 82% of cases 12 months postoperatively, being reduced by 50% in 59% of cases and normalized …

Underestimation of nocturnal sleep duration in central disorders of hypersomnolence: an underrecognized feature?

STUDY OBJECTIVES: Sleep misperception is well-documented in insomnia but remains understudied in central disorders of hypersomnolence (CDH). This study aimed to examine (1) total sleep time (TST) misperception in CDH and healthy controls, (2) the prevalence of accurate estimators, underestimators, and overestimators, and (3) the relationship between misperception and polysomnography fragmentation variables. METHODS: We included 420 adults with CDH (38 narcolepsy type 1, 52 narcolepsy type 2, 192 idiopathic hypersomnia, 138 nonspecified hypersomnia) and 86 healthy controls tested in Montreal, …

Modified STOP-Bang questionnaire for detecting obstructive sleep apnea in individuals with a body mass index below 35 kg/m2.

BACKGROUND: Obstructive sleep apnea (OSA) is a common clinical condition. Due to its high prevalence, the waiting list for polysomnography is often long. A screening tool is needed to identify individuals at high risk for OSA who should undergo polysomnography. The STOP-Bang questionnaire is a widely used screening tool; however, it may require modification for individuals with a body mass index (BMI) below 35 kg/m2. This study aimed to evaluate whether the STOP-Bang questionnaire should be modified for patients with a BMI under 35 kg/m2. METHODS: This retrospective analytical study included …

Excessive daytime somnolence in women with abnormal respiratory efforts during sleep.

An abnormal level of respiratory efforts, indicated by significant increase in peak negative esophageal pressure (Pes), can be associated with daytime somnolence in snoring pre-menopausal women. No drop in oxygen saturation (SaO2) or visual evidence of transient electroencephalographic (EEG) arousals can be found at repeat polysomnography. Nasal continuous positive airway pressure (CPAP) titrated on Pes measurements eliminates somnolence.

Recent advances in paediatric sleep disordered breathing.

UNLABELLED: This article reviews the latest evidence pertaining to childhood sleep disordered breathing (SDB), which is associated with negative neurobehavioural, cardiovascular and growth outcomes. Polysomnography is the accepted gold standard for diagnosing SDB but is expensive and limited to specialist centres. Simpler tests such as cardiorespiratory polygraphy and pulse oximetry are probably sufficient for diagnosing obstructive sleep apnoea (OSA) in typically developing children, and new data-processing techniques may improve their accuracy. Adenotonsillectomy is the first-line treatment …

Snoring toddlers with and without obstructive sleep apnoea differed with regard to snoring time, adenoid size and mouth breathing.

AIM: The difficulty of assessing the likelihood of obstructive sleep apnoea (OSA) in children who snore without full-night polysomnography is widely recognised. Our aim was to identify features that were characteristic of two-year-old children with OSA and evaluate whether this information could be used to assess the likelihood of OSA. METHODS: The study was carried out as part of the Child-Sleep Project, a longitudinal birth cohort study of children born at Tampere University Hospital, Finland. This part of the study focused on the children in the cohort who snored and was carried out betwee…
